Lianqiao (Forsythiae Fructus), Zhike (Aurantii Fructus), Banxia (Pinelliae Rhizoma), Houpo (Magnoliae Officinalis Cortex), Yujin (Curcumae Radix), Caodoukou (Cardamomi Fructus), Huashi (Talcum), Fulingpi (Poriae Sclerotium Exocarpium) and other herbs. If one is affected by turbid summerheat combined with food stagnation, resulting in epigastric stuffiness and fullness, bloating, nausea, and vomiting, accompanied by abdominal stiffness and pain; it is also appropriate to clear and drain the middle jiao (middle burner). Examples include Guanghuoxiang (Patchouli Herba), Chuan Yujin (Curcumae Radix), Huanglian (Coptidis Rhizoma), Zhike (Aurantii Fructus), Baiko (Amomi Fructus Rotundus), Houpo (Magnoliae Officinalis Cortex), Muxiang juice (Aucklandiae Radix Juice), Sheng Zharonrou (Raw Hawthorn Fruit Flesh), Laifuzi (Raphani Semen) and similar herbs. If the pathogen of summerheat-dampness has not been cleared, and tonifying agents are mistakenly administered, leading to chest and diaphragm distension, epigastric stuffiness, hardness, and pain, nearly forming a Jie Xiong syndrome (chest binding syndrome); it is also appropriate to treat it with the Xi Xin Tang method (Draining Heart Decoction method). One may add Er Chen Tang (Two-Cured Decoction), Zhishi (Aurantii Fructus Immaturus), Houpo (Magnoliae Officinalis Cortex), Huanglian (Coptidis Rhizoma), Zharonrou (Hawthorn Fruit Flesh), Yujin (Curcumae Radix), Laifuzi (Raphani Semen), Muxiang juice (Aucklandiae Radix Juice) and similar herbs. Additionally, Wang Mengying treated the wife of Chen Chitang for phlegm-heat obstruction using Xiao Xian Xiong Tang (Minor Chest Binding Decoction) combined with Wen Dan Tang (Warming Gallbladder Decoction) and Xue Geng Tang (Snow Soup), adding Xuanfuhua (Inulae Flos) and Xiebai (Allii Macrostemonis Bulbus). (See Wang Mengying's *Medical Cases*) This demonstrates the subtlety of administering correct medication against symptoms and flexible application.
(Also known as Cold Damage complicated with Epigastric Pain)
[Etiology] A history of Liver-Stomach Qi pain. External contraction of Wind-Cold. Triggered and manifested. There are distinctions regarding exterior-interior, upper-lower, left-right, and differences in Qi-Blood, deficiency-excess, cold-heat.
[Symptoms] Headache, feverish body sensation. Aversion to cold, absence of sweating. Chest and epigastrium fullness and pain. Nausea, vomiting acid. Or pain in both hypochondria. Or abdominal distension and pain. Or lower abdominal pain. Tongue coating white and slippery. Or yellow-white mixed. Or gray-white not dry. Even or yellow and turbid.
[Pulse] Left pulse floating and tight. Right pulse wiry and rapid. Or floating or deep. Even deep, wiry, and choppy. These are all signs of Cold Damage complicated with internal pain.
