The pulse being predominantly wiry indicates fluid retention (Yin), or it may present as deep-wiry-slippery, knotted-thready-hidden. When phlegm-fluids reside in the middle jiao (Spleen and Stomach), the pulse is often slippery, manifesting as wiry-slippery, deep-slippery, or slightly slippery. Fire-phlegm is black; old phlegm is sticky and difficult to resolve; dampness-phlegm is white; cold-phlegm is clear and thin.
Wang Yinjun stated: "Diseases caused by phlegm are difficult to clarify. They may cause dizziness, vertigo, and tinnitus; twitching of the mouth and eyes, pain in the supraorbital ridge, and itching of the auricle; wandering wind-like swelling and hardness in the four limbs; floating and painful/itchy teeth; belching with acid regurgitation; epigastric discomfort; pain or hiccups; difficulty in the throat, where phlegm cannot be expectorated nor swallowed. The phlegm may appear black, resembling broken cotton wadding, peach gum, or clam meat. One may feel as if ice is lodged in the epigastrium, with cold heart-pain, or experience strange dreams. Symptoms may include soreness and weakness of the ankles, sudden pain in the lower back, kidneys, and joints, or pain in the four limbs and tendons/bones. There may be numbness in the hands and feet, arm pain resembling wind-dampness, a red line rising daily along the spine, or a whole-body prickling sensation like lying on thorns. Eyes may feel sticky, dry, and itchy; the mouth may lock up, or there may be throat obstruction. Neck lymph node swellings may occur. A sensation of two qi streams knotting in the chest and abdomen may cause choking, stifling, and oppression. Mental confusion, mania, stroke, and paralysis may ensue. Wind-toxic Hong Kong foot (chronic ulcer of the lower limbs) may develop. Palpitations may feel as if one fears being hunted. Wheezing, coughing, and vomiting may occur, with expulsion of cold saliva, green water, or black juice. Severe cases may develop lung abscesses or sores, nausea, epigastric fullness, diarrhea with alternating chills and fever, and pus in the stool. Rumbling sounds may be heard in the chest. A specific point on the back may feel constantly cold. All these are caused by phlegm. In many diseases, phlegm is a concurrent pathogen, though this is unknown to the world. Treating various diseases should prioritize resolving phlegm. Those skilled in treating phlegm also regulate Qi; when Qi flows smoothly, phlegm is facilitated. For phlegm pulses that are superficial and slippery, descending methods are appropriate. For superficial and excess pulses, emetic methods are suitable; for deep and small pulses, emetics are contraindicated to avoid damaging healthy Qi. In treating phlegm syndromes, one must not rely entirely on diuretic/drainage herbs, as excessive use weakens Spleen Qi, causing phlegm to generate more easily and abundantly. If phlegm is generated due to Spleen deficiency, Spleen-fortifying and dampness-drying methods should be used, combining draining and tonifying effects, with Chenpi (Citri Reticulatae Pericarpium) and Baizhu (Atractylodis Macrocephalae Rhizoma) as adjuncts. Phlegm is turbid filth; it is better to induce vomiting than purging, otherwise other symptoms may arise.
Phlegm belongs to Dampness and is transformed from body fluids.
Erchen Tang (Two-Aged Decoction) treats all diseases arising from phlegm-fluids transforming into various ailments; this formula is indicated.
Chenpi (Citri Reticulatae Pericarpium, white pith removed), Banxia (Pinelliae Rhizoma, processed with ginger), Baifuling (Poria Cocos, skin removed), Gancao (Glycyrrhizae Radix et Rhizoma).
Grind one dose, add three slices of fresh ginger, decoct in water, and take orally.
For cough with white phlegm, it is due to Lung invasion by Wind-Cold. (For subsequent phlegm-related diseases, modify the above formula accordingly.) For Wind-phlegm, add Tianxing (Arisaematis Cum Bile), Jiegeng (Platycodonis Radix), Fangfeng (Saposhnikoviae Radix), Zhishi (Aurantii Fructus Immaturus). For Cold-phlegm, add Ganjiang (Zingiberis Rhizoma), Guongui (Cinnamomi Cortex).
For Food-stagnation phlegm, caused by overeating leading to long-term stagnation and phlegm formation (common in children), add Shanzha (Crataegi Fructus), Shenqu (Massa Medicata Fermentata), Xiangfu (Cyperi Rhizoma).
