In cases where there is mental restlessness, panting of the heart, delirious speech, and hallucinations of seeing ghosts, use the heart blood of a piglet that has not touched water mixed with Borneolum Syntheticum (Bingpian), formed into pills the size of a Chinese chestnut. Take one pill per dose, administered with wine decocted from Arnebiae Radix seu Lithospermi Radix (Zicao). Shortly after, stasis will be discharged, consciousness will clear, and the smallpox lesions will become red, vibrant, and erupt. This condition is beyond the treatment capability of ordinary physicians, yet this method is infallible and extraordinarily effective.
Another prescription: Use only Cimicifugae Rhizoma (Shengma) as a single herb, first decoct it into a concentrated soup, remove the dregs, take more than ten sheets of Carthami Flos (Honghua/Zhiyancai) and rub them in the soup to extract red juice, add approximately one qian of realgar powder (Xionghuang), then dip cotton stained with carmine-red paste into the soup to test on the sores (if it stains like carmine-red flower paste, it indicates the ability to invigorate blood and dissipate toxicity). If the sores do not rise or erupt, testing with this method is also viable, yielding extraordinary effects.
When lesions are present from above the chest to below the navel, but completely absent in the middle section, it is named "Two-Ended Smallpox" (Liangtou Dou). This occurs because Qi and Blood cannot circulate between the upper and lower parts, suggesting stagnation by cold-toxin in the region between the waist and navel. If not treated urgently, after seven days, it will inevitably transform into a grayish-white fatal syndrome. At the stage when spots first appear, urgently administer Rehmanniae Radix Preparata (Sheng Dihuang), Angelicae Sinensis Radix (Danggui), Paeoniae Radix Rubra (Chishao), Platycodonis Radix (Jiegeng), Saposhnikoviae Radix (Fangfeng), Magnoliae Officinalis Cortex (N Houpo), Dipsaci Radix (Xuduan), Angelicae Dahuricae Radix (Baizhi), Crataegi Fructus (Shanzha), and Akebiae Caulis (Mutong), along with thirty soybeans. After decoction and ingestion, if lesions emerge in the middle section, there will be no cause for concern.
When the head, face, and entire body have no empty space, resembling snake skin, hence the name. The lesions will inevitably become dry and fail to form pus, leading to death by the twelfth or thirteenth day. Previously, my granddaughter contracted smallpox in this manner. During fever, she experienced severe chills and teeth grinding, with extremely foul breath smelling like rotting food. I stated: "This is an ominous sign." Urgently administer detoxification using Jiedu Tang (Detoxifying Decoction), three to four doses daily. When the lesions became dense like silkworm eggs and indistinct in granularity, switch to Peucedani Radix (Qianhu), Chuanxiong Rhizoma (Chuanxiong), Moutan Cortex (Danpi), Gardeniae Fructus (Zhizi, stir-fried black), Arnebiae Radix (Zicao Rong), Hominis Placenta (Renzhonghuang), Forsythiae Fructus (Lianqiao), Arctii Fructus (Niubangzi), Scutellariae Radix (Huangqin), and Carthami Flos (Honghua). Continue for several doses until the lesions distinguish their granules. Subsequently, add Rehmanniae Radix Preparata (Sheng Dihuang) and Angelicae Dahuricae Radix (Baizhi), removing Hominis Placenta (Renzhonghuang), Arnebiae Radix (Zicao), Scutellariae Radix (Huangqin), and Carthami Flos (Honghua), allowing the fluid to gradually rise. Then use Shen Gui Lurong Tang (Ginseng, Angelica, and Deer Antler Decoction) added with Caryophylli Flos (Dingxiang), Cinnamomi Cortex (Rougui), Angelicae Dahuricae Radix (Baizhi), and Paeoniae Radix Alba (Baishao), followed by adding Atractodis Macrocephalae Rhizoma (Baizhu), at which point the pus becomes sufficient. During the pus-raising phase, the appearance resembled snake skin. Upon scab shedding, some pieces came off with the skin, leaving the whole body looking like shed snake skin; fingernails and toenails all fell off, and hair was completely replaced. The condition was indeed Snake-Skin Smallpox. With skilled treatment, safety could ultimately be achieved, demonstrating that even incurable conditions should not be abandoned without treatment. However, when the condition is malignant and fierce, medicines must attack vigorously; procrastination and hesitation cannot save the patient.