[Treatment] First, regulate Qi and induce sweating. Use Xiang Su Cong Chi Tang (Perilla and Scallion Decoction) added with Yanhusuo (Corydalis Rhizoma) and Ruxiang (Olibanum). Remove exterior pathogens to stop pain. If the exterior pathogen is removed but pain persists, there must be congealed phlegm and hidden fluid retention. Or there may be retained food and blood stasis; one must clearly distinguish the root cause. Carefully examine the location to apply treatment. If chest pain radiates to both hypochondria in a string-like manner, it belongs to Phlegm-Qi mutual knotting. Initially use Chai Hu Xian Xiong Tang (Bupleurum Minor Chest Binding Decoction) added with Ruxiang (Olibanum) and Moyao (Myrrha). Harmonize and resolve depression/knotting to stop pain. Subsequently use Da Chai Hu Tang (Major Bupleurum Decoction) to administer Kong Xian Dan (Control Fluid Elixir) slowly purging phlegm-fluids to remove the root. If ineffective, immediately use Juan Yin Wan Ling Tang (Fluid-Purging Ten Thousand Spirit Decoction) to administer Zi Jin Pian (Purple Gold Tablets) rapidly removing phlegm-fluids. For hard and painful epigastrium, even with palpable masses, where pain cannot bear pressure, it belongs to Retained Food obstructing Qi mechanism. Initially use Shen Shu Tang (Divine Skill Decoction) added with Ruxiang (Olibanum) and Moyao (Myrrha) to warm the middle and disperse stagnation to relieve pain. Subsequently use Zhishi Dao Zhi Tang (Immature Aurantii Guiding Stagnation Decoction) added with Yanhusuo (Corydalis Rhizoma) to purge retained stagnation to remove the root. If ineffective, immediately use Liu Mo Yin Zi (Six Mortars Infusion) to administer Dang Gui Dao Qi San (Angelica Guiding Qi Powder) descending Qi and attacking stagnation. For pain that cannot bear pressure, yet feels soft upon pressing, or even extreme pain resembling madness, or fainting and loss of consciousness, it belongs to Blood Stasis coagulation. Mild cases: Wu Ren Ju Pi Tang (Five Seeds Tangerina Peel Decoction) combined with Dai Di Dang Wan (Substitute Leech Pill) lubricating and unblocking stasis to stop pain. Severe cases: Tao Ren Cheng Qi Tang (Persica Seed Descending Qi Decoction) vigorously attacking heat-stasis to remove the root.
If Liver Fire scorches the Stomach, causing hunger without desire to eat, vomiting roundworms after eating, even irritability and fainting, it belongs to Roundworm Jue (Jue Syndrome caused by Ascaris). Initially use Lian Mei An Hui Tang (Forsythia and Mume Calming Roundworm Decoction) clearing Liver heat and stopping pain to stabilize Jue.
Subsequently use Xue Geng (Snow Soup) to swallow Geng Yi Wan (Change Clothing Pill), purging Liver heat and killing parasites to remove the root. Only if repeated purgative treatments result in increased pain, it belongs to Deficiency Pain. For Cold bias: Jia Wei Xiao Jian Zhong Tang (Modified Minor Construct the Middle Decoction) doubling Dang Gui (Angelicae Sinensis Radix) to gently warm Liver and Spleen for tonification. For Heat bias: Si Wu Jiang Fu Tang (Four Substances Red Covering Decoction) moistening and nourishing blood collaterals to soothe. Even then, guide fire back to its source, such as Jia Wei *Jin Gui* Shen Qi Tang (Modified *Essential Prescriptions from the Golden Cabinet* Kidney Qi Decoction) drawing Qi back to the Kidney. Or Xin Jia Ba Wei Di Huang Tang (Newly Added Eight Ingredient Rehmannia Decoction) etc., can be used酌情 (discretionarily) according to the disease. However, these are rare. At the bedside, deliberate carefully; do not experiment lightly.
In short, pain implies blockage; patency implies no pain. Use Warm Patency, Cool Patency, Soothing Patency, or Attacking Patency. Assess symptoms according to time, formulate prescriptions according to constitution. Ensure Qi and Blood flow smoothly.
Then suppressed pathways become patent, knotted areas become open, and pain naturally ceases. These are the essential methods for Cold Damage complicated with pain.
