Treats all types of Gucao Feng (Bone Trench Wind, a severe form of periodontal disease involving bone necrosis). As well as conditions such as Ru Yan (Breast Cancer), Luo Li (Scrofula), Tan He Heng Sheng (Horizontal proliferation of Phlegm Nodules), Fei Yong (Lung Abscess), Xiao Chang Yong (Small Intestine Abscess), and Liu Zhu (Metastatic Abscesses).
Xi Huang (3 fen) Xiang Ru (1 liang, stir-fried with Deng Xin Cao [Juncus effusus] to remove oil) Mo Yao (1 liang, processed similarly) Zhen She Xiang (1.5 qian)
Grind all into fine powder. Take 1 liang of millet rice, pound it into a paste to form pills the size of mung beans. Dry in the sun; avoid baking. Take 3 qian per dose, washed down with warm aged wine until slightly intoxicated. Cover with blankets to induce sweating. After waking up from the sweat, the abscess will dissipate and pain will cease naturally.
The condition is named "Zou Ma" (Running Horse) to indicate its rapid progression; failure to treat promptly leads to death. Teeth belong to the Kidney and connect with the Stomach. The Kidney governs the Yuan Qi (Primary Qi) of the whole body. Whenever one suffers from accumulated heat and fire toxins, Gan Qi (Gangrene Qi) rushes to the Upper Jiao. It may arise after measles or smallpox, or during febrile diseases caused by Shang Han (Cold Damage) and miscellaneous disorders. It can also result from long-term overconsumption of Yang-aiding hot medicines or poisoning. Initially, the breath is extremely foul, called Chou Xi (Foul Breath). Subsequently, teeth turn black, called Beng Sha (Collapsing Sand). In severe cases, gums rot, called Kui Cao (Rotting Trench). Hot blood spurts out, called Xuan Lu (Exposed Roots). In extreme cases, teeth fall out, called Fu Gen (Decayed Roots). Once teeth fall out, they do not grow back. Thus, this condition requires prompt treatment.
For initial stages of Ya Gan (Gum Gangrene) presenting with blackening, rotting, foul odor, and bleeding, Lu Hui Xiao Gan Yin (Aloe Vera Gangrene-Resolving Decoction) is appropriate. For those with Spleen and Stomach deficiency, Ren Shen Fu Ling Zhou (Ginseng and Poria Porridge) should be taken concurrently, and Shen Gong Dan (Divine Efficacy Powder) blown into the lesion. If caused by residual toxins after smallpox/rash, Qing Gan Jie Du Tang (Clear Gangrene and Detoxify Decoction) is suitable. For mild external symptoms, Ren Zhong Bai San (Human Tooth Ash Powder) is applied externally. If hard, cyanotic, gradually rotting, penetrating the cheek, and causing tooth mobility, Lu Hui San (Aloe Vera Powder) is applied externally. Seeing red blood flow in gum gangrene is auspicious. However, if stubborn flesh does not detach, necrotic tissue opens and swells, emitting a foul odor, accompanied by persistent fever, these are untreatable signs.
There are five untreatable signs of Gum Gangrene: Teeth falling out without bleeding—untreatable. Cheek collapse and lip rupture—untreatable. Black necrosis that does not slough off—untreatable. Abnormal foul odor—untreatable. No response to medication—untreatable.
