A woman suddenly experiences massive vaginal bleeding, her eyes go dark, and she faints to the ground. People attribute this to excessive fire causing blood movement; however, this is not excess fire (Excess Fire), but deficiency fire (Deficiency Fire). When people see uterine bleeding (Metrorrhagia), they often use astringent drugs. Although these may yield temporary results, if deficiency fire is not nourished, it easily causes further冲击 (impact), leading to recurrence immediately after stopping. Cases that cannot be cured throughout the year or month are due to the use of such hemostatic drugs for metrorrhagia, which absolutely should not be used. One must employ hemostasis within the context of tonification. The formula used is Gu Ben Zhi Beng Tang (Root-Firming Metrorrhagia-Stopping Decoction): Shu Di Huang (Prepared Rehmannia Root, 1 liang), Bai Zhu (Atractylodes Macrocephala Rhizome, 1 liang), Huang Qi (Astragalus Membranaceus Root, 3 qian), Ren Shen (Ginseng Radix, 3 qian), Dang Gui (Angelica Sinensis Radix, 5 qian), Chao Hei Gan Jiang (Charred Dried Ginger, 2 qian). Decoct in water and take orally. After one dose, bleeding stops; after ten doses, it never recurs. If one fears the heavy dosage, reducing it by half makes the medicinal power too weak to stop bleeding.
This formula does not directly aim to stop bleeding but instead tonifies blood; moreover, it not only tonifies blood but also tonifies qi; furthermore, it tonifies qi and simultaneously tonifies fire (Mingmen Fire). Why? When metrorrhagia leads to darkness in the eyes and fainting, it indicates that blood has been largely lost, leaving only a thread of qi. If one does not urgently tonify qi but instead tonifies blood first, tangible blood cannot be generated quickly, while intangible qi will surely dissipate. This is why we do not tonify blood first but save qi first. However, tonifying qi without tonifying blood means blood cannot be easily generated. Tonifying blood without tonifying fire means blood will stagnate and cannot generate rapidly with the qi. Moreover, Gan Jiang (Dried Ginger) guides blood back to its vessels; it contains astringency within tonification. Therefore, tonifying qi and blood drugs are used together.
This condition can also be treated with Bu Xu Ning Xue Tang (Tonifying Deficiency Calming Blood Decoction): Dang Gui (Angelica Sinensis Radix, 5 qian), Shu Di Huang (Prepared Rehmannia Root, 1 liang), Huang Qi (Astragalus Membranaceus Root, 1 liang), Gan Cao (Licorice Root, 1 qian), Chao Hei Jing Jie (Charred Schizonepeta Spike, 3 qian). Decoct in water and take orally. Bleeding stops after one dose; full recovery occurs after four doses.
An elderly woman suffers from metrorrhagia, with darkened vision and fainting. People believe this is due to extreme deficiency in the elderly caused by imprudent sexual activity. However, who would know it is actually due to excessive talking damaging qi and improper diet? Elderly women should ideally restrict their diet and remain silent so that qi is not damaged and spirit remains vigorous. Unfortunately, an elderly woman hearing joyful news becomes happy and praises, refusing to keep her mouth closed, inevitably saying things that are inappropriate. Furthermore, since she has chronic ailments, how can she refrain from speaking? Thus, once triggered, it becomes uncontrollable. Elderly women have depleted blood; because qi is extremely deficient, it cannot generate blood. Moreover, excessive talking consumes qi, making it impossible to assist qi in generating blood. As qi declines further, blood becomes difficult to grow. Consequently, the Ren Mai (Conception Vessel) and Chong Mai (Penetrating Vessel) open widely, making it impossible to avoid metrorrhagia. The treatment method must stop the blood. Yet, paradoxically, the more one tries to stop the blood, the more there is, because deficient qi fails to control the blood. The formula used is Zhu Qi Lian Xue Tang (Assisting Qi Gathering Blood Decoction): Bai Zhu (Atractylodes Macrocephala Rhizome, 2 liang, stir-fried with soil), Huang Qi (Astragalus Membranaceus Root, 4 liang, stir-fried with vinegar), San Qi Powder (Notoginseng Radix et Rhizoma Powder, 3 qian). Decoct in water and take orally. After one dose, blood volume decreases and stops slightly; after two doses, bleeding stops; after four doses, full recovery occurs.