[Xiu's Note] Whenever there is chronic pain in the chest, hypochondria, epigastrium, or abdomen, triggered and manifested by external contraction aggravating old diseases, Yu Jun uses Xiang Su Cong Chi Tang (Perilla and Scallion Decoction) added with Yanhusuo (Corydalis Rhizoma) and Ruxiang (Olibanum). It can both release the exterior and relieve pain, promote Qi circulation and activate Blood, move meridians and unblock collaterals, treating both exterior and interior. The formula is particularly light and stable. Because when Exterior Qi is vented, Interior Qi also becomes unblocked, and pain will slightly ease. Even for abdominal pain due to latent phlegm-fluids in the epigastrium, foot pain due to Kidney deficiency unable to support the body, lower body flaccidity and bone/joint pain due to Kidney decline and Wind invasion, although the disease originates internally, the pain actually lies in the meridians and collaterals. Therefore, exterior-releasing medicines generally do not harm the primary disease. Secondly, for various pains in the chest, hypochondria, shoulders, and back, although syndromes vary, they are all diseases of the Yang divisions. Even with Cold Damage exterior signs, if pain is unbearable, it is acceptable to treat the pain concurrently. Adding Yanhusuo (Corydalis Rhizoma) and Ruxiang (Olibanum) to this formula is excellent for pain relief and carries no risk of drawing pathogens into the Three Yin divisions. Thirdly, for pains in the waist, umbilicus, and lower abdomen, although they are ailments of the Yin divisions, since there are exterior signs, one must first release the exterior. After the exterior is resolved, treat the interior. Yu Jun clearly distinguishes the causes of accompanying Phlegm, Fluid, Food, Stasis, Parasites, and Deficiency. With clear symptom differentiation, prescriptions will be effective. Truly grasping the essential secret of "patency implies no pain."
[Lian's Verification] For Wind-Cold complicated with various pain syndromes, Yu Shi seeks the source at the bedside and administers medication against symptoms. The methods are indeed mostly appropriate. However, one must refer to the chapters on accompanying Food, Phlegm, Fluid, Stuffiness, and Blood above, to respond flexibly to the disease. Regarding the method of calming roundworms and stopping pain, following Yu Shi's treatment works for mild cases but fails for severe ones. One must use Chen Xiang Zhi Zhen Wan (Agarwood Precious Pill) (Chenxiang (Aquilariae Lignum Resinatum), Guangmuxiang (Aucklandiae Radix), Gong Dingxiang (Caryophylli Flos) each 4 qian, Guangpi (Citri Reticulatae Pericarpium), Qingpi (Citri Reticulatae Pericarpium Viride), Wuyao (Aucklandiae Radix), Ezhu (Curcumae Rhizoma), Bashuang (Crotonis Semen Preparatum), Chuanlian (Coptidis Rhizoma), Binglang (Arecae Semen) each 1 liang, formed into pills with Shenqu paste, take 3-5 pills each time, delivered with light ginger-salt water or rose flower water, excels in treating nine types of stomach pain, all Liver-Stomach Qi pains, hypochondriac distension, vomiting and reverse flowing, phlegm-Qi and food stagnation syndromes, killing and expelling parasites, having special merit.) to finally expel the worms downward. Once worms are expelled, pain stops. If one chronically suffers from Head Wind (chronic headache), occasionally contracting Wind-Cold, often after taking Xiang Su Cong Chi Tang (Perilla and Scallion Decoction), sweating occurs after 1-2 doses, body temperature normalizes, yet headache worsens, crying out all night. This is due to excessive sweating from pungent dispersal, agitating Wind-Fire, severely damaging Blood, thus pain increases. One should use Ju Hua Cha Tiao San (Chrysanthemum Tea Regulating Powder) modified (Chu Juhua (Chrysanthemi Flos), Su Bohe (Menthae Haplocalycis Herba), Nen Sangya (Mori Ramulus), Jingjie Sui (Schizonepetae Spica), Zhi Xiangfu (Cyperi Rhizoma), Xiakucao (Prunellae Spica), Kudingcha (Ilicis Kudingchae Folium), Heyebian (Nelumbinis Nelumbo Leaf Margin) each 1 qian, Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) 0.5 qian, finely ground into powder, taken 2 qian after meals mixed with tea liquor.) Pungent-cool dispersing Wind to vent Heat. External treatment using Castor Paste Method (Bimamu (Ricini Semen), Ruxiang (Olibanum) each 0.5 fen, Shexiang (Moschus) 0.03 fen, pounded together into a cake, applied to Taiyang acupoint (Ex-HN 5); once pain settles, urgently loosen hair on the head to let air escape, then remove the medicine cake.) Or Tou Ding San Nasal Snuff Method (Xixin (Asari Radix et Rhizoma) 3 stems, Gong Dingxiang (Caryophylli Flos) 7 grains, Tianguadi (Cucumeris Fructus), Chixiaodou (Phaseoli Semen Rubrum) 7 grains, Meibing (Borneolum Syntheticum) 0.05 fen, Shexiang (Moschus) 0.1 fen, the first four ingredients ground fine first, then mixed with Borneol and Musk extremely uniformly, stored in a small-mouthed bottle, tightly sealed, instruct patient to hold clear water in mouth, snuff about one bean-sized amount into nose left or right, after some time saliva flows out and patient is relieved; if not cured, snuff again after three days.)
