Pulse: The pulse in dysentery is often slippery; when pressed deeply, it feels weak or even imperceptible. A faint pulse at the Chi position indicates Yin deficiency; a choppy pulse suggests blood deficiency. A deep and fine pulse indicates a favorable prognosis, while a surging and wiry pulse indicates an unfavorable one.
Dysentery, regardless of whether it presents with red (blood) or white (mucus) discharge, should be treated as damp-heat. This is clear. Red corresponds to the Blood level, white to the Qi level. Dysentery with mixed red and white discharge, containing pus and blood, is caused by dysfunction of the Spleen and Stomach, food stagnation, and excessive accumulation in the intestines and stomach. Summer-dampness injuring the Spleen causes dysentery. Symptoms include abdominal pain, tenesmus (feeling of incomplete defecation), short and dark urine. Cases with cool body temperature and a slow pulse are easy to treat; those with fever and a wiry, rapid pulse are difficult to treat. In the first one or two days of onset, if the patient's Yuan Qi (Original Qi) is strong, use Xuanbai San (Mysterious White Powder) initially; for weak patients, use Shaoyao Tang (Peony Decoction) to unblock stagnation. After three or four days, as Yuan Qi gradually weakens, treatment should focus on regulating diet and modifying prescriptions. If dysentery persists, administer Shengui Shaoyao Tang (Ginseng and Angelica Peony Decoction) to regulate the Spleen and Stomach and tonify Yuan Qi. For chronic cases that do not resolve, Ren Shen Yang Zang Tang (Ginseng Nourishing Viscera Decoction) may be used with modifications. One must absolutely avoid using astringents like Ying Su Ke (Poppy Shell) abruptly too early; premature astringency may prevent the expulsion of internal stagnation, leading to Resting Dysentery (chronic/recurrent dysentery), or cause toxic heat to attack upward, resulting in chest and abdominal fullness, pain, nausea, vomiting, and hiccups, which becomes difficult to treat due to toxic vapors attacking the Stomach. If watery diarrhea precedes pus and blood, it indicates transmission from Kidney to Spleen and is hard to cure; if pus and blood precede watery diarrhea, it indicates transmission from Spleen to Kidney and is easier to cure. Other severe signs include passing bright red blood, stool resembling rotten dust or leaking roof water, a rectum open like a bamboo tube, accompanied by vomiting, hiccups, restlessness, and fever—all are difficult to treat. Stool resembling fish brain matter, with fever and a large, rapid pulse, indicates a critical condition with half chance of survival. Generally, in the first one or two days of dysentery, before Yuan Qi is depleted, treatment should focus on unblocking stagnation, following the principle of "treating obstruction with unblocking methods" (Tong Yin Tong Yong). After three or four days, forcing unblocking should be avoided to prevent damaging Yuan Qi; instead, clearing heat and detoxifying, along with nourishing the Spleen and Stomach, should be the main approach. The Classic states: "Moving blood heals pus and blood in stools; regulating Qi eliminates tenesmus." If tenesmus is caused by excess accumulation in the Large Intestine, it should be guided out; if caused by Qi deficiency and sinking in the Large Intestine, it should be lifted and tonified.
In initial dysentery, regardless of color, it is damp-heat. Strong individuals with initial dysentery should be treated with draining methods.
Xuanbai San (Mysterious White Powder): Treats initial dysentery with tenesmus, abdominal pain, and urgent pus/blood discharge. One dose cures strong individuals.
Qian Niu Zi (Pharbitis Seed - Black for red dysentery, White for white dysentery, both for mixed; half raw, half stir-fried and crushed), Sheng Di Huang (Rehmannia Root), Chi Shao (Red Peony Root), Gui Wei (Angelica Tail), Bing Lang (Areca Nut), Zhi Qiao (Bitter Orange, pith removed, bran-fried), E Zhu (Curcuma Rhizome, baked), Huang Lian (Coptis Rhizome) (each 1 qian), Da Huang (Rhubarb Root and Rhizome) (2 qian). Add Xiang Ru (Mosla Herb) (1 qian, stir-fried) in summer months.
