In cases of weapon injuries, if there is bleeding and severe thirst, one must not drink cold water or cold tea, nor eat cold food. Decoct children's urine (Tongbian) with aged wine and administer orally, then proceed with treatment. This way, errors can be largely avoided.
Ziran Tong (Ferrosum Oritum) is a crucial herb for bone setting. It is used in almost all external applications, and should never be omitted in trauma powders. Xu Duan (Dipsaci Radix) and Wu Jia Pi (Acanthopanacis Cortex) serve as adjunct herbs, while Huo Xue Gui Hong (promoting blood circulation and restoring redness/health) is the main action. Zhi Qiao (Aurantii Fructus) and Qing Pi (Citri Reticulatae Pericarpium Viride) regulate Qi as adjuncts. Po Xue Mu Xiang (Costi Radix) and Tao Ren (Persicae Semen) act as sovereign herbs for breaking blood stasis. Bu Xue Shao Yao (Paeoniae Radix Alba) and Sheng Di Huang (Rehmanniae Radix) are most important for nourishing blood. Dispersing wind relies mainly on regulating Qi, and promoting blood circulation requires smoothing Qi as a priority. For hand injuries, Gui Zhi (Cinnamomi Ramulus) must be used; for foot injuries, Mu Gua (Chaenomelis Fructus) is essential. Although these formulas are family secrets, herbal selection should adapt to changes in condition. If bleeding occurs, applying Jin Qiang San (Golden Spear Powder) is effective; San Qi (Notoginseng Radix), Song Xiang (Colophonium), and Bing Fen (Borneolum Syntheticum) are potent. For excessive blood loss, Si Wu Tang (Four Substances Decoction) should be administered; if Ming Xuan reaction (severe adverse reaction indicating strong drug effect) occurs, adding Ginseng urgently yields good results. For internal trauma from falls and blows causing nausea and vomiting, Ji Ming San (Rooster Crowing Powder) promotes recovery. If the patient experiences extreme dry mouth and refuses fluids, enduring this state temporarily may prevent disaster.
First, injury to Naohu (GV16) and Tianchuang (SI10) acupoints. Second, injury to the jumping pulse in the arm, or the Yin Gu area of the Spleen meridian. Third, injury to the Heart or breasts. Fourth, injury to Jiawei (CV15) acupoint, small intestine, or five zang organs. Fifth, injury to brain marrow or skull fracture, accompanied by hoarseness and fixed gaze. Sixth, pain located away from the injury site. Seventh, unceasing bleeding that turns from red to black. Eighth, foul-smelling necrotic muscle. These eight conditions are considered incurable. Apart from these incurable cases, observe the pulse: before bleeding, a洪大 (Hóng Dà, surging and large) pulse is favorable; after bleeding, a 洪大 pulse is unfavorable. A 虚细沉小 (Xū Xì Chén Xiǎo, deficient, fine, deep, and small) pulse indicates survival, while a 数实浮大 (Shuò Shí Fú Dà, rapid, excess, floating, and large) pulse indicates death. If there is heavy bleeding at the injury site, a 微缓 (Wēi Huǎn, faint and slow) pulse suggests survival, whereas an 急疾 (Jí Jí, rapid and urgent) pulse suggests death.