This formula tonifies qi without tonifying blood, because qi can stop bleeding. Adding vinegar-stir-fried Huang Qi and Bai Zhu utilizes the sour flavor to rescue blood. Adding San Qi is because it can sever bleeding. However, multiple doses are necessary for cure because the elderly woman suffers from blood deficiency and qi decline; only substantial tonification can stop the source of dissipation of primordial yang, allowing qi to flourish and generate blood. Nevertheless, this formula can temporarily stop the elderly woman's bleeding but cannot sustainably strengthen her qi. Alternatively, use the previous formula removing San Qi and adding more Dang Gui (Angelica Sinensis Radix), taking Bu Xue Tang (Blood-Tonifying Decoction) morning and evening, combining therapies to achieve results.
Another elderly woman suffers from metrorrhagia with symptoms identical to the previous case. People assume it is merely elderly deficiency, failing to realize it is due to deficiency combined with imprudent sexual conduct. Women past fifty have scarce Tian Gui (Heavenly Water/Reproductive Essence) and should ideally abstain from sexual activity. If arousal occurs and acts are concluded hastily, kidney fire (Kidney Fire) is not greatly stirred. However, if engagement is passionate and vigorous, like in youth, rarely does one avoid opening the Blood Chamber (Xue Shi) wide, leading to rupture and collapse. The formula used is Dang Gui Bu Xue Tang (Angelica Astragalus Decoction) with modifications: Huang Qi (Astragalus Membranaceus Root, 1 liang), Dang Gui (Angelica Sinensis Radix, 1 liang), San Qi Gen Mo (Notoginseng Root Powder, 3 qian), Sang Ye (Mulberry Leaf, 14 slices). Decoct in water and take orally. Bleeding stops after two doses; no recurrence after four doses. However, sexual abstinence is mandatory; relapse guarantees severe illness.
Bu Xue Tang is a divine agent for dual tonification of qi and blood; San Qi Gen is a holy medicine for stopping bleeding. Adding Sang Ye nourishes yin in the kidneys and offers astringent wonders. But since the elderly woman's yin essence is already depleted, using this formula to stop temporary leakage indeed has miraculous effects, though one cannot blame it for not providing permanent continuity, as the ingredients for tonifying essence are few. After four doses of this formula, add Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Shu Di Huang (Prepared Rehmannia Root, 1 liang), Shan Yao (Dioscorea Oppata Rhizome, 4 qian), Mai Dong (Ophiopogon Japonicus Tuber, 3 qian), Bei Wu Wei Zi (Schisandra Chinensis Fruit, 1 qian). Taking this for three months will eliminate metrorrhagia completely.
Bi Xue Tang (Closing Blood Decoction) is also effective for this condition: Ren Shen (Ginseng Radix), Bai Zhu (Atractylodes Macrocephala Rhizome) (each 1 liang), San Qi Gen Mo (Notoginseng Root Powder, 3 qian), Bei Wu Wei Zi (Schisandra Chinensis Fruit, 2 qian). Decoct in water and take orally. Metrorrhagia stops after one dose. Reduce Ren Shen (Ginseng Radix) to 5 qian, add Shu Di Huang (Prepared Rehmannia Root, 1 liang), Shan Zhu Yu (Cornus Officinalis Fruit, 5 qian), Mai Dong (Ophiopogon Japonicus Tuber, 5 qian). Take for another four doses for full recovery.
A young woman conceived only three months ago suffered metrorrhagia, and the fetus fell with it. People attribute this to trauma from jolting or falling, but who knows it is due to imprudent sexual intercourse? Sexual intercourse is normal for young women; why does it cause metrorrhagia here? It is because of her qi decline. Women with declining qi cannot endure prolonged intercourse; prolonged activity leads to seminal emission. Excessive loss of essence further prevents qi from controlling blood. Moreover, prolonged activity stirs deficiency fire internally, opens the essence gate, and prevents the blood chamber from closing. Thus, the fetus cannot be secured, internal and external forces move together, and how can blood remain contained? Treatment should primarily focus on tonifying qi, with minor additions of hemostatic agents. The formula used is Gu Qi Tang (Qi-Securing Decoction): Ren Shen (Ginseng Radix, 5 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Dang Gui (Angelica Sinensis Radix, 3 qian), Shu Di Huang (Prepared Rehmannia Root, 5 qian), Fu Ling (Poria, 2 qian), Gan Cao (Licorice Root, 1 qian), Du Zhong (Eucommia Bark, 3 qian), Shan Zhu Yu (Cornus Officinalis Fruit, 2 qian), Yuan Zhi (Polygala Root, 1 qian), Wu Wei Zi (Schisandra Chinensis Fruit, 10 grains). Decoct in water and take orally. Bleeding stops after one dose; full recovery after ten consecutive doses.