Decoct one dose in water, take warm on an empty stomach. Stop when bowel movements occur two or three times.
Weak individuals with initial dysentery should be treated with clearing methods.

Decoct one dose in water, take on an empty stomach. For red dysentery, follow this formula. For white dysentery, add Wu Zhu Yu (Evodia Fruit) (1 qian), and stir-fry Huang Qin and Huang Lian with wine. For mixed dysentery, add Bai Zhu (Atractylodes Macrocephala Rhizome), Fu Ling (Poria), Chen Pi (Tangerine Peel), and Xiang Fu (Cyperus Rhizome) (each 1 qian).
For dysentery caused by heat accumulation leading to Qi stagnation, clearing heat and smoothing Qi is appropriate.
Li Xiao San (Instant Effect Powder): Treats red and white dysentery with mixed pus and blood, tenesmus, and pain. Stops after one dose.
Jing Huang Lian (Pure Coptis Rhizome) (4 liang, washed with wine, stir-fried with 3 liang Wu Zhu Yu [Evodia Fruit], discard Evodia), Chen Zhi Qiao (Aged Bitter Orange, pith removed, bran-fried) (2 liang).
Grind into fine powder. Take 3 qian per dose on an empty stomach, mixed with yellow wine. For diarrhea, use rice soup. For Jin Kou Li (Silent Mouth Dysentery), use aged rice soup.
For dysentery with persistent fever, wind pathogen exists in the intestines and stomach.
Cang Lin San (Granary Powder): Treats red and white dysentery with persistent fever, wind-pathogen heat toxin in intestines/stomach, and epidemic dysentery causing widespread silent mouth dysentery. Highly effective. It is Ren Shen Bai Du San (Ginseng Detoxifying Decoction) plus Huang Lian (Coptis Rhizome) and 300 grains of Chen Cang Mi (Aged Rice), decocted with Ginger and Jujube. Add Bing Lang (Areca Nut) and Mu Gua (Chaenomeles Fruit) if hands/feet hurt after dysentery. Add Chen Cang Mi (Aged Rice) and Shi Lian Rou (Stone Lotus Seed Flesh) for silent mouth dysentery.
For dysentery with fever and constipation, there is excess heat in both exterior and interior. Liu Yi Shun Qi Tang (Six-to-One Smooth Qi Decoction). (Formula see Shang Han section.) Treats dysentery regardless of red/white mix, abdominal pain, tenesmus, whole-body fever, dry mouth/thirst. Unblocking stops symptoms.

Shen Lian Tang (Ginseng and Coptis Decoction):
Ren Shen (Ginseng Root) (5 qian), Huang Lian (Coptis Rhizome) (1 liang).
Decoct one dose in water, sip throughout the day. If vomiting occurs, force drink again; even one sip down the throat brings improvement. Adding Shi Lian Rou (Stone Lotus Seed Flesh) (3 qian) is especially effective. Externally, apply mashed Field Snail to the umbilicus to guide medicine downward.
Kai Jin Tang (Open Silencing Decoction): Treats silent mouth dysentery. Passed down by Xu Yuanji.
Sha Ren (Amomum Fruit) (1 qian, ground), Sha Tang (Rock Sugar) (7 qian), Xi Cha (Fine Tea) (5 qian), Sheng Jiang (Fresh Ginger) (5 slices).
Decoct one dose in 2 zhong of water until 0.8 zhong remains. Leave exposed overnight. Take warm facing North next morning. Externally, grind Mu Bie Zi (Momordica Seed) (2 qian, shelled) and She Xiang (Musk) (0.2 fen), paste onto umbilicus to stimulate appetite.
Another prescription: Highly effective for silent mouth dysentery where grain cannot pass.
