I have heard that "shang" (injury with bleeding) and "sun" (bone injury with pain) vary in urgency, and treatment should be flexible (physicians must not adhere rigidly to a single method). "Die" (falling from height or being crushed/twisted is called die; this involves injury first, then fright), and "Da" (fighting or beating with sticks/clamps is called Da; this involves fright first, then injury). Their severity differs, each having its own principle (both die and da involve shang and sun; one must treat according to severity. For die, treat the injury first, then calm the fright; for Da, calm the fright first, then treat the injury. This is the general rule, but flexibility is required in practice). For instance, if skin and muscles are broken, stopping bleeding and dispelling wind is superior (for broken skin/muscles, regardless of location, use external hemostatic and flesh-regenerating medicines, and internal wind-dispelling medicines. If there is internal injury with vomiting blood or epistaxis, harmonizing qi and activating blood is primary). If tendons and bones are broken, activating blood and stopping pain is best (for tendon/bone injuries, externally apply reduction, splinting, and plasters; internally take blood-activating and pain-stopping medicines. If organs are damaged leading to coma, breathlessness, and unconsciousness, harmonizing qi and moving blood is primary). For tidal fever (exterior pathogen), dispersing methods can be used. For constipation, purging and draining is acceptable. For swelling of skin and flesh, breaking qi and treating blood is key (swelling at the site, whether red, purple, black, or blue, is due to stagnation and rebellion of qi and blood not dispersing. Externally use warming moxibustion/plaster methods and apply medicaments; internally take qi-breaking and blood-breaking medicines. If the condition persists and excessive medication fails to reduce swelling, harmonization is needed. If swelling follows an open wound, dispelling wind is primary). For abdominal distension, harmonizing Ying and Wei is appropriate (trauma to chest, hypochondrium, abdomen, or back causing abdominal distension and unceasing pain: externally apply patches and warming methods; internally take qi-breaking and stasis-resolving medicines. If bowel movements normalize, harmonize blood and move qi). For the elderly and weak, harsh purgatives/draining agents must be avoided excessively (elderly and frail patients should avoid harsh drugs; excessive use may cause other diseases). For the young and strong, tonification should not be rushed (young people should not be tonified early, lest pathogens remain; remember this). Since the exterior has been released, do not induce heavy sweating, lest evil pathogens enter through deficiency (when treating trauma, release the exterior first, then treat the injury; after releasing the exterior, do not repeat it; be cautious). Do not use purgatives again for spontaneous diarrhea/urination, fearing damage to the Yuan Shen (if stool or urine flows spontaneously, do not use purgatives, lest vital energy be depleted). It is essential to know that if blood has not exited, a surging and large pulse is favorable (high falls with internal blood stasis and abdominal distension: a firm and strong pulse indicates survival, while a small and weak pulse indicates death). If blood has exited, a fine and subtle pulse is auspicious (for cuts/falls with bleeding: if bleeding cannot stop and the pulse is large, death occurs in seven days; a slippery and fine pulse indicates survival. For cut wounds bleeding for three days, a large pulse means death in twenty days. For severe metal wounds with excessive bleeding, a deficient and fine pulse indicates survival, while a rapid, full, and large pulse indicates death. For metal wound bleeding, a deep and small pulse indicates survival, while a floating and large pulse indicates death). If the Mingmen pulse is gentle and relaxed, and the Guan pulse is solid, even severe injuries are not fatal (if the Mingmen pulse is gentle and the Guan pulse is full and large, severe injuries are not fatal). If the Mingmen pulse is deficient, rapid, and drifting away, even mild injuries will cause harm (the secret of treating fractures states: if the Yuji pulse does not cease, death occurs). Therefore, for stopping bleeding, use Taohua Sanbao (Taohua San and Sanbao San treat broken skin with unceasing bleeding). For activating blood, use Guirui Yiyang (for broken tendons, after reduction, mix Guirui San with boiled wine and take together with Yiyang Dan; these four formulas are listed later). For dispelling wind-heat, Xiaofeng San is praised as perfect (Xiaofeng San treats open wounds with bleeding or swollen flesh). For stopping pain, Zhutong San is hailed as king (Huoxue Zhutong San specifically activates blood and stops pain, effective for both internal and external injuries). For broken skin and swollen flesh, Jiangshuai San applied externally yields miraculous effects (for wind-involved swollen flesh after injury, apply Jiangshuai Dingfeng San externally and take Xiaofeng San internally). For broken bone pain, Shensheng San applied externally is unparalleled (for broken bones, after reduction, apply Shensheng San to the fracture site, secure with thin fir wood slices and silk bandages, avoiding exposure to wind). For bone/muscle stasis generating phlegm/saliva, first eliminate filth (if a bone fracture is untreated for a long time, generating saliva/phlegm preventing reduction, first take Bihui Wan, then reduce the next day for self-efficacy). For rotting sores and foul flesh, pungent aromatics are suitable (untreated or improperly treated wounds with foul pus and blood: wash with Xinxiang San decoction, then apply Shengji San several times until healed). To disperse exterior pathogens, Wujisan is indicated (for exterior cold with tidal fever). To unblock interior excess, Wanling Tang is used (for major trauma with body bruising/swelling; it is effective everywhere). Honghua Mo can manage abdominal distension (Honghua Huoxue San treats distension and shortness of breath). Sumu San can treat widespread swelling injuries (treats extreme swelling from whole-body trauma). For coma and breathlessness, Tongguan or Qibao can be chosen (for trauma-induced coma, blow Tongguan San externally and take Qibao Dan internally to regain consciousness, then adjust treatment based on injury). For unconsciousness, Heqi Liushen is worthy of trial (for severe cases where qi/blood attacks the heart causing inability to speak, urgently administer Heqi San orally to regain consciousness, apply warming medicine externally, then resume original medicine adding Chenxiang and Sangui powder, or use Liushen Muxiang Tang which is better). For unceasing hematemesis, Bangshuang San responds immediately; for stagnant qi, Aihui Gao applied resolves it (for persistent pain at the injury site, apply Aihui Yulong Gao to heal itself). Baiyu San is essential for hands and feet (for hand/foot pain, Baiyu Lingyan San is primary). Zijin Dan serves as a pillar for the whole body; for labor-induced body ache, Taibao San is the essential remedy (Shisan Taibao San treats minor common injuries. For overexertion, long travel fatigue, whole-body soreness, and limb weakness, it can be taken. Formulas follow). For sprain/twist lower back pain, Jiangjun Mo is most appropriate (Jiangjun Yunqi San treats sprain/twist lower back pain; regular use is miraculous). Foshou Miaosan should not be used casually for mild conditions (Shenmiao Fo San is for broken tendons/bones or severe metal wounds near death; only use this medicine for miraculous effect, value it highly). Huangu Lingdan ensures recovery and health (Shenxian Huangu Dan is used for final recovery after trauma healing). Lijin Wan is effective for all types of trauma and combat (Lijin Wan, also known as Tiebu Shan, is effective for trauma and martial arts sparring). Zhenbao Gao is very strong for injuries and carbuncles (this plaster adheres to injuries and carbuncles). How can medicines lack wonderful principles? Stick injuries have their own unique prescriptions (stick/clamp refers to official beatings with boards/clubs). Used appropriately, treatment is like turning a palm; mishandled, changes arise unpredictably. Alas, treating these syndromes is not ordinary. Externally entangling skin, internally connecting to zang-fu. Altering appearance, like snake shedding skin or dragon changing bones. Dripping pus and blood, like earthworms in ash or boiling soup. Physicians value recognizing syndromes; do not mistake deer for horse. Medicines should match the disease; do not view tiger as wild wolf. Stubbornly erroneous without change, fearing hesitation when encountering disease, leaking the true essence of marrow. Not a gentleman who would not teach the secrets of the lungs and intestines; memorize and recite without forgetting. This is the preface.
