The Qi of the Foot-Taiyang (Bladder) channel governs the exterior. When pathogenic factors injure the Taiyang, the exterior Qi becomes deficient, leading to aversion to wind; therefore, it is appropriate to drain the pathogenic factors from the Taiyang channel.) Severe pain that cannot be pressed indicates a disease of the Marrow (bone marrow pathology). Use a Chan needle (the first of the Nine Needles, shaped like an arrowhead for superficial pricking and bleeding) to puncture the Jiegu (GB39) acupoint until bleeding occurs, and the condition will cease immediately. ("Suan" sounds like "Chan". This refers to wind pathogens penetrating deeply into the bone marrow, where it is suitable to puncture the Jiegu acupoint of the Foot-Shaoyang (Gallbladder) channel, as the Shaoyang Qi governs the bones. "Sui" refers to the foot bones; "Tong" refers to pain on the dorsum of the foot. Since wind pathogens invade and injure the bone marrow, the pain is severe and intolerable to pressure. The Chan needle is the first of the Nine Needles, primarily used to drain Yang-heat Qi. Jiegu is located three cun above the lateral malleolus in the arterial pulse; insert two fen deep, retain for seven breaths. Ni Zhongxuan stated: The lateral aspect of the foot belongs to the region governed by the Foot-Yangming (Stomach) channel. Here, wind-Wood pathogenic factors harm the Stomach-Earth, hence it is named a disease of the Marrow.) Mild body pain should be treated by puncturing Zhiyin (BL66). (This discusses Wind-Malaria with body pain, for which it is suitable to select the Zhiyin channel. The Spleen is known as the Ultimate Yin within Yin, and externally governs the muscles of the four limbs; thus, the Classic states: Fullness in the Spleen collaterals causes pain throughout the body. Therefore, for mild body pain, it is suitable to puncture the channel of the Spleen organ, as this is also due to wind-Wood pathogenic factors harming the Spleen-Earth.) Do not bleed the Jing-Well points of all Yin channels; puncture once every other day. (This continues the previous discussion to explain that the Jing-Well points of the Hand and Foot Three-Yin channels are not suitable for bleeding. Since Jing-Well points are intersections of channel Qi, when pathogens are in the Qi aspect of the Yang, it is suitable to drain their blood; however, when the disease is in the Yin channels and the Yin Jing-Well points are indicated, one may puncture once every other day so that pathogenic Qi naturally dissipates, without needing to cause bleeding, thereby avoiding the depletion of True Yin Qi. Zhang Zhaohuang stated: This clarifies that the previously mentioned "puncturing Zhiyin" refers to puncturing the Zhiyin Jing-Well point, and further clarifies that the term "Zhiyin" here does not refer to the Zhiyin point of the Taiyang channel, but rather to the Zhiyin point of the Spleen channel.) In malaria without thirst occurring every other day, puncture the Foot-Taiyang (Bladder) channel; in malaria with thirst occurring every other day, puncture the Foot-Shaoyang (Gallbladder) channel. (This discusses the distinction between thirst and no thirst in malaria, involving the transformational Qi of Water-Fire and Cold-Heat. Above the Taiyang, Cold-Water governs; thus, those without thirst are treated via the Foot-Taiyang channel. Above the Shaoyang, Ministerial Fire governs; thus, those with thirst are treated via the Foot-Shaoyang channel. Occurring every other day indicates the pathogen has entered the interior. Pathogens entering the interior lead to thirst; therefore, those occurring every two days or several days involve alternating dominance of Yin and Yang, varying in severity. If Yang-pathogens enter the interior, thirst or lack thereof depends on the specific Cold-Heat dynamics.) For Warm-Malaria without sweating, apply the Fifty-Nine Acupoints needling method. (Warm-Malaria arises from contracting wind-cold in winter, with pathogenic Qi hiding in the Kidney. If there is no sweat, it indicates the pathogen cannot exit to the Yang aspect; therefore, the Fifty-Nine Acupoints method should be applied. The Fifty-Nine Acupoints method involves using the fourth type of needle to puncture the bone region to vent the pathogen.)