Grind Shi Lian Rou (Stone Lotus Seed Flesh) into powder. Take 2-3 qian per dose, mixed with Chen Cang Mi Tang (Aged Rice Soup). Effective immediately. Add 2-3 spoons of Sheng Jiang Zhi (Fresh Ginger Juice) if vomiting.
For chronic dysentery not stopping, tonifying Qi and Blood with lifting and astringing is appropriate.
Shen Gui Shaoyao Tang (Ginseng, Angelica, and Peony Decoction): Treats dysentery lasting 10-20 days, frequent and unceasing. Tonifies Qi and Blood for self-healing.
Ren Shen (Ginseng Root) (1 qian), Dang Gui (Angelica Sinensis, wine-washed) (2 qian), Fu Ling (Poria), Bai Zhu (Atractylodes Macrocephala Rhizome) (1 qian), Sha Ren (Amomum Fruit) (0.7 fen), Shan Yao (Chinese Yam, stir-fried), Chen Pi (Tangerine Peel) (each 1 qian), Gan Cao (Licorice Root) (0.5 qian).
Decoct one dose with 1 W Mei (Smoked Plum), a bundle of Deng Cao (Juncus Effusus Stem), and 7 Lian Rou (Lotus Seeds) in water. Take warm. Modify as follows: For silent mouth dysentery due to extreme Stomach heat, add stir-fried Huang Lian, Lotus Seeds, Ginseng, stir-fried Rice, and Smoked Plum to clear heat and open the stomach. For abdominal pain and tenesmus due to extreme heat and Qi stagnation: Tenesmus implies abdominal discomfort. Also, false urge to defecate without passage indicates Blood deficiency. Blood deficiency causes tenesmus; treat with Si Wu Tang (Four Substances Decoction) type formulas, adding Mu Xiang and Bing Lang to harmonize and eliminate stagnant Qi, resolving tenesmus.
Chronic tenesmus due to deficient Qi sinking: If purgatives fail to relieve tenesmus, it is Yang failing to rise. Use Sheng Ma (Cimicifuga Rhizome) as Sovereign herb, add Ren Shen and Dang Gui. Or use Shao Yao (Peony Root) as Sovereign, add a little Sheng Ma to lift Qi.
Generally, pain in dysentery indicates heat flowing downward; add stir-fried Huang Qin and Shao Yao to clear it.
Post-dysentery persistent fever, or only false urging with little stagnation, indicates Blood deficiency. Double Dang Gui, Shao Yao, and Di Huang (Rehmannia Root) to nourish Yin and Blood; fever will subside.
Purple blood in stool indicates Blood stasis; add Shao Yao and Hong Hua (Safflower) to generate and harmonize Blood; hematochezia will heal.
Stool resembling mung bean juice indicates Dampness; add Cang Zhu (Atractylodes Lancea Rhizome) and Bai Zhu to leach out Dampness and promote urination.
Shi Chang San (Intestinal Solidifying Powder): Treats chronic dysentery with excessive loss, any color. Changes stool to yellow.
Gan Shan Yao (Chinese Yam, stir-fried yellow) (1 liang), Hao Lian Rou (Good Lotus Seed Flesh, stir-fried, heart removed) (1 liang), Chao Huang Mi (Stir-fried Yellow Rice) (1 he).
Grind into fine powder. Mix with Rock Sugar and hot water to form a paste. Take gradually. Rinse mouth with clear rice soup afterward. Regular use is most effective.
Tang Pao Yin (Soup Soaking Drink): Treats chronic dysentery, any color.
Ying Su Ke (Poppy Shell, honey-water stir-fried) (3 qian), Wu Mei (Smoked Plum, pitted) (1), Gan Cao (Licorice Root) (0.3 qian), Mi (Honey) (3 spoons).
Crush, soak in 1 zhong boiling water for 1 hour. Strain. Take in three doses.