This formula secures qi and simultaneously tonifies blood; lost blood can regenerate quickly, and impending loss can be fully retained. For any metrorrhagia caused by deficiency, this formula is suitable for general treatment, not just for post-abortion bleeding. This formula does not explicitly aim to stop bleeding, yet all hemostatic elements are included within it, allowing for general application. Ren Shen (Ginseng Radix, 3 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 5 qian), Fu Ling (Poria), Shan Yao (Dioscorea Oppata Rhizome), Mai Dong (Ophiopogon Japonicus Tuber) (each 3 qian), Yuan Zhi (Polygala Root, 5 fen), Du Zhong (Eucommia Bark), Shan Zhu Yu (Cornus Officinalis Fruit) (each 2 qian), E Jiao (Donkey-Hide Gelatin, 3 qian), Gan Cao (Licorice Root, 1 qian). Decoct in water and take orally. Cure after one dose.
A woman experiences continuous bleeding upon sexual contact. Although not as severe as metrorrhagia, if unresolved until year-end, it damages both qi and blood, potentially leading to blood depletion and amenorrhea. Such diseases arise from engaging in sexual intercourse during menstruation, where semen impacts the blood vessels. Blood vessels should not be injured by semen. Conception occurs when blood vessels are clear. If intercourse happens at the onset of menstruation when blood flow is strong, and semen is ejaculated as blood exits, the discharged blood retreats and contracts inward. Since conception cannot occur, accumulated essence transforms into blood. Upon subsequent sexual contact, lustful qi stirs this old essence; the two qi interact, and as essence desires to exit, blood follows. Treatment must unblock the qi of the uterus and fallopian tubes, guide the essence outward, and supplement with drugs that fill essence and tonify qi, thereby repairing vessel damage. The formula used is Yin Jing Zhi Xue Tang (Guiding Essence Stopping Blood Decoction): Ren Shen (Ginseng Radix, 5 qian), Bai Zhu (Atractylodes Macrocephala Rhizome, 1 liang), Fu Shen (Poria with Hostwood, 3 qian), Che Qian Zi (Plantago Seed, 3 qian), Huang Bo (Phellodendron Bark, 5 fen), Chao Hei Gan Jiang (Charred Dried Ginger, 1 qian), Shu Di Huang (Prepared Rehmannia Root, 1 liang), Shan Zhu Yu (Cornus Officinalis Fruit, 5 qian), Chao Hei Jing Jie (Charred Schizonepeta Spike, 3 qian). Decoct in water and take orally. Cure after four consecutive doses; no recurrence after ten doses.
This formula uses Ren Shen and Bai Zhu to tonify qi, and Shu Di Huang and Shan Yao to tonify essence. Once qi and essence are robust, blood vessels flow naturally. Adding Fu Shen and Che Qian Zi benefits the urinary orifice; benefiting the urinary orifice also benefits the blood orifice. Huang Bo enters the blood vessels directly to draw out stored essence from the vessel openings. Jing Jie (Schizonepeta Spike) guides stagnant blood out of the vessels. Charred Gan Jiang seals the vessel openings. This formula possesses subtle harmonizing wisdom, thus eliminating old ailments and removing stale residues. However, after taking this medicine, sexual abstinence for three months is required; otherwise, broken parts will suffer further injury, and tonified parts will be damaged again. Otherwise, only temporary effects can be achieved. Be cautious!