The human head possesses joints but lacks specific signs for fracture verification. However, crushing injuries from falls occur. If brain marrow leaks out, it is difficult to treat. If bones appear green, they are hard to heal. Bone fragments the size of millet seeds can be removed, but larger ones cannot. If such a case occurs, first apply Huo Xue San (Blood-Activating Powder), avoid wind, abstain from sexual activity, and the patient must exercise caution. Generally, apply Shu Feng Li Qi Tang (Wind-Dispersing and Qi-Regulating Decoction) externally and internally for five to six doses until the wound heals flatly, then administer Bu Xue Shun Qi Tang (Blood-Nourishing and Qi-Smoothing Decoction) for three to four doses to ensure stability. If wind-toxin enters the wound (tetanus), presenting with trismus and opisthotonos, urgently administer Fei Long Duo Ming Tang (Flying Dragon Life-Extending Decoction) for cure. This formula is universally effective and must not be neglected. On the following day, if there is prolapse of the eyeball (Luo Zhu) from combat injuries, first apply Shou Zhu San (Eyesball-Receiving Powder). Dip a silver needle in well water, dab the previous powder onto the blood vessels, then wash the area with old blue silk soaked in warm water. Administer Huan Hun Tang (Returning Soul Decoction) for three to four doses. Once stabilized, use Ming Mu Sheng Xue Yin (Brightening Eyes and Generating Blood Drink) to restore health. For nasal bone fractures, first apply Jie Gu San (Bone-Setting Powder) directly on the bone, then apply Sheng Ji San (Flesh-Generating Powder) mixed with vegetable oil, followed by Huo Xue Zhi Tong San (Blood-Activating and Pain-Stopping Powder) for external stabilization. For cleft lip, first apply Dai Tong San (Pain-Substituting Powder), suture the gap with oiled thread, apply Sheng Ji Powder, and internally take Huo Xue San and Zhi Tong San. The hip joint (Tunjia) is unique and difficult. When dislocated inward, have the patient lie on their side. The inner hand moves inward, the outer hand outward. One hand holds the waist, the other presses the lever point, lifting the knee. Pull right when left comes out, pull left when right comes out, extending upward to the right. Internally take Sheng Xue Bu Sui Tang (Blood-Generating and Marrow-Nourishing Decoction). This serves as a model for hip reduction. The shoulder joint (Tianjing) is the most difficult. In cases of falling from heights, bones may dislocate and cannot be easily bound. Administer Chuan Qi Tang (Qi-Raising Decoction) to align bones, then apply Jie Gu Powder wrapped in cotton. For shoulder and back dislocations, use Ti Qi Huo Xue Tang (Qi-Lifting and Blood-Activating Decoction) for three to four doses. Observe tendons and bones; if misaligned, Diaosou Yin (Cough-Hanging Drink) may be necessary. Apply Jie Gu Powder and take Sheng Xue Bu Sui Tang for several doses to recover. Leg fractures are common; if broken into two segments, reduction relies on binding. First wash with Kuan Jin San (Tendon-Loosening Powder) decoction. Have the patient lie on their side, aligning with the uninjured leg. Apply Jie Gu Powder, wrap in cotton, and use eight splints (4 cun long, wrapped in cotton paper). Secure with three ropes aligned with fir wood boards. Internally take Huo Xue Zhi Tong Powder for three to four doses, alternating with Zhuang Jin Xu Xue Gu Dan (Tendon-Strengthening and Blood-Continuing Bone Pill) for recovery. For patella (Bin Gu) displacement, cover with the old bone, stabilize with a cotton bandage. Have the patient lie supine, one person lifts the ankle. If displaced left, guide down left; if right, guide down right. The physician slowly supports the knee with both hands, bending the knee with the upper hand and catching the foot with the lower hand. Align the patella by pulling right-down with the right hand and left-down with the left hand. Lift the foot to align. Apply Jie Gu Powder, wrap in cotton cloth, place the bandage on the site, and internally take Sheng Xue Bu Sui Tang for three to four doses, followed by Zhuang Gu Xu Jin Dan (Bone-Strengthening and Tendon-Continuing Pill). The lower leg has two bones, one large and one small. Single fractures are easy to treat; double fractures are difficult. Fractures resembling lotus root nodes are easier than those split into two segments. Puncture wounds with bone breakage are ominous; if not ominous, treat like thigh fractures. If bone protrudes through skin, use Ran Lan Dan (Rotting-Dissolving Elixir) to remove flesh. Then realign bones. Do not wash with hot soup to avoid toxin absorption. Apply Sheng Ji Powder. If skin is intact, apply Jie Gu Powder and bind with six fir boards (3 chi 5 cun long). This injury is very painful; take Sheng Xue Bu Sui Tang for three to four doses, then