The *Huangdi Neijing* (Yellow Emperor's Inner Canon) states: "When the Yin pulse beats forcefully and is distinct from the Yang pulse, this indicates pregnancy." It also says: "If the pulse corresponding to the Hand-Shaoyin Heart meridian beats significantly vigorously, it indicates pregnancy." Some argue that a smooth, quick pulse resembling the pecking of a sparrow is a sign of several months' gestation. However, upon my frequent examination, I find this not always to be the case. Only when I observe a smooth and fluent pulse at both Guan positions (middle position on the radial artery, corresponding to Liver and Spleen) do I confirm pregnancy. Someone asked: "Why does the pulse manifest at the two Guan positions?" I replied: "The Guan positions correspond to the Liver and Spleen. The Liver stores blood, and the Spleen controls blood. When blood does not flow normally through the meridians but instead coagulates to form a pregnancy, the pulse manifests at the Guan positions. A smooth, harmonious, and even pulse here is most accurate. This is a secret principle unexposed by predecessors." Someone asked: "Does the pulse for amenorrhea (cessation of menstruation) also manifest at the Guan position?" I replied: "The pulse for amenorrhea must be choppy and stagnant, and it is not necessarily confined to the Guan positions. In contrast, the pulse for pregnancy must be smooth and fluent, and it definitely manifests at the Guan positions. At the early stage of pregnancy, it is common to see a deep and hidden pulse at both Chi positions (distal position on the radial artery, corresponding to Kidney). This is a secret I have realized over decades of practice, now recorded in this book as a standard criterion. By combining this with whether the patient has other diseases, whether the pulse matches the condition, and referencing the theories of former sages, one can avoid errors in clinical diagnosis."
Ancients stated: The left Cun position (proximal position on the radial artery) belongs to Yang; a floating and large pulse indicates a male fetus. The right Cun position belongs to Yin; a deep and solid pulse indicates a female fetus. If both Cun pulses are large, it suggests twins of the male sex. If both Cun pulses are solid, it suggests twins of the female sex. I often observe that a large and vigorous pulse tends to indicate a girl, while a calm pulse with harmony between both hands indicates a boy. If the pulse on the left hand is larger, it indicates a boy; if the right hand is more vigorous, it indicates a girl. This is relatively easy to distinguish. However, some individuals have constitutions naturally characterized by Six-Yang or Six-Yin pulses, so gender cannot be determined solely by pulse size. One must carefully examine the specific situation. A solid pulse at the left Chi position indicates a boy; a solid pulse at the right Chi position indicates a girl. Regarding twin pregnancies, generally, both hands show large and vigorous pulses; why restrict judgment only to the Cun position? One must understand that predecessors discussed principles but could not fully depict forms in writing. The examiner should grasp the essence intuitively, observing the thickness of the constitution and the vigor or decline of Qi and Blood. Furthermore, individuals with abundant Qi and Blood may show a pregnancy pulse before one month, while those with deficient Qi and Blood may not show it until after two months. This is also due to differences in constitutional thickness.