Jie Liu Dan (Stream-Intercepting Pill) is also very effective for this condition: Fu Ling (Poria), Chao Hei Jing Jie (Charred Schizonepeta Spike), Che Qian Zi (Plantago Seed) (each 3 qian), Niu Xi (Achyranthes Bidentata Root), Ren Shen (Ginseng Radix) (each 3 qian), Shu Di Huang (Prepared Rehmannia Root, 1 liang), Bai Zhu (Atractylodes Macrocephala Rhizome, 1 liang), Qi Ai (Artemisia Argyi Herb, 1 qian), Rou Gui (Cinnamon Bark, 3 fen). Decoct in water and take orally. Full recovery after one dose.
A woman with deep depression, dry mouth and thirst, vomiting, and acid regurgitation suffers from downward metrorrhagia. Treating with fire-clearing methods yields inconsistent results because it involves Liver Qi Stagnation (Liver Qi Stagnation). The Liver stores blood; qi stagnation should lead to blood stagnation, yet it causes metrorrhagia? The Liver nature is urgent; qi stagnation makes this urgency worse. Urgency prevents blood storage. Treatment should prioritize resolving stagnation. However, merely resolving stagnation without soothing the Liver causes Liver Qi to expand excessively, intensifying Liver Fire, making it impossible to stop bleeding. The formula used is Ping Gan Zhi Xue Tang (Soothing Liver Stopping Blood Decoction): Bai Shao (White Peony Root, 2 liang), Bai Zhu (Atractylodes Macrocephala Rhizome, 1 liang), Dang Gui (Angelica Sinensis Radix, 1 liang), Chai Hu (Bupleurum Root, 1 qian), San Qi Gen Mo (Notoginseng Root Powder, 3 qian), Gan Cao (Licorice Root, 2 qian), Dan Pi (Moutan Cortex, 3 qian), Jing Jie (Schizonepeta Spike, 2 qian), Sheng Di (Rehmannia Glutinosa Root, 3 qian). Decoct in water and take orally. Vomiting stops after one dose; dryness and thirst resolve after two doses; metrorrhagia heals spontaneously after four doses.
Bai Shao soothes the Liver; combined with Chai Hu, stagnation resolves completely. Bai Zhu benefits the waist and umbilical region, preventing blood accumulation. Jing Jie unblocks meridians, allowing blood to return joyfully. Dan Pi cools bone marrow heat; Sheng Di clears visceral inflammation. Dang Gui and San Qi perform hemostasis within blood tonification. Naturally, as stagnation disperses, bleeding stops.
Shu Gan Cang Xue Tang (Soothing Liver Storing Blood Decoction) is also excellent for this condition: Bai Shao (White Peony Root, 1 liang), Xiang Fu (Cyperus Rotundus Rhizome), Jing Jie (Schizonepeta Spike), San Qi Gen Mo (Notoginseng Root Powder) (each 3 qian), Chen Pi (Citrus Reticulata Peel, 5 fen), Gan Cao (Licorice Root, 1 qian), Dang Gui (Angelica Sinensis Radix), Bai Zhu (Atractylodes Macrocephala Rhizome) (each 5 qian), Bai Jie Zi (White Mustard Seed, 1 qian). Decoct in water and mix for oral administration.
A woman falls from height or suffers sprains/injuries, causing evil blood to rush downward resembling metrorrhagia. Treating this as metrorrhagia with astringents harms rather than helps. Symptoms include pain upon pressure; long-term suffering leads to sallow complexion and emaciation. Treatment must promote blood circulation and remove stasis, invigorate blood and relieve pain, so bleeding stops naturally. If stasis and pain are not resolved, using tonics and astringents causes stasis to attack internally, pain persists, new blood fails to generate, and old blood wreaks havoc. The formula used is Zhu Yu Zhi Beng Tang (Expelling Stasis Stopping Metrorrhagia Decoction): Da Huang (Rhubarb Root, 3 qian), Sheng Di (Rehmannia Glutinosa Root, 1 liang), Dang Gui Wei (Angelica Sinensis Radix Tail, 5 qian), Bai Gui Ban (Tortoise Plastron, 3 qian), Shao Yao (Peony Root, 2 qian), Dan Pi (Moutan Cortex, 1 qian), Zhi Ke (Aurantii Fructus, 5 fen), Tao Ren (Peach Kernel, 10 grains). Decoct in water and take orally. Pain lightens after one dose; stops after two doses; bleeding ceases completely after three doses; no need for fourth dose.
