A person who has been infected by cold-dampness may experience testicular pain; the pain becomes unbearable upon exposure to cold. This is due to dampness invading the Kidney Meridian (Kidney Channel). When dampness invades the Kidney, one might expect the disease to manifest in the lower back. Why does the back not hurt, but rather the testicles? It is unknown that the testicles belong to the Kidney. If Kidney Qi fails to reach the testicles, the external vitality cannot be revitalized. Generally, when the Kidney receives dampness, it becomes cold; cold in the Kidney means cold in the testicles, and Qi stagnates in the waist and Kidney region, so the testicles fail to respond appropriately. The reason for testicular pain is the formation of hernia (Shan). Although formed due to cold in Kidney Qi, it is also formed due to dampness in the testicles. After ejaculation, if a person sits in a cold-damp area, internal and external pathogens simultaneously attack, and only the testicles bear this burden alone. The treatment method is to warm the cold in the Kidney and eliminate dampness in the testicles; the disease will be removed as easily as sweeping dust. Use Jiu Wan Tang: Rou Gui (Cinnamon Twig, 2 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang), Fu Ling (Poria, 1 liang), Yi Yi Ren (Coix Seed, 1 liang), Ju He (Tangerine Seed, 1 qian). Decoct in water and take orally. After one or two doses, symptoms lighten; after three doses, pain disappears; after ten doses, complete recovery occurs, with no recurrence.
This condition belongs to the Shaoyin Kidney Meridian. The Kidney contains extreme cold, leading to obstruction of Kidney Qi; the Kidney holds heavy dampness, causing further stagnation of Kidney Qi. Once cold and dampness are removed, Kidney Qi naturally circulates within the testicles. Moreover, Rou Gui and Ju He are particularly adept at entering the testicles, ensuring success upon application.
This condition can also be treated with Gui Li Tang:
Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang), Rou Gui (Cinnamon Twig, 2 qian), Shan Yao (Dioscorea Rhizome, 1 liang), Xiao Hui Xiang (Fennel Fruit, 2 qian), Li Zhi He (Lychee Seed, 3 pieces, crushed). Decoct in water and take orally.
A person infected by damp-heat may also suffer from testicular pain, which flares up upon heat exposure, though the pain is not severe. This is due to heat qi invading the Kidney Meridian. The Kidney detests heat most; even if deficiency fire flares up spontaneously, there is a risk of persistent erection. How much more so when pathogenic fire invades, adding heat to heat, making testicular pain inevitable? However, fire nature is urgent, and fire-pain should not last long. Why does it persist for months or years? Even if temporarily asymptomatic, why does it recur upon heat exposure? This is because heat is accompanied by dampness. Heat is urgent, while dampness is slow. Damp-heat attacks mutually: heat desires to dissipate but dampness remains; dampness desires to moisten but heat dries. Inside the testicles, Yin and Yang become discordant, making pain unavoidable. The treatment method is to eliminate damp-heat qi, and the hernia disease will naturally resolve. Use Li Wan Tang: Fu Ling (Poria, 1 liang), Yi Yi Ren (Coix Seed, 1 liang), Sha Shen (Glehnia Root, 2 liang). Decoct in water and take orally. Symptoms lighten after one dose, further lighten after two doses, and root out after ten doses without recurrence.
This formula uses Fu Ling and Yi Yi Ren to separate and drain dampness, and Sha Shen to transform heat in the Kidney. Since Sha Shen is particularly effective for treating hernia, using both achieves success.
Sha Shen Tang is also very effective for this condition.
Fu Ling (Poria), Bai Zhu (Atractylodes Macrocephala Rhizome), Sha Shen (Glehnia Root) (each 1 liang), Gan Cao (Licorice Root, 1 qian), Dan Pi (Moutan Bark, 5 qian), Rou Gui (Cinnamon Twig, 2 fen). Decoct in water and take orally.
