Heat accompanied by panting. Puncture the Foot-Shaoyin [Kidney] channel. (Wang Bing states: Yongquan [KI-1] and Dazhong [KI-4] both govern this condition. Yongquan is located in the depression at the center of the sole, where the Foot-Shaoyin channel emerges. Dazhong is located behind the heel at the pulsating artery, belonging to the Foot-Shaoyin channel.) Difficulty in defecation. Puncture the Foot-Shaoyin [Kidney] channel. (Wang Bing states: Yongquan [KI-1] governs this condition.) Fullness in the lower abdomen. Puncture the Foot-Jueyin [Liver] channel. (Wang Bing states: Taichong [LR-3] governs this condition. It is located between the medial side and the proximal interphalangeal joint of the great toe, where the arterial pulse is palpable, governing the Foot-Jueyin channel.) Pain feeling as if broken, inability to bend forward or backward, inability to lift the body. Puncture the Foot-Taiyang [Bladder] channel. (Wang Bing states: For the sensation of being broken, Shugu [BL-65] governs; for inability to bend forward or backward, Jinggu [BL-64] and Kunlun [BL-60] both govern; for inability to lift the body, Shenmai [BL-62] and Puxan [BL-61] both govern. Shugu is located in the depression at the red-white flesh margin, distal to the proximal interphalangeal joint on the lateral side of the fifth toe, where the Foot-Taiyang channel pours in. Jinggu is located in the depression at the red-white flesh margin under the large bone on the lateral side of the foot, palpable upon pressure, where the Foot-Taiyang channel passes. Kunlun is located in the depression posterior to the lateral malleolus above the calcaneus, where a fine pulse beats and is palpable, where the Foot-Taiyang channel travels. Shenmai is located five fen below the lateral malleolus, covering the width of a nail, from which the Yangqiao vessel originates. Puxan is located in the depression below the calcaneus, an intersection of the Foot-Taiyang channel and the Yangqiao vessel. I opine that the acupoints selected by Mr. Wang merely inherited previous practices, or were arbitrarily chosen by physicians of that time, not deriving from the original intent of the Classic.) Low back pain radiating to the medial aspect of the spine. Puncture the Foot-Shaoyin [Kidney] channel. (Wang Bing states: Fuliou [KI-7] governs this condition. Regarding the text "coldness ascending from low back pain" up to here, apart from annotations and combined vermilion corrections, the New Corrections state: According to the Quan Yuanqi edition, *Jia Yi Jing*, and *Tai Su*, there is no such text from "coldness ascending from low back pain" to this point; it was added by Wang Bing. The current annotation stating "from coldness ascending from low back pain" to "combined with vermilion corrections" totaling nineteen characters is also not Wang Bing's words, likely added by later generations.) Low back pain radiating to the hypochondrium and pulling the lower abdomen, inability to lie supine. (This reiterates the low back pain caused by the collateral of the Foot-Taiyin [Spleen] channel. 'Kong' means pulling/radiating; ' ' refers to the hollow area below the costal margin. The collateral of the Foot-Taiyin channel merges with the Yangming channel from the thigh, ascends through the middle of the sacrum, intersects with the Jueyin and Shaoyang channels in the lower region, follows the sacrum, enters the abdomen internally, and ascends to connect with the throat. Hence, low back pain radiates to the lower abdomen and pulls the hypochondrium. Abdominal tension causes inability to breathe while lying supine. Note: This chapter continues the discussion on abdominal conditions from the previous chapter and simultaneously records puncture treatments for somatic low back pain. The three Yin and three Yang channels of the foot all circulate up and down along the waist. The Foot-Taiyin channel, from the medial thigh, enters the abdomen and connects to the Spleen, governing the interior of the abdomen. Therefore, the first section only discusses the Shaoyin and Jueyin channels, omitting the Foot-Taiyin. However, the branch of the Taiyin channel, passing from the thigh through the sacrum, can also cause low back pain. Thus, it is recorded again at the end of the chapter to inform students of the distinct differences between internal and external meridians of the soma.) Puncture the intersection of the lumbar and sacral regions, at the swelling above the two ankles. Determine the number of needle insertions based on the waxing and waning of the moon; the effect will be immediate upon needling. ('Shen' sounds like 'shen'. The intersection of the lumbar and sacral regions refers to the space between the hip bones below the waist, where the Foot-Taiyin, Jueyin, and Shaoyang channels intersect left and right, hence called the lumbar-sacral intersection. The two ankles refer to the protruding bones on both sides below the waist; 'Shen' refers to the raised muscle above these ankle bones. Determining the count by the moon's phases: On the first day of the waxing moon, one insertion; on the second day, two insertions, increasing gradually; on the fifteenth day, fifteen insertions; on the sixteenth day, fourteen insertions, decreasing gradually. When the moon waxes, human Qi and Blood gradually flourish; when the moon wanes, they gradually decline. Acupuncturists adjust the number of insertions according to the fluctuation of Qi and Blood. Exceeding the prescribed number depletes Qi; falling short fails to drain pathogenic factors. Thus, the lunar cycle serves as the guideline. Zhang Zhaohuang states: The beginning of the month is called 'Shuo', representing the first day of the new moon's growth.) Treat the left side with points on the right, and the right side with points on the left. (The major collaterals of the vessels flow from left to right and right to left. This indicates pathogenic factors lodged in the major collaterals, requiring treatment by selecting points between the left and right sides. If dealing with superficial floating collaterals that have split horizontally, one should collectively treat the horizontal vessels on the lateral aspect of the )