For cough with thread-like phlegm, it is due to Dampness in the Spleen and Stomach. Add Cangzhu (Atractylodis Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Shanzha (Crataegi Fructus), Sharen (Amomi Fructus Rotundus).
For Phlegm-Qi stagnation, characterized by distending pain in the chest and diaphragm due to phlegm-Qi (phlegm in the throat feels like cotton wadding or a plum pit, cannot be expectorated or swallowed, moves up and down, causing obstruction, eventually leading to dysphagia/Jieye disease), add Sharen (Amomi Fructus Rotundus), Xiangfu (Cyperi Rhizoma), Gualou (Trichosanthis Fructus), Zhishi (Aurantii Fructus Immaturus), Suzi (Perillae Fructus), Jiegeng (Platycodonis Radix), Danggui (Angelicae Sinensis Radix), Beimu (Fritillariae Bulbus), remove Banxia (Pinelliae Rhizoma).
For Phlegm-Fluids, where phlegm resides in the chest and diaphragm causing pain with sound, add Cangzhu (Atractylodis Rhizoma), Gualou (Trichosanthis Fructus), Zhishi (Aurantii Fructus Immaturus), Muxiang (Aucklandiae Radix), Sharen (Amomi Fructus Rotundus), Danggui (Angelicae Sinensis Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma), Xiangfu (Cyperi Rhizoma), Qingpi (Citri Reticulatae Pericarpium Viride), Baijiezi (Sinapis Semen). This is highly effective for phlegm-fluids.
For Phlegm-Saliva syndrome, characterized by unbearable pain throughout the chest, back, and hypochondrium (pulling pain, cold numbness in hands and feet indicates phlegm-saliva in the chest and diaphragm), add Baijiezi (Sinapis Semen), Sharen (Amomi Fructus Rotundus), Muxiang (Aucklandiae Radix), Huixiang (Foeniculi Fructus), Xiangfu (Cyperi Rhizoma), Zhishi (Aurantii Fructus Immaturus), Danggui (Angelicae Sinensis Radix), Jiuhuangqin (Scutellariae Radix stir-fried with wine).
For Phlegm-Dampness flowing and depositing, characterized by lumps throughout the body (wandering bone/joint pain or alternating chills and fever indicate damp-phlegm flowing into meridians), add Gualou (Trichosanthis Fructus), Zhiqiao (Aurantii Fructus), Cangzhu (Atractylodis Rhizoma), Jiuqin (Scutellariae Radix stir-fried with wine), Qianghuo (Notopterygii Rhizoma et Radix), Fangfeng (Saposhnikoviae Radix), Lianqiao (Forsythiae Fructus), Danggui (Angelicae Sinensis Radix), Xiangfu (Cyperi Rhizoma), Sharen (Amomi Fructus Rotundus), Muxiang (Aucklandiae Radix), Honghua (Carthami Flos), Zhuli (Bambusae Sap), Jiangzhi (Zingiberis Succus); for heat, add Chaihu (Bupleuri Radix); for upper pain, add Chuanxiong (Ligustici Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix); for lower pain, add Huangbai (Phellodendri Cortex), Niuxi (Achyranthis Bidentatae Radix); for lump pain, add Rushxiang (Olibanum), Moyao (Myrrha); for head/neck pain, add Weilingxian (Salviae Miltiorrhizae Radix et Rhizoma). For lump pain externally, add Wubeizi (Gallae Chinensis), Xiaoyan (Natrii Sulfas), Dahuang (Rhei Radix et Rhizoma), Nanxing (Arismae Rhizoma); grind these four into fine powder, mix with vinegar, apply to the lump to gradually dissipate; if it does not dissipate, it will suppurate.
