Leukorrhea is divided into red and white types. The human body possesses a meridian named the Dai Mai (Belt Vessel), which encircles the waist horizontally, resembling a belt. Diseases occurring at this location are thus named "Dai" (Belt). Red leukorrhea (i.e., Chi Lin, bloody strangury) often arises from worry, thought, depression, and anger, which damage the Heart and Spleen. When Liver Fire flares up intermittently, blood fails to return to its vessels, resulting in the downward discharge of red fluid. Treatment should focus on nourishing the Heart and Spleen, benefiting the Liver, cooling the blood, and clearing fire. If the condition persists for a long time, Yin-blood gradually becomes deficient, and Middle Qi gradually declines; in such cases, major tonification of Qi and Blood is required. White leukorrhea is caused by Damp-Heat combined with Phlegm, presenting as either deficiency or excess patterns. According to Zhong Chun: White leukorrhea mostly belongs to Spleen Deficiency. When Liver Qi stagnates, the Spleen is injured; when the Spleen is injured, the Qi of Earth-Dampness sinks downward, causing continuous discharge of white, slippery substances. This is all due to Wind-Water stagnating within the interior. Treatment should primarily involve raising and lifting Liver Qi and supplementing Spleen Yuan, assisted by herbs that clear heat and remove dampness. Furthermore, if the discharge resembles thick rice water and has a foul odor, it indicates severe Damp-Heat; treatment should prioritize clearing heat and removing dampness, assisted by agents that raise and lift. If the discharge resembles egg white, it indicates extreme deficiency of the Spleen and Kidney. The complexion will necessarily lack luster, the calves will necessarily be edematous, and the lower back and legs will necessarily ache. Treatment should involve boosting Qi and strengthening the Spleen, combined with nourishing Yin and tonifying the Kidney using two formulas administered separately. Chen Ziming discussed the five different colors of leukorrhea, assigning them to the Five Zang Organs. Li Dongyuan proposed a theory regarding cold as the primary factor in treating leukorrhea; clinicians should carefully examine these aspects during practice.
Jiawei Guipi Tang (Modified Guipi Decoction) Jiawei Xiaoyao San (Modified Xiaoyao Powder) Tianwang Buxin Dan (Emperor of Heaven's Heart-Nourishing Pill) [All seen in Xu Lao (Deficiency Consumption)].
These three formulas generally treat red leukorrhea; they should be modified according to clinical suitability. Guipi Tang and Xiaoyao San also treat white leukorrhea.
Buzhong Yiqi Tang (Tonify the Middle and Boost Qi Decoction) [Seen in Xie Xie (Diarrhea)]. This formula, modified, is the main treatment for white leukorrhea.
The original formula [Supplements Spleen Yuan, raises and lifts Liver Qi] should be combined with Er Miao San (Two Marvels Powder), adding Maidong (Ophiopogonis Radix), Fuling (Poria), Cheqian (Plantaginis Semen) [Clears heat and removes dampness], and Zaoren (Ziziphi Spinosae Semen) [Astringes]. [Decoct strongly and drink frequently.]
Additionally use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill); add Niuxi (Achyranthis Bidentatae Radix), Duzhong (Eucommiae Cortex), Muli (Concha Ostreae), Haipiaoxiao (Endoconcha Sepiae) [Make honey pills, take on an empty stomach].
Zhong Chun stated: White leukorrhea belongs to Qi Deficiency; boosting Qi and strengthening the Spleen is the general principle of treatment. Wang Shuhe stated: Prolonged bleeding from metrorrhagia transforms into leukorrhea and leakage; one should tonify both the Spleen and Kidney.
Jiawei Ermiao San (Modified Two Marvels Powder)
Cangzhu (Atractylodis Rhizoma) Baizhu (Atractylodis Macrocephalae Rhizoma) [Dries dampness] Huangbai (Phellodendri Cortex) Huangqin (Scutellariae Radix) [Clears heat] Fuling (Poria) [Resolves phlegm] Cheqian (Plantaginis Semen) [Promotes urination].
Or add Baizhi (Angelicae Dahuricae Radix) [Raises and lifts]. This treats cases where Damp-Heat combines with Phlegm, and the leukorrhea resembles pus-like rice water with a foul odor.
