Leprosy was anciently known as *Li Feng* (Leprosy). *Li* refers to a condition where Ying Qi (nutritive qi circulating in the vessels) is heat-filled and the internal organs' qi is turbid, causing the nasal bridge to collapse, complexion to deteriorate, and skin and muscles to ulcerate and rot. When Wind-Cold pathogens lodge in the collateral vessels and do not disperse, it is named *Li Feng*. Medical texts generally treat it indiscriminately with wind-dispelling drugs without distinguishing sequence or priority. Some use pungent and penetrating substances like Qishe (Agkistrodon), Hugu (Tiger Bone), and Shan Jia (Pangolin Scales), while Wugong (Scolopendra) is warm and toxic; taking these invariably triggers exacerbation of the disease. Based on my extensive experience, this syndrome involves various types such as Wind-Dampness, Damp-Toxin, and Toxic Leprosy, differing in whether the pathology lies in the muscle-surface or the meridians. The Lung governs the skin and hair; the Stomach governs the muscles. If Lung Qi is deficient, the Cou Li (skin pores and interstices) are not dense; if Stomach Qi is weak, the muscles are loose and porous, making one susceptible to external pathogens. Exposure to damp, misty, and obscure environments, or sitting/lying on wet ground, causes Qi and Blood stagnation. Initially, there is localized numbness, insensitivity to pain and itch, closed hair follicles, and obstructed sweat pores. Gradually, it spreads over the body, presenting as cloud-like patches that are slightly raised, either white or red. Lesions located upperward are often associated with Wind; Wind is a Yang pathogen affecting the upper body. White patches with slight redness indicate Wind combined with Heat. Lesions lower down are often associated with Dampness; Dampness is a Yin pathogen attacking from below. Red, raised patches indicate Dampness combined with Heat. Toxic Leprosy enters through the mouth and nose, primarily affecting the Yangming channels, causing blood stasis and steaming heat. Initially, nodules form lumps on the face and body; after prolonged elevation, toes may ulcerate, the nasal bridge collapses, joints dislocate, and Qi shifts, leading to skin and muscle ulceration. In the early stage, sweating therapy is suitable to open the Cou Li, using Wan Ling Dan. For predominance of Wind, Xiao Feng San and Ji Li Wan are used; for predominance of Dampness, Ku Shen Wan and Shen Shi Tang are selected. For severe toxicity, Shuang Jie San, Tong Sheng San, Ling Yang Jiao San, and Jie Du Tang can be chosen. These formulas address manifestations on the muscle-surface. The Lung and Stomach are most commonly affected. If the pathology involves the meridians, numbness occurs in the limbs and finger joints, accompanied by spasms and muscle wasting; later, feet may ulcerate and palms perforate. Upper facial numbness, facial paralysis (mouth deviation), tearing, and ectropion are signs of Wind-Dampness invading the meridians. Early treatment also requires sweating therapy, followed by selection of Ji Li Wan, Ku Shen Wan, Xiao Feng San, Li Shi Tang, or Tong Jing Tang. Avoid spicy, roasted foods, alcohol, and vinegar; protect against wind and rain; and abstain from sexual activity. Six or seven out of ten patients can be preserved; patients must not neglect the disease due to mild initial symptoms.
Wan Ling Dan: Treats carbuncles and sores at onset with aversion to cold and high fever, generalized stiffness and pain; also treats Leprosy with numbness and loss of sensation.
