Tumor Distension 2 (Early-stage Liver Cirrhosis)
Patient: Sun XX, Female, 60 years old
First Visit
The patient has suffered from chronic hepatitis for over 10 years. Recently, examination at a certain hospital revealed decreased plasma protein and an inverted albumin-globulin ratio, indicating progression to early-stage liver cirrhosis. Traditional Chinese Medicine (TCM) treatment was recommended.
Current symptoms include distension and discomfort in the epigastrium and hypochondriac regions, worsening after meals; dull pain in the right hypochondrium exacerbated by pressure; poor appetite; general fatigue; irritability and restlessness; and frequent dreaming during sleep. The pulse is wiry, slippery, and rapid bilaterally; the tongue is red with a thin yellow coating. This indicates liver-gallbladder qi stagnation transforming into heat entering the blood level. A method of clearing and resolving damp-heat will be used initially.
Chai Hu (Bupleuri Radix) 6g, Huang Qin (Scutellariae Radix) 10g, Xiang Fu (Cyperi Rhizoma) 10g, Chuan Lian Zi (Meliae Fructus) 10g, Yuan Hu (Corydalis Rhizoma) 10g, Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma) 10g, Chi Shao (Paeoniae Radix Rubra) 10g, Yu Jin (Curcumae Radix) 10g. Seven doses.
Second Visit
After taking the medicine, abdominal distension gradually lightened, sleep became more peaceful, and nightmares decreased. The pulse remains wiry and slippery but feels soft upon pressure; the tongue is red with a white coating. The method of harmonizing the liver and stomach will still be used.

[Commentary]: The patient in this case had early-stage liver cirrhosis, primarily presenting with epigastric and abdominal distension, accompanied by pain in the hypochondriac, lumbar, and back regions. Analysis of the pulse, tongue, and symptoms indicates that damp-heat accumulated and stagnated without transformation, and liver-gallbladder stagnation heat entered the blood level. Therefore, treatment focused on clearing and draining liver-gallbladder stagnation heat, clearing and resolving spleen-stomach damp-heat, cooling blood and resolving stasis, and unblocking the Triple Burner. To clear and drain liver-gallbladder stagnation heat, Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Chuan Lian Zi (Meliae Fructus), Long Dan Cao (Gentianae Radix), and Xia Ku Cao (Prunellae Spica) were used; to clear and resolve damp-heat, Pei Lan (Eupatorii Herba), Huo Xiang (Agastaches Herba), Yin Chen (Artemisiae Scopariae Herba), and Su Ye Geng (Perillae Caulis and Folium) were used; to cool blood and resolve stasis, Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma), Qian Cao (Rubiae Radix et Rhizoma), Chi Shao (Paeoniae Radix Rubra), Yu Jin (Curcumae Radix), and Yuan Hu (Corydalis Rhizoma) were used; to unblock the Triple Burner, Jiao San Xian (Fried Hawthorn, Fried Medicated Leaven, and Fried Barley), Shui Hong Hua Zi (Polygoni Multiflori Caulis), Da Fu Pi (Arecae Pericarpium), and Bing Lang (Arecae Semen) were used; to regulate qi mechanism, Qing Chen Pi (Citri Reticulatae Pericarpium Viride and Citri Reticulatae Pericarpium), Xiang Fu (Cyperi Rhizoma), Mu Xiang (Aucklandiae Radix), and Zhi Ke (Aurantii Fructus) were used; to unblock collaterals and relieve pain, Si Gua Luo (Luffae Fructus Retinervus), Sang Zhi (Mori Ramulus), and Bai Jie Zi (Mustard Seed) were used. During treatment, tonifying methods were not used, nor was the middle jiao guarded exclusively, nor were cloying substances employed. Following this method according to syndrome differentiation, combined with dietary regulation and exercise, after nearly one year of treatment, all symptoms disappeared, physical strength increased, liver function tests normalized, the albumin-globulin ratio returned to normal, and the patient resumed normal work.
Cancer 1 (Metastatic Liver Cancer)
Patient: Zhou XX, Male, 40 years old
First Visit
On May 20, 1985, due to a mass in the epigastrium accompanied by pain and vomiting, the patient was diagnosed with gastric cancer in November 1984 at the Affiliated Hospital of a Provincial Medical College and underwent total gastrectomy. In 1985, due to pain in the liver region, he visited a hospital in Beijing. Ultrasound indicated a space-occupying lesion in the liver, diagnosed as metastatic liver cancer.
