A patient presented with abdominal distension, vomiting, and vertigo. Treatment with Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Shan Zhi (Gardeniae Fructus), Zi Su (Perillae Folium), Xing Ren (Armeniacae Semen Amarum), Zhi Qiao (Aurantii Fructus), Jie Geng (Platycodonis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Dang Gui (Angelicae Sinensis Radix), Chi Shao (Paeoniae Radix Rubra), Hong Hua (Carthami Flos), and Tao Ren (Persicae Semen) for four doses stabilized the condition. Subsequently, due to excessive bleeding, the patient became comatose with blackened vision. Recovery was achieved using Shi Quan Da Bu Tang (Decoction of Ten Tonic Ingredients) and other herbs. At that time, muscles were ulcerated with dripping pus, tendons were spasmodic, and bones ached. Xue Ji palpated the pulse as floating and choppy on the superficial level, and weak on the deep level. This indicated that Qi and Blood were depleted, failing to nourish the tendons; the weakness of the tendons failed to bind the bones. Therefore, herbs tonifying Qi and Blood were administered, leading to recovery.
Another patient, after healing from a beating injury, neglected proper recuperation, resulting in a cloudy head and eyes. Taking wind-dispelling and phlegm-resolving herbs aggravated the vertigo, and taking Niu Huang Qing Xin Wan (Calculus Bovis Heart-Clearing Pill) caused abdominal pain. The beating wound remained swollen, itchy, accompanied by fever, thirst, anorexia, and rising phlegm-Qi. It was considered a recurrence of residual toxins from the beating injury. Palpation of the left Chi (Cunian) pulse revealed it was surging but empty upon pressure. Xue Ji stated: "This is insufficiency of the Kidney Meridian, failing to grasp Qi and return it to its source." Treatment with Ren Shen (Ginseng Radix et Rhizoma), Huang Qi (Astragali Radix), Fu Ling (Poria), Chen Pi (Citri Reticulatae Pericarpium), Dang Gui, Chuan Xiong, Shu Di Huang (Rehmanniae Radix Praeparata), Shan Yao (Dioscoreae Rhizoma), Shan Zhu Yu (Corni Fructus), Wu Wei Zi (Schisandrae Chinensis Fructus), Mai Men Dong (Ophiopogonis Radix), and Zhi Gan Cao (Glycyrrhizae Radix et Rhizoma Praeparata cum Melle) led to rapid recovery. Later, due to exertion, symptoms of low-grade fever, thirst, headache, fatigue, and poor appetite appeared. Recovery was achieved using Bu Zhong Yi Qi Tang (Decoction for Supplementing the Middle and Augmenting the Qi) plus Mai Men Dong and Wu Wei Zi.
A patient suffered from distension and stuffiness in both hypochondria, a desire to cough without success, a metallic taste in the mouth, swelling and pain in the buttocks and legs, fatigue, anorexia, irritability, thirst, and a large pulse. This was identified as irritability due to blood loss. Administration of child urine mixed with wine, along with Bian Fa (stone pricking therapy) at the affected sites, expelled a significant amount of stagnant blood and necrotic tissue. Suddenly, fever, irritability, and sweating occurred. Three doses of Du Shen Tang (Decoction of Single Ginseng) slightly alleviated the symptoms. Subsequently, several doses of herbs tonifying Qi and Blood and clearing Liver Fire improved dietary intake. Intermittent use of Du Shen Tang resolved all symptoms and significantly increased appetite. However, insomnia persisted. Four doses of Gui Pi Tang (Restore the Spleen Decoction) plus Shan Zhi and Zhu Ru (Bambusae Membrana Sacca) induced deep sleep. Due to mental overexertion, irritability, thirst, and spontaneous sweating recurred with a large but forceless pulse. Two doses of Dang Gui Bu Xue Tang (Tang Gui's Decoction for Tonifying Blood) brought stability. Then, Shi Quan Da Bu Tang, modified by removing Chuan Xiong and adding Mai Men Dong, Wu Wei Zi, Mu Dan Pi (Moutan Cortex), Di Gu Pi (Lycii Cortex), Ma Huang Gen (Ephedrae Radix et Rhizoma), and Chao Fu Mai (Triticum Vulgare Fructus Praeparatus) for several doses stopped the sweating. Necrotic tissue began to slough off, and after more than twenty doses, new granulation tissue grew.
