The Kidney opens into the ears. However, it is the Lung that enables hearing, because the Lung governs Qi, and the Qi of the entire body penetrates into the ears. In treating deafness (hearing loss), one must first regulate Qi and open stagnation. When the Zong Mai (Gathering Vessel) is deficient, Wind Evil exploits this weakness, causing Qi obstruction without free circulation; this is termed Feng Long (Wind Deafness). Internally, there must be itching, or accompanied by headache. When Jue Qi (Juevian Qi/Pathogenic Qi of the Jue channels) clashes with the ear, it is termed Jue Long (Juevian Deafness). With obstruction and blockage, dizziness and vertigo must also accompany it. Labor injuring Blood and Qi, and excessive sexual desire depleting essence, leading to emaciation, fatigue, and mental confusion, is termed Lao Long (Fatigue Deafness). If one rests properly, the deafness will naturally lighten; if one becomes increasingly fatigued day by day, it becomes chronic deafness. Rich flavors stir Stomach Fire, causing deafness in both ears; anger stirs Gallbladder Fire, causing deafness in the left ear; sexual desire stirs Ministerial Fire, causing deafness in the right ear.
Ear sores belong to the Sanjiao Channel. If accompanied by fever and burning pain, it is caused by Wind-Heat. If accompanied by internal heat, itching, and pain, it involves Blood Heat of the Liver Channel.
Ding Er (Cerumen impaction) occurs when Wind-Heat clashing causes body fluids to accumulate and form nodules, leading to sudden deafness. Si Wu Tang (Four Substances Decoction) added with Qiang Huo (Notopterygii Rhizoma et Radix), Fang Feng (Saposhnikoviae Radix), Chai Hu (Bupleuri Radix), Huang Qin (Scutellariae Radix), Lian Qiao (Forsythiae Fructus), and Xuan Shen (Scrophulariae Radix) is suitable.
In the ear, Qi stagnation generates Phlegm, and internal Fire attacks and rushes upward, causing sores that resemble red flesh, or discharging pus and water. Er Chen Tang (Two Aged Substances Decoction) added with Xuan Shen (Scrophulariae Radix), Fen Hua (Trichosanthis Radix), Huang Qin (Scutellariae Radix), Shan Zhi (Gardeniae Fructus), Lian Qiao (Forsythiae Fructus), Man Jing Zi (Viticis Fructus), and Chai Hu (Bupleuri Radix) is suitable.
Er Nu (Auricular Epistaxis) refers to bleeding from the ear. If the pulse at the Left Guan position is wiry and rapid, it indicates Fire of the Shaoyang Channel; Chai Hu Qing Gan San (Bupleurum Liver-Clearing Powder) is suitable. If the Chi pulse is either restless or weak, it indicates deficiency of the Shaoyin Channel; Liu Wei Di Huang Wan (Six-Ingredient Pill with Rehmannia) is suitable.
The nose receives Heaven's Qi, thus it is called Tian Pin (Heavenly Nostril). It is the pathway of Zong Qi (Pectoral Qi) and the gateway of the Heart and Lung. When the Heart and Lung have diseases, respiration becomes obstructed. The meridians reaching here belong exclusively to the Yangming Channel. From above the Shangen (root of the nose), it connects to the Taiyang and Du Mai (Governor Vessel) to communicate with the brain. External contraction of Wind-Cold causes Qi congestion and Heat stagnation, mixing Clear and Turbid Qi. Mild Heat results in clear nasal discharge, termed Qiu (Allergic Rhinitis/Coryza); severe Heat results in turbid nasal discharge, termed Yuan (Chronic Sinusitis/Purulent Rhinitis). Or it may discharge foul yellow fluid, also known as Nao Lou (Brain Leakage). Food accumulation and Heat-Phlegm in the Stomion flow into the Lung, causing Turbid Qi to condense and form polyps, blocking the nasal cavity, also known as Bi Weng (Nasal Polyps). For external contractions, treatment should use acrid dispersants. For internal Fire flaming upward, treatment should use cooling agents. Furthermore, for chronic non-healing Qiu, Yuan, sores, or hemorrhoids, it is either a deficiency of Heart Blood or a scarcity of Kidney Water. Nourishing Blood lowers the Fire; tonifying the Kidney clears Metal (Lung). Chronic nasal congestion may also result from internal injury to the Lung and Stomach, where Clear Qi cannot ascend; it is not entirely due to external contraction.
Epistaxis (Nosebleed) must originate from above the Shangen, near the Jingming point. Chong Mai (Penetrating Vessel) is the Sea of Blood of the Twelve Channels, attaching to the Yangming. Generally, emotional strain and labor injury cause Yin deficiency and stirring of Fire, with blood rushing directly from the meridians to invade the clear pathways and exit through the nose. Treatment should use sweet and neutral herbs to nourish True Yin.
Fang Feng Tang (Saposhnikovia Decoction) treats nasal diseases manifesting on the exterior branch.
Ingredients: Fang Feng (Saposhnikoviae Radix), Chuan Xiong (Ligusticum Chuanxiong Rhizoma), Huang Qin (Scutellariae Radix), Jie Geng (Platycodonis Radix), Gan Cao (Glycyrrhizae Radix), Da Li Zi (Arctii Fructus).
For external Wind, add Qiang Huo (Notopterygii Rhizoma et Radix), Jing Jie (Schizonepetae Spica), Bo He (Menthae Haplocalycis Herba), Xi Xin (Asari Radix et Rhizoma), Xin Yi (Magnoliae Flos), Bai Zhi (Angelicae Dahuricae Radix).
For internal Fire, add Shan Zhi (Gardeniae Fructus), Lian Qiao (Forsythiae Fructus), Fen Hua (Trichosanthis Radix), Yuan Shen (Scrophulariae Radix), Sang Bai Pi (Mori Cortex).