Ultrasound findings on March 14, 1985: Hypoechoic areas were seen in the lateral segment and medial margin of the left liver lobe, measuring 2.0×2.1 cm and 3.0×2.9 cm respectively, with relatively clear boundaries. The right liver showed uniform echogenicity with no obvious abnormal areas. Impression: Multiple space-occupying lesions in the left liver (Malignant).
The patient felt fullness and discomfort under the right hypochondrium, intermittent pain, irritability, frequent dreaming during sleep, dry mouth and throat. The tongue was red and thin with a dry white coating. The right pulse was wiry, fine, and slippery; the left pulse was wiry and fine. This indicates liver heat injuring yin, qi mechanism obstruction, and collateral disharmony. This resulted from prolonged emotional constraint leading to liver qi stagnation, which transformed into fire and injured yin over time. Initially, the method of regulating the qi mechanism to relieve liver stagnation will be used.

He was instructed to avoid spicy and rich foods, eat only light vegetarian food, and persist in daily walking exercises without interruption.
Second Visit: June 10, 1985
After medication, hypochondriac discomfort gradually eased, and pain significantly reduced. The pulse remained wiry and fine; the tongue was red with a dry white coating; irritability and frequent dreaming persisted. Recent ultrasound re-examination results:
Ultrasound findings on July 27, 1985: A hypoechoic mass measuring 1.3×1.2 cm was seen in the medial segment of the left liver lobe, with clear and regular boundaries. Other echoes were acceptable.
Ultrasound impression: Sinusoidal space-occupying lesion in the left liver lobe (Malignant).
The pulse was wiry, fine, slippery, and rapid; frequent dreaming occurred during sleep. This still indicates uncleared stagnation heat, with heat in the yin level. The method of cooling blood, resolving stasis, benefiting qi, and activating collaterals will continue.

The patient took the above prescription back to his hometown in Shandong and continued taking it for 3 months. All symptoms disappeared, and his physique gradually strengthened. He came to Beijing for re-examination in November 1985. Results were as follows.
Ultrasound findings on November 9, 1985: Liver echoes were uniform; no obvious abnormal echo masses were seen; blood vessels were clear. The pancreas was poorly visualized.
Ultrasound impression: No obvious abnormalities in the liver.
In August 1986, the patient returned to Beijing for another re-examination, and no abnormalities were found.
[Commentary]: This case involved liver cancer metastasizing after gastric cancer surgery, with a high degree of malignancy. Master Zhao achieved satisfactory curative effects using TCM syndrome differentiation and treatment. Reviewing the entire treatment process of this case, it can be roughly divided into three stages.
The first stage includes the first and second visits, focusing mainly on regulating the qi mechanism to relieve liver stagnation.
Although the patient suffered from qi and yin deficiency due to cancer consumption and long-term dysfunction of spleen-stomach transportation after surgery, the main contradiction in the pathogenesis was still liver stagnation heat. Because the patient learned he had cancer, with postoperative recurrence and metastasis, believing there was no cure and that he would soon die, his mood was heavy, spirits low, and he was unhappy all day. This was one of the main factors causing liver qi mechanism disorder. Prolonged liver stagnation inevitably transforms into heat, manifesting as liver stagnation heat such as irritability and frequent dreaming. Heat stagnating internally inevitably injures yin, manifesting as dry mouth, fine pulse, and thin tongue. Comparatively, liver stagnation is primary and foremost. Therefore, treatment focused on regulating the qi mechanism to relieve liver stagnation heat. The formula used Sheng Jiang San (Ascending and Descending Powder) as a base. Chan Yi (Bombyx Batryticatus) and Jiang Can (Bombyx Batryticatus) possess clear-resolving properties to raise yang and ascend; combined with Xuan Fu Hua (Inulae Flos), Xing Ren (Armeniacae Semen Amarum), and Pi Pa Ye (Loquat Leaf) to diffuse lung qi, descend qi, and send turbidity down; Pian Jiang Huang (Curcumae Longae Rhizoma) was used to unblock qi and blood stasis. Dan Shen (Salviae Miltiorrhizae Radix et Rhizoma) assisted in invigorating blood and resolving stasis; Mu Xiang (Aucklandiae Radix) assisted in regulating qi; Jiao San Xian (Fried Hawthorn, Fried Medicated Leaven, and Fried Barley) eliminated accumulation and unblocked the Triple Burner. He taught the patient to eat vegetarian food to protect transportation, exercise to circulate qi and blood, and avoid spicy and rich foods so that the six fu organs remain pure and stagnation heat does not generate. Such comprehensive regulation allowed liver stagnation heat to dissipate. Although cancer was not directly treated, the root cause of cancer generation was addressed.
