Treatise on Carbuncles and Ulcers:
Huangdi asked: "You speak of Yong (Carbuncle) and Ju (Deep Abscess); how do we distinguish them?"
Qibo answered: "When Ying Qi (Nutritive Qi) and Wei Qi (Defensive Qi) stagnate in the meridians, blood becomes viscous and fails to flow. When blood fails to flow, Wei Qi follows it. Following it but failing to circulate smoothly, it becomes obstructed and blocked, thus generating heat. If intense heat persists, excessive heat causes flesh to rot; rotten flesh becomes pus. However, it does not sink into the skin and muscle to reach the bone marrow, nor does the bone marrow become scorched and dry, nor are the Five Zang Organs damaged. Therefore, it is named Yong."
Huangdi asked: "What is Ju?"
Qibo answered: "When pathogenic heat is extremely abundant, it sinks deeply into the skin, muscle, sinews, and bones, connecting internally to the Five Zang Organs. Blood and Qi are exhausted. Below the swelling, the sinews, bones, and healthy flesh are completely destroyed. Therefore, it is named Ju. The skin over a Ju is withered, stagnant, and hard, resembling the skin of an ox's neck. The skin over a Yong is thin and lustrous. These are their signs."
Huangdi said: "Excellent." (From *Waitai Mi Yao*, Vol. 24)
Regarding the treatment of Yongzhong (Swollen Carbuncles): Deep pain upon heavy pressure indicates a deep pathology; superficial pain upon light pressure indicates a shallow pathology. If depression remains after pressing, there is no pus; if it rebounds immediately, there is pus. If incision is required at the upper part, ensure complete drainage of pus. If drainage is incomplete, it may penetrate deeper, eroding bone, causing bone fragmentation. One should incise the side near the 'fish-mouth' area, starting from the lower head, to ensure complete pus discharge, thereby addressing the bone issue...
Take Bai Di Hui (Ash of White Reed Stems), leach with water, and decoct into a paste. This medicine should not be prepared in advance, as its efficacy diminishes after ten days. It can also remove black nevi. Apply the medicine to the nevus and wipe it off immediately; delaying wiping will damage the skin. Another effective method uses Sangbai Hui (Ash of Mulberry White Bark). (From *Waitai Mi Yao*, Vol. 24)
For any ruptured lesions with thick purulent tissue, one must first widely seal the four sides. Otherwise, rupture of the sore and leakage of Qi will lead to death, which is incurable. Long-standing sores from carbuncles may become Baiyong (Failed Carbuncle) or Shenju (Deep Abscess). Sores frequently occur on the shins. Chronic malignant sores, frozen by frost and cold that do not heal for years, or Guju (Bone Abscess) also known as Jingchuang (Shin Sore), present with deep rotting, cyanotic-black discoloration, firm edges, and central discharge of pus, blood, and foul fluid. Hundreds of medicines fail to cure it; the fluid swells healthy surrounding flesh, and sometimes fragmented bones are expelled. Wash with Wenchidlongpi Tang (Warm Red Dragon Skin Decoction). In summer, wash daily; in winter, wash every three or four days. For cases with much ulcerated flesh, apply Bailvru San (White Smilax Powder) intermittently to remove necrotic tissue. Apply three times within a day...
Take Zhushi (Porcine Feces), burn to ash, sift through silk gauze, and sprinkle the powder into the Ju or failed sore until full. As fluid drains, reapply. Continue application until healed. If green fleshy tissue regrows, apply Bailvru again as before. (From *Waitai Mi Yao*, Vol. 14)