Zhen Ren Yang Zang Tang (True Person Nourishing Viscera Decoction): Treats adults and children with cold-heat imbalance, red/white dysentery, pus/blood/fish-brain-like stool, tenesmus, umbilical/abdominal pain, prolapse, or alcohol-induced hematochezia.
Rou Gui (Cinnamon Bark) (0.5 qian), Ren Shen (Ginseng Root, remove rhizome), Dang Gui (Angelica Sinensis), He Zi (Chebula Fruit, baked, pitted), Mu Xiang (Aucklandia Root), Gan Cao (Licorice Root, honey-fried), Rou Dou Kou (Nutmeg, flour-baked) (each 1 qian), Shao Yao (Peony Root), Bai Zhu (Atractylodes Macrocephala Rhizome) (each 1 qian), Zhi Qiao (Bitter Orange, honey-fried) (1 qian).
Decoct one dose in water. Take warm before meals. For Cold Viscera, add Zhi Fu Zi (Prepared Aconite Root) (1 qian).
For chronic dysentery unable to get up, anorexia, severe emaciation: Use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) minus Chai Hu (Bupleurum Root), add stir-fried Shao Yao, Ze Xie (Alisma Rhizome), Mu Xiang, Sha Ren, Bai Kou (Cardamom), Di Yu (Sanguisorba Root), and Vinegar-stir-fried Mi Ke (Poppy Shell).
Generally, chronic dysentery stems from Qi and Blood deficiency; Ba Zhen Tang (Eight Treasures Decoction) is suitable. For Spleen Qi deficiency with Blood weakness, use Si Jun Zi Tang (Four Gentlemen Decoction); for Stomach Qi deficiency with Blood weakness, use Bu Zhong Yi Qi Tang. If this condition persists or manifests various complications, adhering to previous methods leads to recovery over time. For Hematochezia and prolonged bleeding: Liu Wei Wan (Six-Ingredient Pill) plus Di Yu, E Jiao (Donkey-Hide Gelatin), Huang Lian, Huang Qin, and Sheng Di Huang. (The above four formulas are seen in the Tonification section.)

Grand Minister Chun Gang, Mr. Liu, nearly 70 years old, suffered from dysentery with pus, blood, and abdominal pain. Physicians failed. I diagnosed a slightly rapid pulse in all six positions. This was due to excessive rich/sweet foods causing internal heat accumulation. I prescribed 1 liang of Wine-steamed Da Huang (Rhubarb) to clear and drain. Mr. Liu said, "I am old and frail, I fear I cannot tolerate it. Only tonifying and mild agents are acceptable." I explained repeatedly. He finally agreed. He took it and recovered the next day.
Prefect Zhufeng, Mr. He, suffered from red/white dysentery day and night, generalized itching, and irritability. I diagnosed a large, rapid pulse in all six positions, with the Ren Ying (Carotid) pulse being predominant. This was Wind-Pathogen Heat Toxin. I prescribed Ren Shen Bai Du San (Ginseng Detoxifying Decoction) plus Fang Feng (Saposhnikovia Root), Jing Jie (Schizonepeta Spike), and Huang Lian, removing Ren Shen. Two doses cured him. Later, his pulse remained large and rapid. I said, "Rapid indicates irritability; large indicates disease progression. He will likely suffer from Phlegm-Wheezing and not recover." Over a month later, it happened exactly as predicted.
A patient had swollen feet, chest fullness, and scanty urine after dysentery. Using diuretic agents caused generalized edema and wheezing. I said, "Swollen feet indicate Spleen Qi sinking; chest fullness indicates Spleen deficiency forming痞 (Pi syndrome); scanty urine indicates Lung failing to generate Kidney; generalized edema/wheezing indicates Spleen failing to generate Lung." Treated with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) plus Fu Zi (Aconite Root). Recovered. Half a year later, due to alcohol, food, and fatigue, he had swollen eyes and scanty urine. Taking the previous prescription cured him instantly. (Formula see Tonification section.)