For chronic pain not healing, one must prevent cataracts harming the eyes. Ancient sages like Dong Yuan and Dan Xi believed the right side belongs to Dampness and Phlegm, mostly Qi deficiency, using Ban Xia Bai Zhu Tian Ma Tang (Pinellia, Atractylodes Macrocephala, and Gastrodia Decoction) (Jiang Banxia (Pinelliae Rhizoma Preparatum), Sheng Baizhu (Atractylodis Macrocephalae Rhizoma), Ming Tianma (Gastrodiae Rhizoma) each 1.5 qian, Lu Dangshen (Codonopsis Radix), Jiangzhi Chao Huang (Prepared with Ginger Juice), Zhe Fuling (Poria) Guangpihong (Citri Reticulatae Pericarpium), Liushenqu (Massa Medicata Fermentata), Zexie (Alismatis Rhizoma) each 1 qian, Zhi Cangzhu (Atractylodis Rhizoma Preparatum) 0.7 fen.) The left side belongs to Wind and Fire, mostly Blood deficiency, using Si Wu Tang (Four Substances Decoction) added with Cang'er (Xanthii Fructus), Xixin (Asari Radix et Rhizoma), Bohe (Menthae Haplocalycis Herba), Yacha (Tea Sprouts). One mainly addresses headache in fat people, mostly Wet-Phlegm rising; the other addresses headache in thin people, mostly Blood deficiency with Fire. These are indeed important words. However, based on my experience, none lack Wind. Without Wind entering, only dizziness occurs, not pain. Moreover, it is mostly Wind-Toxin, blocking beside the Sea of Marrow, invading the brain membrane collateral branches. The Brain Meridian connects to the Eyes, the Eye Meridian enters the Brain. Thus, the exit path of the disease often resolves through the eyes. If not resolved, it harms the eyes; blindness leads to sudden cure of Head Wind. Verified repeatedly without error. I treat hemicephalic wind pain, often using Dan Popogen Tang (Pale Popo Root Decoction) (Dan Popogen 3 qian, Ming Tianma (Gastrodiae Rhizoma), Manjingzi (Vittariae Spora) each 1.5 qian, Chu Juhua (Chrysanthemi Flos), Baizhi (Angelicae Dahuricae Radix) each 1 qian, Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), Muzeicao (Equiseti Hiemalis Herba) each 0.7 fen, Xiaoheidou (Sojae Semen Nigrum) 100 grains. Note: Dan Popo is commonly called Dan Qinjiamu, taste pale nature neutral, purchased at herbal shops.) Effective repeatedly in early stages. External treatment to detoxify and dispel Wind, using mild-natured substances, gently instilling. Such as Chu Juhua (Chrysanthemi Flos), Xi Yacha (Fine Tea Sprouts) brewed in water, cooled and instilled nasally. Clear orifices naturally open; orifice patency heals Head Wind spontaneously. Or use Eye-Dotting Pain Relief Method (Xiongjing (Stalactitum), Xiguaxiao (Salpetra) each 0.1 fen, Bingpian (Borneolum), Shexiang (Moschus) a little, Juhua (Chrysanthemi Flos), Yacha (Tea Sprouts) brewed in water to mix, dot into Jingming acupoint (BL1) inside the eye, male left female right, walk several steps, stops lateral and frontal headaches effectively, also instantly effective for epigastric pain.) Effect is also miraculous. Although, Head Wind syndrome often presents with Cold at the surface and Heat at the root. Furthermore, belonging to long-standing Wind-Toxin, it mostly transforms into Fire. One should use Light-Clear Venting Upper methods, such as Lingjiao He Qiao Tang (Antelope