The second stage of treatment, from the third to sixth visits, after the first stage, the qi mechanism gradually smoothed, symptoms decreased, the patient's mood calmed compared to before, and confidence in treatment increased. The focus of treatment gradually shifted to cooling blood and resolving stasis. Because the liver stores blood, prolonged liver stagnation heat inevitably causes heat to enter the blood level resulting in stasis. Therefore, although regulating the qi mechanism alone is necessary, it lacks specificity; it must be combined with cooling blood and resolving stasis to regulate both qi and blood. Thus, on the basis of Sheng Jiang San (Ascending and Descending Powder) regulating the qi mechanism, Ban Zhi Lian (Scutellariae Barbatae Herba) was added to clear heat and detoxify; Bai Tou Weng (Pulsatillae Radix), Chi Shao (Paeoniae Radix Rubra), and Qian Cao (Rubiae Radix et Rhizoma) were added to cool blood and resolve stasis. After more than a month of regulation, the patient felt various symptoms gradually decreasing, mental state improving, confidence increasing, and actively cooperating with treatment. Re-examination by ultrasound indicated that of the two original cancerous tumors in the liver, one disappeared, and one shrank significantly.
The third stage of treatment began from the seventh visit. On the basis of regulating the qi mechanism and cooling blood/resolving stasis, herbs to benefit qi and support healthy qi were added. Because qi and yin deficiency existed from the beginning of this disease, belonging to a pattern of healthy qi deficiency and evil excess. After the first two stages of treatment, stagnation heat and evil qi gradually dissipated. At this time, discussing tonification poses no risk of retaining evil. Moreover, the pulse was soggy and soft, indicating insufficiency in the qi level. If one solely relied on clearing and resolving methods, it would not be the best strategy; now is the most appropriate time to choose supporting healthy qi and dispelling evil. Therefore, on the basis of regulating the qi mechanism and cooling blood/resolving stasis, Sheng Huang Qi (Astragali Radix) was added to benefit qi and support healthy qi, and Sheng Mai San (Generate the Pulse Powder) was used to benefit qi and nourish yin. Before returning home, Jiao San Xian (Fried Hawthorn, Fried Medicated Leaven, and Fried Barley) and Shui Hong Hua Zi (Polygoni Multiflori Caulis) were added to the prescription carried by the patient. Because he needed to take it long-term after returning home, these were added to aid digestion. Thus, the combination was comprehensive, suitable for long-term use. The patient persisted in taking this prescription for 3 months, the liver tumor completely disappeared, achieving satisfactory therapeutic results.
Tumor 2 (Uterine Fibroid)
Patient: Wan XX, Female, 46 years old
A mass in the right lower abdomen, painful upon pressure, present for over 3 months. Gynecological examination confirmed uterine fibroid, and surgical excision was recommended. The patient refused surgery and sought consultation. For the past year, menstruation has been irregular, periods extended, with heavy flow containing clots. In the last two to three months, menstrual flow was even heavier, occurring twice a month, leading to extreme fatigue. The pulse was wiry, slippery, fine, rapid, and forceful. The method of invigorating blood and resolving stasis was planned.
Chai Hu (Bupleuri Radix) 6g, Chuan Lian Zi (Meliae Fructus) 10g, Huang Qin (Scutellariae Radix) 6g, Xiang Fu (Cyperi Rhizoma) 10g, San Leng (Sparganii Rhizoma) 6g, E Zhu (Curcumae Rhizoma) 6g, Mu Xiang (Aucklandiae Radix) 10g, Qing Chen Pi (Citri Reticulatae Pericarpium Viride and Citri Reticulatae Pericarpium) 10g each, Xia Ku Cao (Prunellae Spica) 10g, Yi Mu Cao (Leonuri Herba) 10g.
The patient took the above prescription for a total of 3 months, over 40 doses. Menstruation returned to normal, the mass in the lower abdomen disappeared, with no tenderness. Gynecological examination confirmed that the uterine fibroid no longer existed.
[Commentary]: Uterine fibroids often present as menorrhagia, which is due to blood stasis blocking collaterals, causing blood to leave its vessels. Treatment does not need to firmly stop bleeding; rather, it requires invigorating blood, resolving stasis, unblocking collaterals, and eliminating the tumor, applying the method of treating obstruction with unblocking agents. If it is a submucosal fibroid, this method is very effective, achieving tumor elimination and hemostasis. Eliminating the tumor relies heavily on San Leng (Sparganii Rhizoma) and E Zhu (Curcumae Rhizoma); the remaining herbs are still for clearing liver heat and regulating the qi mechanism.
