从大便出者,有至经期而血逆行,或吐或衄,或从耳目出,谓之倒经者,有三月一行,而谓之居经者;

1. A patient suffered from Hong Kong foot (a disease characterized by damp-heat swelling and pain in the feet) attacking and flooding the lower limbs. Several river snails were crushed and applied to both thighs. The patient felt a cold sensation flowing down to the feet, bringing relief. This is because snails have the property of separating the clear from the turbid, thus treating the damp-heat of Hong Kong foot. Liu Zinong said: "I suffered from Hong Kong foot, unable to unblock and drain. At night, a mass formed (Hong Kong foot affecting the Spleen), Qi was cut off (Hong Kong foot affecting the Heart), and a stone-like mass appeared in the left hypochondrium (Hong Kong foot affecting the Liver). I was on the verge of death, unconscious for three days due to severe cold, causing my family to wail. Someone passed on a recipe for Fir Twig Decoction. I drank a large dose, resulting in three large bowel movements. Qi flowed freely, the mass dispersed, and I recovered. The formula uses Fir Twig Nodes, which are heavy in substance to reach the lower body and aromatic in odor to disperse stagnation. Tangerine Peel is bitter and strong, stronger than Green Tangerine Peel. Areca Seed is solid and heavy, like iron or stone; its strong flavor induces purgation, and its heavy nature directs downward, thus allowing pathogenic factors to be greatly expelled downwards. Child's urine guides damp-heat downward, exiting through urination. The Classics state: 'For distant pathways, make the dosage large.' Hence, a large dose is taken at once."
Regulating menstruation is the primary focus in treating women's diseases. Women are governed by Blood. Menstruation occurs once a month. "Jing" (Menstruation) means constancy, just as the moon has regular phases of waxing and waning. If it comes early or late, irregularly, or stops completely, this indicates a loss of normal rhythm, necessitating medicinal regulation. Historically, the method focused on regulating Qi and nourishing the Heart and Spleen. Since the Spleen is the source of production and transformation, and the Heart governs the blood of all meridians, if the Heart and Spleen are harmonious, menstruation will naturally regulate itself. However, the causes of irregularity vary. One must assess: if heat, cool it; if deficiency, tonify it; if stagnation, move it; if retention, purge it; if slipping, secure it; if sinking, lift it. Treating according to symptoms ensures efficacy. Danxi said: "Early arrival indicates hot blood, with bright red color. Purple-black indicates severe heat forming clumps. Although Qi stagnation is mentioned, extreme heat also causes this. Treatment should cool blood, clear heat, and tonify the Liver and Kidney. However, some cases arise from anger injuring the Liver, causing Liver Fire to boil blood prematurely. Some result from depression injuring the Spleen, where stagnant fire forces blood to flow prematurely. Some stem from overthinking injuring the Heart, where deficient fire causes blood to flow erratically. Treat each according to its cause. Late arrival indicates Blood deficiency, with pale red color. Pale white indicates severe deficiency. There are also cases where anger injures the Liver or depression injures the Spleen, leading to insufficient Blood, late periods, or amenorrhea. In obese individuals, damp-phlegm obstruction may cause late periods, irregular flow, requiring either Blood tonification or phlegm resolution based on symptoms. For irregular timing, regardless of symptoms, Qi disorder is treated as deficiency. Amenorrhea requires distinguishing deficiency and excess. Without other diseases, amenorrhea belongs to Yin deficiency with scanty Blood and flourishing Fire; toxic medicines for unblocking menstruation should not be used. In consumptive diseases with amenorrhea, insufficient Kidney Water allows Ministerial Fire to move recklessly, boiling True Yin and steaming the Sea of Blood, causing daily depletion of Blood, known colloquially as Dry Blood Consumption. Cases where emotional desires injure the Heart and fatigue injures the Spleen lead to Stomach disease, losing the source of producing essence and Blood, causing amenorrhea. This corresponds to the Classic stating 'Diseases of the Two Yang (Stomach and Large Intestine) originating in the Heart and Spleen' manifesting as no menstruation in women. Cases where unfulfilled concerns deplete Heart Blood, preventing Blood from returning to the Liver, cause amenorrhea; these are all due to deficiency. Cases caused by internal Blood stasis, anger/depression causing Qi stagnation and Blood coagulation, or catching cold/drinking cold during menstruation causing Blood coagulation are easy to treat, merely requiring unblocking. If menstrual flow is excessive, blame heat; treatment should cool blood, clear heat, and tonify Liver and Kidney primarily. If excessive and unceasing, add sour/astringent herbs or lifting agents. Scanty flow indicates Blood deficiency; in obese people, it may be due to Phlegm obstruction or Qi stagnation. Lower back and abdominal pain before menstruation is blamed on Qi stagnation and Blood excess; lingering pain after menstruation is blamed on Blood deficiency and Qi stagnation. These are the general rules of regulating menstruation. There are variations not mentioned by ancients. Such as Shicai saying: Some never menstruate but bleed from the rectum; some experience retrograde bleeding during menstruation, vomiting or epistaxis, or bleeding from ears/eyes, called Retrograde Menstruation; some menstruate every three months, called Residence Menstruation; some once a year, called Avoidance Year; some never menstruate but conceive, called Hidden Menstruation; some menstruate monthly after conception and give birth, called Filthy Pregnancy; some suffer sudden heavy bleeding months into pregnancy without miscarriage, called Leaking Pregnancy. Though explained by surplus or deficit of Qi and Blood, these are abnormal. The Classics state: 'At fourteen (two sevens), Tian Gui arrives; at forty-nine (seven sevens), Tian Gui ceases,' which is normal. Some girls bear children at twelve or thirteen; some women at fifty or sixty. These are extreme abnormalities. Physicians must know them.