Zhuang Jin Xu Gu Dan. Ankle dislocation is easy to cause but hard to reduce. Hold the root with one hand, pull the toes with the other. Pull right with right hand, left with left hand, lifting the heel and extending the foot upward. Take Kuan Xiong Huo Xue Tang. Shoulder dislocation resembles knee dislocation but moves downward. Press the shoulder with one hand and the hand with the other, rotating slowly until tendons relax. Have the patient sit low, another person holds them, the physician manipulates the arm against the bone, using the knee to clamp the arm, and lifts together. Wrap cotton like an egg, place on the knee, apply Jie Gu Powder, and take Sheng Xue Bu Sui Tang. Arm dislocation upwards: hold the armpit with one hand, the elbow with the other, lift the wrist, and extend upward. Apply Jie Gu Powder, wrap in cotton, take Sheng Xue Bu Sui Tang. Wrist dislocation: hold five fingers with one hand, palm with the other, lift the wrist joint, and extend downward/upward. This involves the knee joint area; take Kuan Jin Huo Xue San. Do not bind; apply Gu Jia San, wrap in cotton cloth, use a wide board and four 3-cun fir boards, bind for seven days. Finger joints have three segments; only the middle segment dislocates. It is easy to reduce. Take Huo Xue Zhi Tong Powder, otherwise pain is severe. Upper and lower arm fractures are treated like thigh and calf fractures. Add Niu Xi (Achyranthis Bidentatae Radix) and Mu Gua for lower body prescriptions, and Gui Zhi for upper body. These are key points. Descendants learning this must choose worthy teachers, sit together, and explain in detail—true oral transmission. Bone setting relies on binding; fir wood is used for its lightness and flexibility. The key herbs of these formulas are priceless; all fractures stem from here. Herbs have processing methods, and decoctions require flexible application; do not adhere rigidly. If heat disease complicates the injury, medication may be ineffective. Joint dislocation descriptions are concise. Carefully distinguish joint heads to avoid tendon contraction. Axe cuts require discussion of Gusuibu (Drynariae Rhizoma). Generally, loosening tendons requires washing with Kuan Jin San decoction. Hand and foot tendons connect to the back; finger movement activates these tendons. Wash and gently stretch these tendons. Stiff neck (Shijin) can occur during sleep or sudden error. Sit low, pull the head with one hand and the lower neck with the other to straighten. Gunshot wounds: assess lethality and depth. Superficial fatal zone injuries are harmless. Abdominal wounds require probing depth; deep visceral injuries are hard to treat. Straight wounds: apply Zhixue Ding Tong San. Deep wounds: probe with cotton, sprinkle dry powder, wait for bleeding to stop, then apply Sheng Ji Powder, and internally take Hufeng Tuoli San (Wind-Protecting and Interior-Supporting Decoction). Head/forehead axe injuries: prevent chills/fever; prioritize wind protection. Deep/fine pulse indicates survival; surging/large pulse indicates difficulty. Hard area injuries: check bone damage. Soft area injuries: check depth. Treat bone injuries by healing bone, meat injuries by generating flesh. Axe cuts differ from punctures; prioritize Sheng Ji Powder externally and Hufeng/Tuoli formulas internally. Refer to previous joint injury discussions. Throat strangulation: assess cut smoothness and depth. Double cuts are easier than single. If esophagus is stiff, suture with oiled thread, apply Sheng Ji Powder, and take Hufeng Tuoli San. Severed esophagus is fatal; suturing with silk thread may help. Protruding intestines: dangerous but manageable if viscera are intact. Remove fingernails to prevent intestinal damage. Patient likely dies, but if viscera are safe, normal diet ensures survival. Use a spinning wheel to fan the patient, keep wet soup off the wound, suture skin with oiled thread, apply Sheng Ji Powder, and take Tong Chang Huo Xue Tang (Intestine-Unblocking and Blood-Activating Decoction). Mulberry bark thread can also be used. Fingers are critical; injury to one causes heart pain. Middle finger is most important. Wind-toxin infection risk: apply Zhixue San first. Human bites: remove tooth toxins, apply medicine, take Huxin Wan (Heart-Protecting Pill) urgently. If tetanus occurs, take Fei Long Duo Ming Tang. Axe cuts are easier than bites; bites are toxic, take Tuidu Dingdu San. Nine out of ten bite victims die; difficult to treat. Bone fragmentation: assess injury. If broken, remove bone; if intact, use Zuangu San (Bone-Boring Powder) to extract fragments. Apply Sheng Ji Powder, take Dingtong Busui Tang (Pain-Stopping and Marrow-Nourishing Decoction). Incomplete removal prevents healing. Meticulous extraction leads to natural recovery. This book must not be neglected; treasure it.
