(There are two discussions and three formulas in total.)
The Master said: There are diseases of Ben Tun (Rushing Pig), vomiting of pus, fright and terror, and fire pathogen. These four categories of diseases all arise from fright and shock.
[Supplementary Note] This is Zhongjing stating that the origin of Ben Tun lies in fright, thus also mentioning other diseases caused by fright. Ben Tun refers to a water beast. The syndrome of Ben Tun belongs to Kidney disease. The Classics state: "When Liver Wood in the East becomes diseased, it manifests as fright and alarm." Since the Liver is the mother of Fire, when the Liver is diseased, it fails to adequately generate Ministerial Fire, and the element that it normally controls may reverse to oppress it. The Liver is the son of Water; therefore, Liver disease inevitably disturbs Kidney Water, while its offspring tends to protect it. The Jueyin organ is the residence for storing blood. Fright causes Qi stagnation; Qi stagnation leads to Blood stasis. Thus, in the Jueyin chapter, it is mentioned that patients with carbuncles and pus vomiting will recover naturally once the pus is exhausted. Yangming Earth naturally fears Wood control. If Wood is aided by pathogens, it attacks Spleen Earth below, transmitting into fright and terror syndromes. Hence, the Classics say: "When seeing Liver disease, one should first strengthen the Spleen." Once Liver disease results in a lack of nourishment from Water, heat must become excessive generating wind, so the blazing fire cannot be extinguished. In summary, all these conditions stem from fright, manifesting according to the specific deficiency locations in the individual.
The Master said: Ben Tun disease begins in the lower abdomen, rushing up to the throat. During an attack, the patient feels as if death is imminent, after which it subsides and stops. All such cases arise from fright and shock.
[Supplementary Note] Fright implies being genuinely terrified by external stimuli. Due to this fear, one feels anxious and fearful, which is termed "terror" (Kong). Therefore, fright injures the Heart, and terror injures the Kidney. The Kidney is the official of strength and vigor; when injured, evil Qi becomes prevalent. The Heart is the residence of the Spirit; when injured, righteous Qi declines. Water naturally overcomes Fire. Consequently, Kidney evil desires to ascend and overwhelm the Heart, which explains why Qi rushes from the lower abdomen up to the throat. Why? Because the Shaoyin meridian circulates along the throat, making upward rush convenient along its connected pathway. Although Earth can control Water, because fright injures the Liver, Wood Qi is sufficient to overcome Earth. Furthermore, because fright injures the Heart, Fire Qi is insufficient to generate Earth. Thus, the cessation of Water Qi is merely due to its declining momentum returning, rather than truly being dammed by Yangming or Taiyin (Spleen/Earth).
Ben Tun disease, characterized by Qi rushing to the chest, abdominal pain, and alternating chills and fever, is primarily treated with Ben Tun Tang.
Ben Tun Tang Formula
Gancao (Licorice Root), Xiong (Ligusticum Chuanxiong Rhizome), Danggui (Angelica Sinensis Radix) (each 2 liang), Banxia (Pinellia Tuber) (4 liang), Huangqin (Scutellaria Root) (2 liang), Shengge (Pueraria Root) (5 liang), Shaoyao (Peony Root) (2 liang), Shengjiang (Fresh Ginger) (4 liang), Ganligen Baipi (Bark of Prunus Spinosa Root, White Part) (1 sheng).
These nine ingredients are decocted in 2 dou of water, reduced to 5 sheng. Take 1 sheng warm, three times daily and once at night.
[Supplementary Note] Qi rushing to the chest is slightly milder than rushing to the throat. However, abdominal pain clearly indicates Wood attacking Earth. Alternating chills and fever is a fundamental Shaoyang disease, as Jueyin and Shaoyang are exterior-interior pairs. Therefore, sweet herbs are used to tonify Earth to control Water; Banxia and Shengjiang dissipate accumulation; pungent-warm herbs dispel cold; bitter-cold herbs resolve heat. Danggui nourishes Ying (Nutritive) Qi, and Shaoyao alleviates pain. Any condition arising from fright is primarily treated with this formula, named after the disease.
After sweating therapy, if pricking with heated needles induces sweat, and the needle site becomes cold, presenting with nodular redness, Ben Tun will certainly develop, with Qi rushing from the lower abdomen to the heart. Moxibustion of one cone on each nodule should be performed, followed by Guizhi Jia Gui Tang.
Guizhi Jia Gui Tang Formula
Guizhi (Cinnamon Twig) (5 liang, peeled), Shaoyao (Peony Root) (3 liang), Shengjiang (Fresh Ginger) (3 liang), Gancao (Licorice Root) (2 liang, honey-fried), Dazao (Jujube Fruit) (12 pieces).
These five ingredients are decocted in 7 sheng of water over low heat, reduced to 3 sheng. Strain and take 1 sheng warm.
[Supplementary Note] Ben Tun is the evil of the North Kidney. Using heated needles to induce sweat may be improper, but how does this relate to Shaoyin to cause Ben Tun? It is because Taiyang and Shaoyin are exterior-interior partners. Cold invading the needle site causing nodular redness indicates that previous pathogens were not dispersed, and new pathogens have re-entered. Only Guizhi can subdue Kidney evil; hence, extra Guizhi is added to Guizhi Tang to release exterior wind pathogens externally and vent interior Yin cold internally. Moxibustion on each nodule eliminates swelling caused by cold.
After sweating therapy, if palpitations occur below the umbilicus, indicating impending Ben Tun, it is primarily treated with Fuling Guizhi Gancao Dazao Tang.
Fuling Guizhi Gancao Dazao Tang Formula
Fuling (Poria) (8 liang), Gancao (Licorice Root) (2 liang, honey-fried), Dazao (Jujube Fruit) (15 pieces), Guizhi (Cinnamon Twig) (4 liang).
These four ingredients are decocted in 1 dou of Ganlan Water (Agitated Water). First boil Fuling until the volume reduces by 2 sheng, then add remaining herbs. Boil to obtain 3 sheng. Strain and take 1 sheng warm, three times daily.
[Supplementary Note] Sweat is the fluid of the Heart. Palpitations below the umbilicus after sweating indicate Heart Qi deficiency and stirring of Kidney Qi. Kidney evil desires to overwhelm the Heart, causing initial palpitations below the umbilicus. Fuling is chosen to directly target the Kidney realm to drain dampness; thus, in Zhenwu Tang, Fuling serves as the sovereign herb, capable of capturing scattered exterior fluids and consolidating Northern Water. How much more so for impending Ben Tun within the Shaoyin storehouse?
Method for preparing Ganlan Water: Take 2 dou of water, place in a large basin, and agitate with a ladle until thousands of pearl-like bubbles form and chase each other on the surface. Use this water.