This formula assists descending action within blood-invigorating herbs, thus expelling stasis like sweeping and stopping bleeding miraculously. These injuries from falls or jolts are not caused by internal damage; the root is intact. Removing the branch disease suffices; why worry about the root and tonify internally?
Ling Gui San Xue Tang (Spirit Turtle Dispelling Blood Powder) is also very effective: Bai Gui Ban (Tortoise Plastron, 1 liang), Sheng Di (Rehmannia Glutinosa Root, 1 liang), Da Huang (Rhubarb Root, 1 qian), Dan Pi (Moutan Cortex, 3 qian), Hong Hua (Carthamus Tinctorius Flower, 2 qian), Tao Ren (Peach Kernel, 14 grains). Decoct in water and take orally. Lightens after one dose; cures after two doses.
Some women experience menstrual flow immediately upon sexual intercourse, resembling metrorrhagia. People think the uterus/fallopian tubes are injured and touched, stirring blood. Who knows it is due to Heat failing to secure the Uterus and Blood Sea (Chong Mai)? The Uterus is below the fallopian tubes, and the Blood Sea is above. The Blood Sea refers to the Chong Mai (Penetrating Vessel). Cold in Chong Mai causes blood deficiency; Heat in Chong Mai causes blood boiling. Metrorrhagia is precisely Heat in Chong Mai. If Chong Mai is hot, blood should bleed daily; why wait for intercourse? Because Spleen and Liver are healthy. A healthy Spleen controls blood; a balanced Liver stores blood. Before intercourse, Ministerial Fire and Sovereign Fire are quiet. Even if Chong Mai is hot alone, blood does not leak. During intercourse, the Uterus opens wide, Ministerial and Sovereign Fires move together, stirring the essence chamber, causing the Blood Sea to overflow uncontrollably. The Liver wants to store blood but cannot; the Spleen wants to control blood but fails. Thus, menstruation arrives with intercourse, responding swiftly like sound echoing. Treatment requires three months of sexual abstinence, followed by yin-nourishing and fire-descending drugs to cool the Blood Sea, curing lifelong illness in six months. The formula used is Qing Hai Wan (Sea-Clearing Pill): Shu Di Huang (Prepared Rehmannia Root, 1 jin), Sang Ye (Mulberry Leaf, 1 jin), Bai Zhu (Atractylodes Macrocephala Rhizome, 1 jin), Xuan Shen (Scrophularia Root, 1 jin), Shan Zhu Yu (Cornus Officinalis Fruit, 8 liang), Bei Wu Wei Zi (Schisandra Chinensis Fruit, 3 liang), Mai Dong (Ophiopogon Japonicus Tuber, 10 liang), Sha Shen (Glehnia Root, 10 liang), Di Gu Pi (Lycium Bark, 10 liang), Dan Pi (Moutan Cortex, 10 liang), Bai Shao (White Peony Root, 1 jin), Long Gu (Fossilized Bones, Vinegar Calcined, 2 liang), Shan Yao (Dioscorea Oppata Rhizome, 10 liang), Shi Hu (Dendrobium Stem, 8 liang). Grind into fine powder, form pills with honey. Take 5 qian with plain boiled water morning and evening daily. Full recovery after six months.
This formula tonifies yin without risk of floating, contracts blood without cold harm. Day-by-day gains seem small, but month-by-month they accumulate, subtly transforming the body. The Uterus becomes cool, and the Blood Sea secures itself. If one does not treat the root, merely applying ash, alum, Coptis, and Chinese Gall externally to hidden areas, I fear sealing will only increase flow.
Qing Huo Gui Jing Tang (Clearing Fire Returning to Meridians Decoction) is also effective: Ren Shen (Ginseng Radix), Bai Shao (White Peony Root) (each 1 liang), Jiu Zong Lu (Old Palm Fiber, Charred, 2 qian), Huang Bo Mo (Phellodendron Bark Powder, 2 qian), Gan Cao (Licorice Root, 1 qian), San Qi Gen Mo (Notoginseng Root Powder, 3 qian). Decoct in water and mix for oral administration. Cure after ten doses; full recovery after twenty doses. However, three months of sexual abstinence are mandatory; otherwise, relapse is inevitable.