Guipi Tang (see Consumptive Disease) treats Heart, Spleen, and Liver deficiency, irregular menstruation (early, late, irregular, amenorrhea, or menorrhagia with dripping). Modify this formula accordingly.
Add Rehmannia Root, White Peony Root, and Four-Processed Cyperus Rhizome. (Cyperus is a Qi herb within Blood herbs. Regulating Qi harmonizes Blood. Soak in Mugwort and Vinegar for two nights, divide into four parts. One part stir-fried with salt water, one with wine, one with child's urine, one with human milk.) If early due to Spleen injury from depression, add Bupleurum Root (guides Spleen/Stomach Qi upward, relies on lifting power), Gardenia Fruit (clears stagnant fire and Blood heat), Ophiopogon Tuber, and Turtle Shell (nourishes Yin, removes heat), or additionally Evodia Fruit and Stir-fried Coptis Rhizome (reaches Heart/Spleen stagnant fire). If late due to Spleen injury from depression, or scanty flow/amenorrhea, add Biota Seed, Goji Berry, Elk Antler, and Human Milk. (All tonify Blood. Drinking one bowl of human milk daily can promote menstruation.) If excessive dripping continues, add Ophiopogon Tuber, Turtle Shell (cold clears heat), Lotus Stamens, and Oyster Shell (astringe to prevent collapse), Schisandra Berry, and Cornus Fruit (sour to converge), and Burnt Hair. (Hair is the surplus of Blood, seeking similar energy.)
This formula acts on the Heart, Spleen, and Liver meridians. Lizhai said: "The Heart governs Blood, the Liver stores Blood, both regulated by the Spleen. Tonifying Spleen and harmonizing Stomach generates Blood." Thus, it is often used for regulating menstruation, especially for weak Stomach and deficient Spleen.
Note: This formula was created by Yan Yonghe of the Song Dynasty to treat diseases of the Two Yang originating in Heart and Spleen, involving hidden distress and female amenorrhea. Yangming Stomach and Large Intestine are the Two Yang. Emotional desires injure the Heart; when the Mother is injured, the Child suffers. Fatigue injures the Spleen; when Zang is injured, Fu organs are affected. Internal injury to Essence and external injury to Form affect the Stomach, hence Two Yang diseases originate in Heart and Spleen. "Hidden distress" refers to male sexual dysfunction. The Stomach is the sea of water and grain, transforming Ying and Wei and moistening the Sinews. As the Classics say: "The anterior yin is the gathering place of sinews, combining Taiyin and Yangming." Stomach disease loses the source of producing essence and Blood, leading to Yang decline and scanty essence. In women, this is amenorrhea. It further states: "If transmitted as Wind Wasting or Breath Accumulation, death is incurable." Long-term Stomach disease transmits; Liver Wood overacts on Earth, Wind flourishes wasting muscles; Stomach disease deprives Lung of nourishment, causing rapid breathing. Qi exhausts above, Essence depletes below, failing Five Zang, leading to death. Therefore, treat at the onset. The original formula lacked Polygala Root and Angelica Sinensis; Xue added them to treat Blood deficiency. He also added Moutan Bark and Gardenia Fruit to treat Blood heat, completing the principle of Yang generating Yin. Flexible application suits all symptoms miraculously.
Dishao Liangxue Tang (Self-made) treats early menstruation due to Blood Heat. Color bright red, purple-black, clotted, or excessive unceasing.
Rehmannia Root, White Peony Root (Cool Blood), Cortex Lycii, Turtle Shell (Clear Heat), Bamboo Leaf (or Bamboo Shavings), Ophiopogon Tuber (Clear Heart to govern Blood), Chinese Yam, Licorice Root (Tonify Spleen to control Blood), Goji Berry, Eucommia Bark (Tonify Liver/Kidney). Add Anemarrhena Rhizome (Bitter/salty/cold, nourishes Yin, removes heat). For robust patients with severe Blood Heat, temporarily add Anemarrhena, Phellodendron, Scutellaria, and Gardenia. If excessive unceasing, add Lotus Stamens (Astringe), Ziziphus Seed (Gather).
