When a heat disease is in the exterior, and sweating has been administered but the pathogen has not resolved, or after vomiting and purging, the heat-toxic qi fails to dissipate, leading to restlessness and delirious speech. This constitutes deficiency at the exterior and excess at the interior, with heat qi agitating externally, thus causing the body to manifest macules resembling brocade patterns. In cases of macule eruption, exterior-releasing herbs must not be used, as this would cause sores to open and leak, further aggravating the macular ulceration due to the existing exterior deficiency.
The liver opens into the eyes. If liver qi is deficient, heat-toxin may rush upward through the deficiency into the eyes, causing redness, pain, or the formation of nebulae (corneal opacities), red-and-white membranes, pterygia (fleshy growths), and sores.
Heat-toxic qi emerges from the zang-fu organs and attacks the hands and feet, resulting in heat, redness, swelling, and pain in these extremities. The Jing-Well, Ying-Spring, and Shu-Acupoints of the five zang and six fu organs all emerge from the fingers and toes; therefore, this toxin originates internally and moves outward.
When there is exterior deficiency and interior excess, with abundant internal heat-toxin, sores frequently occur. In severe cases, they cover the entire body, resembling fire blisters. If the base is red and the head white, the toxin is mild; if the color is purple-black, the toxin is severe. These sores resemble beans and are also called bean sores.
When a disease has just convalesced, blood and qi have not yet recovered, and the skin remains loose and porous, exposure to wind and sunlight can cause fine sores to erupt over the whole body, itching like tinea or scabies, named [text missing].
Heat-toxin knots within the abdomen, grain qi declines, and heat-toxin becomes excessive, causing the "three parasites" to move and devour the five zang organs, often leading to diarrhea and dysentery, along with itching sores in the lower region. If sores form inside the lower lip and the patient desires sleep, this indicates the worms are devouring the lower region; in severe cases, the anus rots, exposing the five zang organs.
This arises from heat qi residing in the intestines and stomach, carrying toxin, which results in the discharge of yellow-red fluid.
This occurs when heat-toxin injures the intestines and stomach, causing the discharge of pus and blood resembling fish brains, or like rotten meat juice, accompanied by high fever and abdominal pain. This is caused by damp-toxic qi.
Heat accumulation in the zang organs often causes diarrhea. If the toxic qi is excessive, it transforms into pus and blood, thereby forming dysentery. Dysentery refers to worms devouring the five zang organs and the lower region. If the lower region is affected, it causes sores in the anal opening and diarrhea, named [text missing] dysentery; if sores form without diarrhea, it is named [text missing].
This arises from heat in the spleen and stomach. Excessive sweating depletes the body fluids; depletion of fluids leads to dryness of the stomach, internal binding of heat, and constipation.
