Sun Simiao discussed: The *Suwen* (Basic Questions) states that when wind pathogen invades the interior of the muscles, the muscles become deficient, and the true qi scatters outward. If it is then combated by cold at the skin, it disperses outward to the interstices (couli), opens the hair follicles, and allows chaotic qi to move recklessly, resulting in itching. Therefore, wind urticaria with itching arises from this cause. There is also red urticaria, which suddenly appears like mosquito bites, causing extreme irritability and itching; vigorous scratching causes the rash to rise, and scratching makes it appear instantly. There is also white urticaria, which presents with chills upon onset; conversely, red urticaria presents with fever. Wind urticaria occurs when pathogenic factors reside in the skin and are subsequently combined with wind-cold, forming urticaria. Red urticaria results from cold-dampness combating within the muscles, where wind-heat combines to form red urticaria. Symptoms worsen with heat and improve with cold. White urticaria results from wind qi combating within the muscles, where wind-cold combines to form white urticaria. Symptoms worsen with cold or exposure to wind, but improve on sunny days or with warm clothing. The pulse is floating and surging; floating indicates wind, and surging indicates qi (pathogenic factor). When wind and qi combat each other, urticaria forms, causing body itching. Zhu Danxi stated: Urticaria belongs to heat and phlegm; treatment should clear fire and descend phlegm in the Lung, or release the exterior through sweating, or there may be cases suitable for purgation. For urticaria at the exterior level, use Xiaodu Yinzi (Disinfecting Decoction) or Fangfeng Tongsheng San (Saposhnikovia Decoction for Unblocking the Hinges). For urticaria at the interior level, use Da Chaihu Tang (Major Bupleurum Decoction) or Sishun Yinzi (Four-Smooth Decoction). For deficiency patterns, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction). The treatment methods are the same as those for Shanghan (Cold Damage). Master Zhu Yuanjun, in his thirties, suffered from chronic urticaria with body numbness and a purple hue; he was treated with Fangfeng Tongsheng San added with Niubangzi (Arctii Fructus), ground into an extremely fine powder. Two qian were taken each time, decocted in one and a half cups of water with ginger juice until pungent, and taken hot before meals.
Someone asked about macules and papules. The answer was: Formulary theories state that moisture in the muscles, if constrained by cool or hot qi, leads to heat accumulation without dispersion, forming red urticaria. If constrained by wind pathogen, wind-heat combats to form white urticaria. Red urticaria aggravates with heat and alleviates with cold because heat is stagnant internally, thus disliking heat and favoring cold. White urticaria worsens on rainy days and resolves on sunny days because heat dissipates externally, thus disliking cold and favoring heat. Heat combing the Blood level causes pathogens to manifest at the exterior as red. Wind-damp combing the Qi level impedes Qi and fluid movement, causing pathogens to manifest at the exterior as white. Thus, treatments correspond accordingly: for pathogenic heat, one dislikes heat and prefers cool; for pathogenic dampness, one dislikes rain and prefers sun. Formularies also mention "wind acne," which refers to what the *Neijing* calls "acne and prickly heat arising from sweat exposed to dampness." It also says "sweat from labor exposed to wind, compressed by cold leading to stagnation, causes acne and prickly heat," which belongs to the category of urticaria. These all refer to heat stagnating in the muscle pores due to external pathogens. However, this differs from the *Neijing*'s discussion on Shaoyang and Shaoyin, and the Ministerial Fire and Sovereign Fire, where excessive guest heat causes Danzhen (red rash) to erupt externally, which is not mentioned in formularies. Therefore, changes caused by human Qi and the stagnation/emission of Ministerial and Sovereign Fires were likely omitted. This condition is one such variation. Thus, the emission of the two fires affecting the Qi and Blood at the exterior is no different from external pathogen stagnation. Furthermore, children developing chickenpox often have a secondary eruption of rashes, which is also due to fetal toxins not fully expelled by the Ministerial and Sovereign Fires.
Chu Yushi treated skin wind-heat with generalized urticaria using equal parts of Niubangzi (Arctii Fructus) and Fuping (Marsilea Ventralis), administered as two qian mixed with Bohe Tang (Mint Decoction), twice daily. Kushen Wan (Bitter Coptis Pill, from *Yanyi*) was used for someone suffering from generalized fine wind-heat rashes. The itching and pain were unbearable, affecting the head, shins, umbilicus, abdomen, and private areas, accompanied by excessive saliva and phlegm, and insomnia at night. One liang of Kushen (Sophorae Radix) powder and two liang of Zaojiao (Gleditsiae Fructus) were rubbed in one sheng of water, filtered, and boiled in silver/copper vessels to form a paste, mixed with Kushen to make pills the size of paulownia seeds. Twenty pills were taken with warm water after meals, curing the patient the next day.
The *Qianjin Fang* (Essential Prescriptions Worth a Thousand Gold) treatment principles suggest wiping white urticaria with Fanshi Zhi (Alumen Juice). Alternatively, bathing with Suoluo (Akebiae Caulis) and a little wine is beneficial. (Yao Shi used this for red urticaria).