This formula cools blood, clears heat, and tonifies Liver/Kidney. Modify as needed. Suitable for Yin deficiency with internal heat who cannot take Ginseng/Atractylodes. Or use Liuwei Dihuang Wan combined with previous herbs.
Xiaoyao San (See Consumptive Disease) treats Liver/Spleen meridian deficiency, irregular menstruation (early, late, amenorrhea, or excessive unceasing).
Add Four-Processed Cyperus Rhizome. If early due to Liver injury from depression/anger, add ingredients from Dishao Liangxue Tang. If late due to Liver injury from depression/anger, or scanty flow/amenorrhea, add Biota Seed, Goji Berry, Achyranthes Root, and Prepared Rehmannia Root. If excessive unceasing, add Lotus Stamens, Ziziphus Seed, and Cornus Fruit.
This formula disperses with acrid, gathers with sour, sweetens to nourish Blood, calms Heart, supports Spleen. It is a key Liver meridian herb, a divine agent in Gynecology. Those with much depression/anger suit it best.
Tianwang Buxin Dan (See Consumptive Disease) treats mental labor causing deficient Fire to move recklessly, leading to early erratic bleeding, or unfulfilled concerns depleting Heart Blood, preventing return to Liver, causing amenorrhea.
For early menstruation, remove Angelica Sinensis (dislikes acrid/warm moving Blood) and Polygala Root (acrid/warm). Add White Peony Root and Tortoise Shell.
For amenorrhea, remove Schisandra Berry and Platycodon Root. Add White Peony Root, Goji Berry, Achyranthes Root, and Human Milk. This formula generates fluids, nourishes Blood, clears heat, calms Heart, settles Spirit. Mental laborers should modify and use it.
Siwu Tang (See Headache) treats Liver meridian Blood deficiency, late absence.
Add Biota Seed, Goji Berry, Achyranthes Root, Deer Antler Glue, and Processed Cyperus Rhizome.
Though called the master of regulating menstruation, it is unsuitable in some cases. Avoid Angelica and Ligusticum Chuanxiong in Blood Heat. Many modern patients have Yin deficiency/Blood Heat; thus, those with hematemesis/epistaxis, cough/wheezing, or vomiting/dysphoria should avoid it. Angelica lubricates intestines; contraindicated in diarrhea.
Bairen Wan (Can also be decocted) treats Yin deficiency, scanty Blood, flourishing Fire, amenorrhea.
Biota Seed (Tonify Blood), Two Di (Rehmannia & Drynaria?), Two Dong (Ophiopogon & Asparagus?), Achyranthes Root (Prepared tonifies essence/Blood, raw promotes menstruation, 4 liang each), Dipsacus Root (Unblocks/promotes Blood vessels), Lygodium Spore (Nourishes Ying Blood, moves old Blood, 2 liang each). Honey pills.
This formula nourishes Yin and Blood, strengthens Water to control Fire. When Blood is sufficient, menstruation comes spontaneously.
Shixiao San (See Epigastric Pain) treats internal Blood stasis, amenorrhea, occasional lower abdominal pain.
Original formula (consult He's additions). Add Cyperus Rhizome and Costus Root. If anger/depression causes Qi stagnation/Blood coagulation, add Curcuma Zedoaria and Amomum/Aucklandia. If lower abdomen full/painful/rejects pressure, use Tao Ren Cheng Qi Tang, modified.
This formula dissipates stasis and moves Blood, i.e., "move stagnation, purge retention."
Xiangsu San (See Cold Damage) treats menstruation interrupted by cold exposure and cold drinks, causing Blood coagulation/stopping.
Add Angelica Sinensis, Red Peony Root, Peach Kernel, Safflower, and Corydalis Rhizome.
This formula warms, disperses, moves Blood, transforms Qi, and unblocks.
Erchen Tang (See Stroke) treats obese individuals with excessive food intake, fat accumulation blocking, damp-phlegm obstruction, causing late/scanty menstruation or amenorrhea.
Add Cyperus Rhizome (Benefits Qi), Salvia Root, Costus Root, Achyranthes Root. (All move Blood without stagnating Phlegm). With deficiency, use Xiangsha Liujunzi Tang plus above herbs, decocted with Motherwort Decoction. (All unblocking menstruation herbs should be decocted with this soup). This formula resolves Phlegm and regulates Qi. Benefiting Qi moves Phlegm; removing Phlegm opens menstruation. Seek the root. Apply carefully; do not blindly administer to all obese people.
