Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction): This formula tonifies Qi and disperses toxin. It is also suitable for cases where the apex of the pustule is depressed. Ingredients: Dang Shen (Codonopsis Radix) 3 qian, Huang Qi (Astragali Radix) 2 qian, Bai Zhu (Atractylodis Rhizoma) 1.5 qian, Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma) 1 qian, Dang Gui (Angelicae Sinensis Radix) 2 qian, Chen Pi (Citri Reticulatae Pericarpium) 0.5 qian, Sheng Ma (Cimicifugae Rhizoma) 0.3 qian, Chai Hu (Bupleuri Radix) 0.3 qian. Decoct with fresh ginger; it can be taken alternately with Jing Fang Di Huang Tang.
Jing Fang Di Huang Tang (Schizonepeta, Saposhnikovia, and Rehmannia Decoction): This formula nourishes Blood and disperses toxin. For patients with Blood deficiency, taking three to four doses when the smallpox rash first appears facilitates the filling of the pustules. It can be taken alternately with other preceding or subsequent formulas without restriction. Ingredients: Jing Jie (Schizonepetae Spica) 1 qian, Shu Di Huang (Prepared Rehmanniae Radix) 4 qian, Shan Yao (Dioscoreae Rhizoma) 2 qian, Dan Pi (Moutan Cortex), Fang Feng (Saposhnikoviae Radix), Fu Ling (Poria), Shan Yu Rou (Corni Fructus), and Sheng Cao (Raw Glycyrrhizae Radix et Rhizoma) each 1 qian. Decoct with two large slices of fresh ginger as a guide, and take with yellow wine mixed in.
Da Wen Zhong Yin (Great Warming of the Middle Drink): This formula tonifies Qi and Blood, dispels cold pathogenic factors, elevates pus formation, and disperses smallpox toxin. In cases where the pustule apex does not rise, the shell is empty without pus, accompanied by vomiting and diarrhea, spleen and stomach function is impaired, the rash color is not red, and collapse is imminent, this formula should be decocted and administered immediately. It should be taken in large doses alternately with Da Bu Yuan Jian to warm the middle and dispel cold, causing rapid eruption; its efficacy is beyond full description. Ingredients: Shu Di Huang (Prepared Rehmanniae Radix) 5 qian, Bai Zhu (Atractylodis Rhizoma) 3 qian, Shan Yao (Dioscoreae Rhizoma) 2 qian, Dang Shen (Codonopsis Radix) 3 qian, Huang Qi (Astragali Radix) 2 qian, Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma) 2 qian, Chai Hu (Bupleuri Radix) 2 qian, Ma Huang (Ephedrae Herba) 1 qian, Rou Gui (Cinnamomi Cortex) 1 qian, Pao Jiang (Prepared Zingiberis Rhizoma) 1 qian. Decoct with three slices of fresh ginger using water infused with Zao Xin Tu (Furnace Hearth Earth); strain through linen cloth, add a small amount of yellow wine, and administer in multiple small doses. Do not omit Ma Huang; if sweating is excessive, remove Ma Huang.
Da Bu Yuan Jian (Great Tonification of the Origin Decoction): This formula greatly tonifies Qi and Blood, specifically treating smallpox complications caused by the mistaken use of cooling medicines, resulting in vomiting, diarrhea, failure of the rash to erupt, and critical condition. Large doses should be administered continuously without omitting Fu Zi (Aconiti Lateralis Radix Praeparata). It can be taken alternately with Liu Wei Hui Yang Yin to achieve miraculous results. If after two or three doses diarrhea persists, increase the dose of Fu Zi and add Gu Long (Concha Ostreae) and Su Ke (Pericarpium Papaveris) each 1 qian. If diarrhea completely stops, remove Fu Zi; excessive Fu Zi may cause urinary retention. Ingredients: Shu Di Huang (Prepared Rehmanniae Radix) 5 qian, Dang Shen (Codonopsis Radix) 3 qian, Shan Yao (Dioscoreae Rhizoma) 2 qian, Du Zhong (Eucommiae Cortex) 2 qian, Zao Ren (Ziziphi Spinosae Semen) 2 qian, Gou Qi Zi (Lycii Fructus) 2 qian, Yu Rou (Corni Fructus) 1 qian, Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma) 2 qian, Gu Zhi (Psoraleae Fructus) 2 qian, Bai Zhu (Atractylodis Rhizoma) 3 qian, Rou Gui (Cinnamomi Cortex) 2 qian, Fu Zi (Aconiti Lateralis Radix Praeparata) 1 qian. After the smallpox resolves, remove Fu Zi and use only a few fen of Rou Gui, administering several doses for regulation to restore vitality within days.
Liu Wei Hui Yang Yin (Six-Ingredient Decoction to Return Yang): This formula greatly tonifies Primordial Yang, specifically treating children with inherent Qi and Blood deficiency, presenting with white collapsed smallpox, or suffering from vomiting and diarrhea due to mistaken use of cooling medicines, approaching slow convulsions, and facing imminent danger. Administer this formula immediately. If there is any sign of recovery, add it to Da Bu Yuan Jian, decoct together, and administer repeatedly; this combination is named Fan Hun Dan (Resurrection Pill), truly a celestial prescription. Ingredients: Fu Zi (Aconiti Lateralis Radix Praeparata) 2 qian, Ju Jiang (Zingiberis Rhizoma Recens) 1 qian, Dang Gui (Angelicae Sinensis Radix) 3 qian, Rou Gui (Cinnamomi Cortex) 2 qian, Dang Shen (Codonopsis Radix) 3 qian, Zhi Cao (Prepared Glycyrrhizae Radix et Rhizoma) 1 qian. Add 0.3 qian of fine pepper powder and decoct with clarified Furnace Hearth Earth water. Alternatively, removing Fu Zi still yields Liu Wei Hui Yang Yin. Administration in larger quantities is preferred.
Bai Hu Di Huang Tang (White Tiger and Rehmannia Decoction): This formula clears excess fire and resolves pathogenic heat. It specifically treats children with smallpox presenting with persistent fever, thirst preferring cold drinks, black sunken lesions, dark scanty urine, constipation, and hot breath from mouth and nose. Adjust the dose of Da Huang (Rhei Radix et Rhizoma) until bowel movement is unobstructed. If urination and defecation are clear and profuse, and there is no desire for cold drinks, despite high body temperature, this indicates deficient fire. Warm tonification is still appropriate, following the principle of "sweet and warm reducing great heat." Do not use this formula recklessly; it is reserved for specific indications and should not be used lightly. Ingredients: Shi Gao (Gypsum Fibrosum) 1 qian, Sheng Di Huang (Rehmanniae Radix) 1 qian, Dang Gui (Angelicae Sinensis Radix) 3 qian, Zhi Qiao (Aurantii Fructus Immaturus) 1 qian, Da Huang (Rhei Radix et Rhizoma) 1.5 qian, Mu Tong (Akebiae Caulis) 2 qian, Sheng Cao (Raw Glycyrrhizae Radix et Rhizoma) 1 qian, Ze Xie (Alismatis Rhizoma) 1 qian. Use Deng Xin Cao (Junci Medullia) as a guide. Once heat subsides and the body cools, regulate with Jing Fang Di Huang Tang.