Horn and Lotus Leaf Forsythia Decoction) (Lingjiao pian (Saigae Tataricae Cornu) 1 qian, Su Bohe (Menthae Haplocalycis Herba) 0.8 fen, Qing Lianqiao (Forsythiae Fructus), Xiakucao (Prunellae Spica), Kudingcha (Ilicis Kudingchae Folium), Jiao Zhipi (Gardeniae Fructus Corticeus) each 1.5 qian, Xian Heyebian (Fresh Nelumbinis Nelumbo Leaf Margin) 3 qian, Xian Qingjuye (Fresh Chrysanthemi Indici Foli) 7 leaves.) Results are most frequent. External use of Yi Di Jin (One Drop Gold) (Renzhongbai (Unguentum Dentis Humanum), Gan Dilong (Pheretima Preparatum) each 2 qian, ground fine together, formed into pills with Sheep Gallbladder Juice, size of mustard seed, use one pill each time, dissolve in one drop of fresh well water, drip into nostrils.) Injected into nostrils frequently, achieving results especially quickly. Yu Shi only discusses accompanying Stomach Pain, which is somewhat incomplete. Initially using Xiang Su Cong Chi Tang (Perilla and Scallion Decoction) modified specifically treats Wind-Cold. Who knows that accompanying Stomach Pain is also most common in Warm Diseases. Although usually triggered by Cold, here one only treats the Heat. Because Damp-Warmth hides in the Membrane Source, Warm-Thermal hides in Blood Collaterals, brewing and steaming transformation, must transform into Fire. Hidden pathogens reach from interior to exterior, triggering chronic Stomach Pain, mostly belonging to Heat Pain. Then simply add Ruxiang (Olibanum) and Moyao (Myrrha) to the hidden-pathogen treating herbs to stop pain, Yanhusuo (Corydalis Rhizoma) and Taoren (Persicae Semen) to activate collaterals, quickly allowing hidden pathogens to emerge, and Stomach Pain will heal itself. No need to generally confuse with ordinary pain-relieving formulas. In short, patency implies no pain, this is the principle of treating pain. But the methods of patency vary. Regulate Qi to harmonize Blood, such as Shu Gan Liu Qi Yin (Soothe Liver Flow Qi Infusion) (Zhi Xiangfu (Cyperi Rhizoma), Su Ye Geng (Perillae Caulis), Yujin (Curcumae Radix), Mi Zhi Yanhusuo (Honey-Processed Corydalis Rhizoma) each 1.5 qian, Zhike (Aurantii Fructus), Qingpi (Citri Reticulatae Pericarpium Viride), Tongcao (Tetrapanacis Medulla) each 1 qian, Danggui (Angelicae Sinensis Radix) 2 qian, Wuyao (Aucklandiae Radix), Foshoupian (Fortunellae Fructus) each 0.8 fen, Congguan (Scallion Tube) 5 cun infused.) Liu Mo Tang (Six Mortars Decoction) (Chenxiang (Aquilariae Lignum Resinatum), Wuyao (Aucklandiae Radix), Zhishi (Aurantii Fructus Immaturus), Guangmuxiang (Aucklandiae Radix), Jian Binglang (Arecae Semen), wine-ground each 1 spoon, Mao Xishen (Panax Notoginseng) 2 qian decocted in soup, add little salt, mix five juices and drink, this formula minus Zhishi and Muxiang is named Si Mo Yin (Four Mortars Infusion).) Xiang Sha Da Yu Tang (Agarwood Amomum Reaching Depression Decoction) (Guangmuxiang (Aucklandiae Radix), Chun Sharen (Amomi Fructus Rotundus) each 0.7 fen, Zhi Xiangfu (Cyperi Rhizoma), Jiao Shanzhi (Gardeniae Fructus Carbonisatus), Guang Yujin (Curcumae Radix) each 2 qian, Chuanxiong (Ligustici Chuanxiong Rhizoma), Zhi Cangzhu (Atractylodis Rhizoma Preparatum) each 