Suo Jiao Yong (Abscess with Leg Contraction) has persisted for three months, with pulling pain in the right hip region, tendon contraction, gradual emaciation of large muscles, deficiency of Yin aspect, and unresolved cold-dampness in the collaterals. The condition threatens to become a disability. Treatment should nourish Ying (Nutritive Qi), warm the meridians, and unblock the collaterals.

Dampness and blood stasis stagnate in the intestines and stomach, leading to Suo Jiao Chang Yong (Intestinal Abscess with Leg Contraction). There is spasm and pulling pain in the right hip region, swelling and hardness in the lower abdomen, and the condition is about to suppurate. Treatment should promote dampness resolution and transform blood stasis.

The pulse at both Guan positions is fine and wiry, with the right side additionally appearing choppy. Spleen has damp-phlegm; Liver Qi is excessively vigorous. Ying (Nutritive Qi) and Wei (Defensive Qi) circulation is unfavorable, resulting in chest and abdominal discomfort, pain in the waist and thigh, inability to turn or change position, and phlegm accumulation in the left hypochondrium forming a fistula. Every other day, purgative agents must be taken to temporarily relieve chest and abdominal distension. However, seeking temporary relief may damage Stomach Qi. Treatment should harmonize Ying and Wei, resolve phlegm, and pacify Liver Wood.

Liver Wood is excessive while Kidney Water is deficient; Spleen has much phlegm-dampness. For years, the patient has suffered from dizziness and syncope due to Liver Wind. Last winter, the condition gradually worsened to bedridden state, with exhaustion of Liver and Kidney blood and essence. After the Spring Equinox, Wood qi acts with Ministerial Fire; damp-phlegm fills the San Jiao (Triple Burner) following wind-fire qi, causing dizziness, fever, and generalized body pain. One might suspect a recurrence of old ailments, but it is known that an abscess/toxin has formed beside the iliac bone for over half a month. This acupoint area is where fire and dampness have condensed in the collaterals of the Liver Meridian. Liver Heat easily ascends; damp-fire steams the Stomach Fu organ. Initially, there was fever and delirious speech, followed by gradual reduction of fever and calm spirit. This indicates damp-heat pathogens converging in the Lower Jiao; this is an external manifestation, not a recurrence of old ailments. Suppuration has existed for half a month without changing clothes; teeth are dry, tongue coating is dry, indicating Yin injury and failure to transform damp-fire. Although the syndrome appears external, medication must primarily treat the internal organs. Fortunately, the pulse is harmonious with a slight wiry image at the Guan position, suggesting no cause for alarm. The proposed treatment uses sweet-cold herbs to nourish Yin, combined with methods to harmonize the Middle Jiao and moisten downward.

Pain from phlegm accumulation at the Back-Shu points, with pus draining from the intercostal spaces, difficult to heal quickly. Accompanied by fever, facial edema, chest and abdominal fullness, diarrhea, and slippery-rapid pulse. This is a syndrome of phlegm-dampness stagnating in the Spleen, not a mild condition. Internal treatment should be prioritized.
