The movement and combination of Yin and Yang lead to the formation of male and female bodies, which are divided into nine orifices and four limbs, constituting the five zang organs and six fu organs. Each part of the body has its specific location, making it difficult to discern the favorable (shun) and unfavorable (ni) changes. If one relies solely on the floating (fu) and sinking (chen) pulses at the cunkou (radial artery), mistakes in diagnosing children will inevitably occur. One must clearly examine the tiger's mouth (the radial side of the infant's index finger) and distinguish the three gates (San Guan): Feng Guan (Wind Gate), Qi Gate, and Ming Guan (Life Gate). By carefully analyzing these signs before prescribing medication, errors can be avoided. For infants under three years old, only the tiger's mouth pulse pattern is observed. For boys, the left hand; for girls, the right hand. The first segment of the second phalanx is called Feng Guan; if the pulse pattern appears here, the disease is in its initial stage. The second segment is Qi Gate; if the pattern appears here, the disease is difficult to treat. The third segment is Ming Guan; if the pattern appears here, death is imminent. Furthermore, the color of the pattern must be distinguished: if the three gates appear cyan, it indicates fright (with convulsions of the four limbs); if red, it indicates water fright; if black, it indicates human fright; if purple, it indicates dysentery or diarrhea; if yellow, it indicates thunder fright. If the pulse pattern extends through all three gates to the fingertip, it signifies acute fright syndrome (Ji Jing Feng), which is fatal, whereas other diseases may be treatable. If the pattern is cyan or red with a single straight line like a thread, it indicates spleen injury from milk/food retention and fright due to fever. If the patterns on both hands are identical, it indicates simultaneous occurrence of fright and food accumulation. If there are three lines, or if they are scattered, it indicates wind-phlegm generated by the lungs. A sound resembling a constrained clam suggests taihan (cold damage) and coughing. A fiery red color indicates diarrhea. The presence of black mixed with other colors indicates dysentery; more red than white indicates white dysentery; predominantly black indicates red dysentery. Purple mixed with other colors, accompanied by thirst but not deficiency, indicates a specific condition. A chaotic pulse pattern in the tiger's mouth indicates disharmony of qi. The visible colors of the pulse pattern include yellow, red, purple, cyan, and black, changing as the disease worsens. For instance, when red dominates in a yellow-red pattern, it turns purple; when purple dominates in a red-purple pattern, it turns cyan; when cyan dominates in a purple-cyan pattern, it turns black; and when the pattern becomes purely black, it is incurable. One must also distinguish the shapes of the patterns: "Long Bead" shape indicates food accumulation and stagnation, manifesting as abdominal pain, alternating chills and fever, and indigestion. "Approaching Snake" shape indicates disharmony in the middle jiao, attacking qi causing organ unrest, and dry retching. "Departing Snake" shape indicates spleen deficiency with cold accumulation leading to diarrhea, mental fatigue, and excessive sleep. "Bow Inside" shape indicates invasion by cold-heat pathogens, manifesting as heavy head and eyes, mental fright palpitations, fatigue, slightly cold limbs, and reddish urine. "Bow Outside" shape indicates phlegm-heat, mental confusion, fever, combined fright and food retention, characteristic of wind abscess syndromes. "Bar" shape (interpreted from 'Luan') indicates pathogenic heat, excessive phlegm generating wind, convulsions, and fright. "Fish Bone" shape indicates fright-phlegm-heat. "Water Character" shape indicates fright, irritability from accumulated heat, mental confusion, night crying, excessive phlegm, locked jaw, and weak convulsions. "Needle" shape indicates heat in the heart and lungs, extreme heat generating wind, palpitations, restlessness, mental fatigue, anorexia, excessive phlegm, and weak convulsions. "Penetrating Gate Shooting Finger" indicates that fright, wind, phlegm, and heat syndromes gather in the chest and diaphragm without dispersing. This shape signifies severe fright syndrome where fright transmits to the meridians, generating wind-heat; it is nearly fatal (nine deaths for one survival) and difficult to treat. These thirteen pattern shapes have varying degrees of severity; by investigating the root cause of the disease, the symptoms can be detailed.