0.6 fen, Liushenqu (Massa Medicata Fermentata) 1.5 qian, if Damp Depression heavy, add Chiling (Polyporus) and Huashi (Talcum), if Heat Depression heavy, add Qingdai (Indigo Naturalis) and Chuanlian (Coptidis Rhizoma), if Phlegm Depression heavy, add Fuhaisshi (Arcae Concha) and Zhuli Banxia (Bamboo Juice Pinellia), if Food Depression heavy, add Zhishi (Aurantii Fructus Immaturus) and Shanzha (Crataegi Fructus), if Blood Depression heavy, add Taoren (Persicae Semen) and Honghua (Carthami Flos), although primarily regulating Qi, can also change according to pathology.) Lv'e Meihua Wan (Green-Eyed Plum Blossom Pill) (Dangshen (Codonopsis Radix), Fuling (Poria), Yizhiren (Alpiniae Oxyphyllae Semen), Sharen (Amomi Fructus Rotundus) each 3 qian, Si Zhi Xiangfu (Four-Processed Cyperi Rhizoma) 2 liang, Huashi (Talcum) 7 liang, Shanyao (Dioscoreae Rhizoma), Huangbo (Phellodendri Cortex) each 1.5 qian, Gangsong (Nardostachyos Radix et Rhizoma), Ezhu (Curcumae Rhizoma) each 5 qian, Yuanzhi (Polygalae Radix) 2.5 qian, Jiegeng (Platycodonis Radix) 1 qian, Zhi Gancao (Glycyrrhizae Radix et Rhizoma Preparatum) 0.7 fen, use Lv'e Mei (Green-Eyed Plum Flower) 3 liang, Danpi (Moutan Cortex) 8 liang decocted in soup, boil previous medicines, dry and grind into powder, honey-pill, each weighing 1 qian, wax-sealed, take one pill each time, dissolved in boiling water, Wang Mengying said: This formula uses strange medicines, dosage ratios are hard to fathom, yet efficacy is significant.) Jiu Zhi Xiangfu Wan (Nine-Processed Cyperi Pill) (Zhi Xiangfu (Cyperi Rhizoma) 14 liang, Aiye (Artemisiae Argyi Folium) 4 liang, Spring 3 days, Summer 1 day, Autumn 3 days, Winter 7 days, first Wine, second Vinegar, third Salt, fourth Child Urine, fifth Xiaohuixiang (Foeniculi Fructus) 2 liang, sixth Yizhiren (Alpiniae Oxyphyllae Semen) 2 liang, seventh Danshen (Salviae Miltiorrhizae Radix) 2 liang, eighth Ginger Juice, ninth Luoboziren (Raphani Semen) 2 liang, processed according to method into paste-pill, take 3-4 qian each time, delivered with boiling water, Wu Shuqing said: Xiangfu is the Qi-medicine in Blood, opens depression and moves Qi, while Blood self-regulates, women should take it regularly.) *Ju Fang* Ju Bao Dan (Treasure Gathering Pill), Ren Xiang Tang (Benevolent Fragrant Decoction), Xiang Sha Er Chen Tang (Agarwood Amomum Two-Cured Decoction) and similar categories. Regulate Blood to harmonize Qi, such as Qi Li San (Seven-Li Powder), Hu Po San (Amber Powder) (Sanleng (Sparganii Rhizoma), Ezhu (Curcumae Rhizoma), Chishao (Paeoniae Radix Rubra), Danpi (Moutan Cortex), Danggui (Angelicae Sinensis Radix), Shudi (Rehmanniae Radix Preparata), Guanzi Gui (Cinnamomi Ramulus), Wuyao (Aucklandiae Radix), Yanhusuo (Corydalis Rhizoma), Hupo (Electrum), Liuji Nu (Artemisiae Abrotani Herba) each 1 liang, ground fine, take 2 qian each time, Xu Shuwei said: Especially wonderful for stopping Qi-blood pain, saving many people.) Si Wu Jiang Fu Tang (Four Substances Red Covering Decoction). Si Wu Jia Er Xiang Tang (Four Substances Plus Two Fragrances Decoction) (Add Nanmuxiang (Canarii Lignum) 0.6 fen, Xiaohuixiang (Foeniculi Fructus) 0.2 fen.)