Another patient presented with irritability, flushed face, dry mouth with thirst, and a surging pulse that felt empty upon pressure. Xue Ji stated: "This is irritability caused by Blood deficiency." Two doses of Dang Gui Bu Xue Tang stopped the symptoms. Later, evening tidal fever occurred. Treatment with Si Wu Tang (Four Substances Decoction) plus Chai Hu (Bupleuri Radix), Mu Dan Pi, Di Gu Pi, Huang Bo (Phellodendri Cortex), and Zhi Mu (Anemarrhenae Rhizoma) reduced the fever and promoted wound contraction. Li Dongyuan stated: "Fever with aversion to cold, extreme unquenchable thirst, and a large but forceless pulse are not indications for Bai Hu Tang (White Tiger Decoction); this is irritability due to Blood deficiency, and Dang Gui Bu Xue Tang should be used." Mr. Pei stated: "Muscular heat and restlessness, red eyes and face, with a surging yet deficient pulse indicate Blood deficiency. If Bai Hu Tang is mistakenly administered, the condition may become critical or fatal."
A patient had sweating on the forehead, fever, thirst, shortness of breath, irritability, bone pain, and non-ulcerated necrotic tissue. The necrotic tissue was excised, releasing fresh blood. Taking Huang Lian (Coptidis Rhizoma) and Huang Qin aggravated the condition. The pulse was surging yet faint, indicating dual deficiency of Qi and Blood with blazing pathogenic fire. Treatment with Si Wu Tang plus Ren Shen, Huang Qi, Bai Zhu (Atractylodis Macrocephalae Rhizoma), and Zhi Gan Cao, with small amounts of Chai Hu and Chao Huang Qin, stopped the head sweating after two doses. Adding Mai Men Dong, Wu Wei Zi, and Rou Gui (Cinnamomi Cortex) for two doses resolved all symptoms. Subsequently, Ren Shen, Huang Qi, Dang Gui, Bai Zhu, Chao Bai Shao (Paeoniae Radix Alba Praeparata), Shu Di Huang, Mai Men Dong, and Wu Wei Zi were taken for over ten doses, causing the necrotic tissue to ulcerate and the pus to become thick. Since new flesh did not grow, doubling the dosage of Bai Zhu in the previous formula promoted wound closure.
Wu Jishi, injured his head falling from a horse, suffered excessive bleeding, fever, irritability, muscle laxity, and twitching. Some suggested administering herbs for Po Shang Feng (Wind-Strike following Injury). I said: "This is caused by Blood deficiency and moving Fire. Profound tonification of Blood is appropriate." Two doses of Sheng Yu Tang (Sage's Decoction for Replenishing Life) brought peace, followed by nourishing Qi and Blood to heal the wound.
Zhang Jinshi, fell from a horse in late autumn, suffering excessive blood loss, sweating, and irritability. The next day, sweating ceased spontaneously, but heat and restlessness worsened, accompanied by trismus and hand tremors. This was Yin-Blood deficiency allowing Yang-Fire to invade, causing sweating; however, Cold Qi constricted the Cou Li (pores/skin texture), preventing sweat release and Fire venting, leading to severe internal stagnation and additional symptoms. Four doses of Si Wu Tang plus Chai Hu, Huang Qin, and Shan Zhi slightly alleviated the condition. Further treatment with Si Wu Tang plus Ren Shen, Huang Qi, Ruan Chai Hu (Soft Bupleuri Radix), Wu Wei Zi, and Mai Men Dong resulted in recovery.