A person suffering from testicular pain where Qi rushes upward into the Liver, causing distension and fullness in both hypochondria, with increased pain upon pressure, is often mistaken for abdominal Yin-cold. In reality, it is Cold invading the Jueyin Liver Meridian. The testicles connect not only to the Kidney but also to the Liver. The genital organ is the gathering point of the Sinew (Jin). Sinews belong to the Liver. Testicles can ascend and descend, and their membranes are intimately connected between the genital organs. Thus, Liver disease affects sinews, and sinew disease affects the testicles. Testicular pain rushing upward to the Liver demonstrates that same-nature diseases affect each other similarly; it is Liver Qi rushing into the testicles. Use Wei Wan Tang: Bai Shao (White Peony Root, 2 liang), Xiao Hui Xiang (Fennel Fruit, 3 qian), Ju He (Tangerine Seed, 1 qian), Chai Hu (Bupleurum Root, 1 qian), Sha Shen (Glehnia Root, 5 qian). Decoct in water and take orally. Pain subsides slightly after one dose, largely stops after two doses, hypochondriac distension resolves completely after three doses, and full recovery occurs after four doses.
This formula calms Liver Qi so it does not rush into the testicles. With Xiao Hui Xiang, Ju He, and Sha Shen dispersing pathogens in the testicles, both testicles remain stable, preventing upper-lower connection and pain.
Jie Shan Tang is also miraculous for this condition.
Rou Gui (Cinnamon Twig, 2 qian), Bai Shao (White Peony Root), Bai Zhu (Atractylodes Macrocephala Rhizome) (each 2 liang), Chai Hu (Bupleurum Root, 1 qian), Sha Shen (Glehnia Root, 5 qian). Decoct in water and take orally.
A person with bladder closure and difficulty urinating, experiencing testicular pulling pain radiating to the Small Intestine, is often attributed to Small Intestine Qi issues. However, it is actually Heat Stagnation in the Bladder. The Bladder transforms fluids; if cold, fluids do not transform; if hot, they also do not transform. Fluids failing to transform lead to Heat Stagnation in the Bladder, forcing fluids into the collaterals. Fluids entering the testicles cause them to enlarge daily, sometimes becoming as large as a bushel and unable to reduce. One must separate and drain these fluids. However, draining fluids without resolving heat allows Bladder Fire to rush directly to the testicles, worsening pain. Use San Wan Tang: Fu Ling (Poria, 1 liang), Ye Du Ruo Gen Zhi (Wild Costus Root and Branches, 1 liang), Sha Shen (Glehnia Root, 1 liang). Decoct in water and take orally. Pain ceases after one dose, testicles gradually shrink after two doses. Continuous use for two doses results in profuse watery diarrhea, and the scrotum returns to normal size.
The uniqueness of this formula lies in Du Ruo, specifically wild blue chrysanthemum found in fields, not garden varieties. This herb is cold and promotes sweating, and can directly enter the testicles to disperse pathogens. It assists Fu Ling and Sha Shen in draining dampness and clearing heat, achieving remarkable results. Since this herb induces sweating, administer Dang Gui Bu Xue Tang (Angelica Sinensis Blood-Nourishing Decoction) for several doses afterward to tonify Qi and Blood, preventing excessive deficiency.
San Long Tang is also excellent for this condition.
Fu Ling (Poria, 1 liang), Che Qian Zi (Plantago Seed, 3 qian), Rou Gui (Cinnamon Twig, 2 fen), Bi Xie (Dioscorea Hypoglauca Rhizome, 2 qian), Gan Cao (Licorice Root, 1 qian), Huang Bo (Phellodendron Bark), Zhi Mu (Anemarrhena Rhizome) (each 1 qian). Decoct in water and take orally.
