The *Suwen* states: All malaria arises from wind pathogens. Why do they have specific times for accumulation and cessation? It is due to the mutual contention of Yin and Yang above and below, and the shifting between deficiency and excess.
When Yang merges into Yin, Yin becomes full and Yang becomes deficient, resulting in chills and trembling of the jaw (Guhan). This occurs because summer heat injury causes heat qi to hide within the skin but outside the stomach and intestines. When the couli (pores) are open, autumn qi arrives; sweat is produced and wind is encountered. The pathogen combines with the Wei Qi (Defensive Qi). Wei Qi travels through the Yang meridians during the day and the Yin meridians at night. When the pathogen follows Wei Qi outward to the Yang level or inward to the Yin level, the external and internal levels contend with each other, causing daily attacks.
In cases where attacks occur every other day, the pathogen is deep, pressing inward on the Yin level. Yang Qi alone flares outward, while Yin pathogens remain fixed internally. Yin and Yang contend but cannot exit, hence the every-other-day attacks.
Regarding late or early onset: The pathogen lodges in the Fengfu point (GV16), moving down along the Du Meridian. Wei Qi convenes at Fengfu once a day and night, moving down one vertebral segment each day. The pathogen first lodges in the back. Whenever it reaches Fengfu, the couli open. When the couli open, the pathogen enters deeply, causing the disease to manifest later.
Cold belongs to Yin Qi, and heat belongs to Yang Qi. If cold is injured first and then wind, there is initial cold followed by heat, named Cold Malaria (Han Nue). If wind is injured first and then cold, there is initial heat followed by cold, named Warm Malaria (Wen Nue). If there is only heat without cold, it is because Yin Qi has exhausted first, leaving Yang Qi in sole dominance. Symptoms include shortness of breath, vexation, heat in the hands and feet, and nausea, named Bi Malaria.
Excess conditions require purging; deficiency conditions require tonifying. Heat is excess; cold is deficiency. Cold cannot be warmed even by fire; heat cannot be cooled even by ice. When Yin overcomes and merges into Yang, or Yang overcomes and merges into Yin, Yin excess manifests as cold, and Yang excess manifests as heat. Thus, malaria is caused by the irregular actions of Yin-Yang, Wind-Cold, and Deficiency-Excess pathogens.