When testicular pain changes to numbness and lack of sensation, it is called "Wood Kidney" (Mu Shen), caused by extreme cold obstructing Qi. Initially, this condition arises from contracting cold-dampness, followed by sexual intercourse, again exposing oneself to cold-dampness. Dampness enters the testicles internally, while cold knots externally, resulting in numbness. For this type of hernia, only Gui (Cinnamon) and Fu (Aconite) can penetrate the testicles to unblock Qi. Without herbs that disperse pathogens, Gui and Fu can only boost Yang, failing to disperse testicular pathogens. Many herbs disperse pathogens, but few target the testicles specifically, explaining why treatments for Wood Kidney often fail. Use Hua Mu Tang: Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang), Fu Zi (Prepared Aconite Root, 1 qian), Rou Gui (Cinnamon Twig, 1 qian), Du Ruo Gen (Costus Root, 1 liang), Chai Hu (Bupleurum Root, 1 qian). Decoct in water and take orally. Wrap in blankets and sleep; the body will sweat profusely until sweat pours like rain from around both kidneys. Recovery occurs after one dose.
In this formula, Bai Zhu benefits Qi in the waist and umbilicus; Du Ruo Gen disperses pathogens in the testicles; Fu Zi and Rou Gui facilitate circulation inside and out; Chai Hu resolves dampness in the Liver. Thus, one dose works miraculously.
Wei Gao Dan is also wonderful for this condition.
Fu Zi (Prepared Aconite Root), Gan Cao (Licorice Root), Yan Hu Suo (Corydalis Rhizome), Chai Hu (Bupleurum Root) (each 1 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 3 liang), Rou Gui (Cinnamon Twig, 3 qian), Huang Qi (Astragalus Root, 1 liang). Decoct in water and take orally.
Fox Hernia (Hu Shan) manifests as retraction and pain during the day, and extension and comfort at night, accompanied by strong erections. This is true Fox Hernia. If retraction occurs by day and extension by night but without sustained performance, it is false Fox Hernia. False Fox Hernia is a cold-damp condition, treatable with Jiu Wan Tang for cure. True Fox Hernia may result from sexual activity near spiritual sites or under moonlight, implying possession by spirits. Treatments differ based on the hernia type. In true Fox Hernia, lascivious Qi remains unresolved, knotting in the testicles. Foxes are lustful and vigorous, seducing at night, belonging to Yin. Retraction prevents coitus; attempting it causes excruciating pain, forbidden by spirits. Spirits belong to Yin; night is ruled by Yin. Human Yang Qi entering Yin combines with spirit Yin, creating harmony where spirits do not interfere, allowing vigorous performance. Upon ejaculation, Yang Qi exits, leaving pure Yin, causing pain again. Treatment seems to require expelling spirits, but spirits enter via deficiency. Tonifying deficiency before expelling spirits is necessary. Use Zhu Hu Tang: Ren Shen (Ginseng Root, 1 liang), Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Fu Ling (Poria, 5 qian), Rou Gui (Cinnamon Twig, 3 fen), Ju He (Tangerine Seed, 1 qian), Bai Wei (Cynanchum Root, 1 qian), Jing Jie (Schizonepeta Spike, 3 qian), Ban Xia (Pinellia Tuber, 2 qian), Gan Cao (Licorice Root, 1 qian). Decoct in water and take orally. Four doses ensure complete recovery.
This formula purely tonifies Yang. When Yang Qi is robust, Yin Qi naturally diminishes, and Fox Hernia heals without direct expulsion. Some argue that night-time vigor indicates strong Yang Fire, so tonifying Yang might increase demonic energy. Why does tonifying Yang extinguish spirits? During the day, Yang Qi dominates, suppressing spirits. At night, spirits exploit Yin Qi, gathering in the genitalia during intercourse—a false sign of Yang strength. By tonifying Yang, it flourishes throughout the day and night, leaving no room for spirits. Additionally, the formula contains herbs to soothe depression, expel pathogens, dissolve phlegm, and dispel spirits. Yin cannot overcome Yang, so spirits depart. Without tonifying Yang, such miraculous efficacy would be impossible.
Huang Qi (Astragalus Root, 2 liang), Rou Gui (Cinnamon Twig, 3 fen), Gan Cao (Licorice Root), Chai Hu (Bupleurum Root) (each 1 qian), Bei Mu (Fritillaria Bulb, 3 qian), Sha Shen (Glehnia Root, 1 liang). Decoct in water and take orally. Two doses ensure recovery.
External Cold invasion can cause a sensation of Qi rushing from the heart down to the scrotum, known as Ben Tun (Running Piglet). Its momentum is urgent, unstoppable, and painfully intense. While considered an external cold syndrome, it stems from Fire Deficiency in the Pericardium and Ming Men (Gate of Life) meridians. Fire of the Pericardium and Fire of the Ming Men reside in the Heart and Kidney respectively, yet they are interconnected. When these fires connect, Cold pathogens cannot invade. When deficient, Cold penetrates. Although Cold usually attacks the Heart first, here it descends to the scrotum because Kidney Qi is deficient and cold, and the Spleen Meridian holds dampness. Cold and dampness share affinity, accelerating descent when encountering dampness. This condition involves sudden phlegm accumulation like wind and rain, an acute illness rather than chronic. It resembles hernia but is distinct. Treatment should not focus on hernia but on tonifying Heart and Kidney deficiency, warming Pericardium and Ming Men fire, and eliminating Spleen dampness. Ben Tun will heal spontaneously. Use An Tun Dan:
Ren Shen (Ginseng Root, 5 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Rou Gui (Cinnamon Twig, 1 qian), Shan Yao (Dioscorea Rhizome, 1 liang), Ba Ji Tian (Morinda Root, 5 qian), She Chuang Zi (Cnidium Fruit, 3 qian), Fu Zi (Prepared Aconite Root, 5 fen), Fu Ling (Poria, 3 qian), Yuan Zhi (Polygala Root, 1 qian), Gan Cao (Licorice Root, 1 qian). Decoct in water and take orally. Peace returns after one dose, full recovery after two.
This formula tonifies Heart and Kidney, ensuring sufficient Qi. Subsequent use of hot herbs like Gui and Fu helps control violent Qi movements for easier elimination. Given the urgency of the pathogen, overly harsh medicines pose risks of excessive rigidity. Adding Gan Cao provides moderation, balancing urgency and slackness, preventing conflict and failure.
Can Ling Gui Zhu Tang is also suitable for this condition.
Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang), Rou Gui (Cinnamon Twig, 2 qian), Ren Shen (Ginseng Root, 5 qian), Ban Xia (Pinellia Tuber, 5 fen), Fu Ling (Poria, 2 qian). Decoct in water and take orally.
Frequent urination, retracted testicles, and worsened pain in cold weather indicate Cold Stagnation in the Bladder. Bladder fluid transformation relies on Ming Men Fire. One might think Bladder cold originates from Ming Men cold, but the Bladder can independently become cold. This condition often results from sitting in cold-damp places, allowing Cold to invade the Bladder and resist dispersion. Even with Ming Men Fire, transformation fails because Ming Men Fire transforms internal dampness but not external dampness. External dampness remaining in the Bladder clashes with Ming Men True Fire. Pathogen excess and Zheng (Righteous) Qi deficiency cause pain. Treatment must directly expel Bladder cold-dampness, allowing testicles to relax and pain to cease. Use Pi Han Dan:
Rou Gui (Cinnamon Twig, 3 qian), Fu Ling (Poria, 5 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Gan Cao (Licorice Root, 1 qian), Ju He (Tangerine Seed, 3 qian), Li Zhi He (Lychee Seed, 3 pieces, crushed). Decoct together in water and take orally.
Symptoms decrease after two doses, fully resolving after four.
Rou Gui serves as the sovereign herb, warming Ming Men Fire and dispelling Bladder Cold. Bai Zhu and Fu Ling promote diuresis. Ju He and Li Zhi He stabilize testicular pain. Without Rou Gui's guidance, these herbs cannot directly penetrate and resolve cold stagnation.
Shu Gui Tang is also excellent for this condition.
Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang), Rou Gui (Cinnamon Twig, 1 qian). Decoct in water and take orally.