For Phlegm Nuclei (Tanhe), lymph node swellings anywhere in the body that do not resolve (lumps are phlegm masses; treatment for phlegm masses, flowing deposits, and nuclei is similar to damp-phlegm flowing deposits), add Zaocili (Gleditsiae Spina) to guide herbs to the toxic site. For damp-phlegm, add Cangzhu (Atractylodis Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Sharen (Amomi Fructus Rotundus), Xiangfu (Cyperi Rhizoma), Zhiqiao (Aurantii Fructus), Jiegeng (Platycodonis Radix); for heat-phlegm, add Huangqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Beimu (Fritillariae Bulbus), Zhishi (Aurantii Fructus Immaturus), Jiegeng (Platycodonis Radix), Maidong (Ophiopogonis Radix), Zhuli (Bambusae Sap), remove Banxia (Pinelliae Rhizoma); for Phlegm-Fire, add Chao Huanglian (Coptidis Rhizoma stir-fried), Zhuli (Bambusae Sap), Beimu (Fritillariae Bulbus), remove Banxia (Pinelliae Rhizoma).

For old phlegm, add Zhishi (Aurantii Fructus Immaturus), Gualou (Trichosanthis Fructus), Haishi (Arcae Concha), Lianqiao (Forsythiae Fructus), Xiangfu (Cyperi Rhizoma), Huangqin (Scutellariae Radix), Jiegeng (Platycodonis Radix), Beimu (Fritillariae Bulbus), remove Banxia (Pinelliae Rhizoma).
For cough with yellow phlegm expectoration, it is due to Heat in the Spleen and Stomach (if prolonged, it becomes Feiwei/Lung萎 [atrophy], with expectoration of pus or bloody phlegm with foul odor, which is difficult to treat). Add Gualou (Trichosanthis Fructus), Zhishi (Aurantii Fructus Immaturus), Jiegeng (Platycodonis Radix), Pianqin (Scutellariae Radix), Zhizi (Gardeniae Fructus), Tiandong (Asparagi Radix), Sangbaipi (Mori Cortex), Xingren (Armeniacae Semen Amarum), Suzi (Perillae Fructus), Zhuli (Bambusae Sap).
For expectoration of black phlegm in clumps, it is due to exhausting the Heart and Kidneys. It is all old phlegm from long-term stagnation; treat similarly.
For Spleen-deficiency generating phlegm, add Baizhu (Atractylodis Macrocephalae Rhizoma), Renshen (Ginseng Radix et Rhizoma), Baishao (Paeoniae Radix Alba), Zhishi (Aurantii Fructus Immaturus), Sharen (Amomi Fructus Rotundus), Jiegeng (Platycodonis Radix).

If vomiting and hiccups with phlegm occur after drinking alcohol, add Sharen (Amomi Fructus Rotundus), Wumei (Mume Fructus). (Modifications of the preceding Erchen Tang end here.)
Inability to speak is due to Phlegm confusing the Heart Orifice. Inability to expectorate phlegm is due to Phlegm knotting. Phlegm in the chest/diaphragm that fails to transform, combined with deficient Yuan Qi, dry body fluids, inability to expectorate, wheezing/cough with body heat, and pain making turning difficult, is Phlegm knotting. Phlegm under the ribs causing alternating chills/fever, cough, rapid breathing, and pain is also Phlegm knotting. Bubbling sound in the throat, rapid breathing, and inability to expectorate phlegm is difficult to treat. Qi stagnation causing wheezing and insomnia is difficult to treat. After taking medicine, expectorating phlegm indicates efficacy; inability to expectorate phlegm is also difficult to treat. (The above six conditions are suitable for the following formulas.)
Gualou Zhishi Tang (Trichosanthes and Immature Bitter Orange Decoction) treats Phlegm knotting with inability to expectorate, chest/diaphragm pain, inability to turn, or Phlegm knotting in chest/diaphragm causing fullness, chills/fever, rapid breathing, and Phlegm confusing the Heart Orifice causing inability to speak; it treats all these.

Generally, taking medicine must be done when going to bed, using one mouthful of warm water to send it past the throat, then lie supine immediately, letting the medicine descend slowly between the pharynx and diaphragm. Do not eat/drink or get up/move/speak for half a day. Wait until the pill slowly reaches the Upper Jiao, phlegm stagnation and evil matter pass the diaphragm into the abdomen, then move. Each course requires two consecutive nights. If the first night's dose results in 3-5 evacuations of phlegm/evil matter the next day, reduce the next night's dose by 10 pills. If 2 evacuations, keep the previous number; if 5-7 or only 2-3 evacuations and disease势头 suddenly stops, reduce the next night's dose by 20 pills. If the first night's dose produces no evacuation of evil matter, increase the next night's dose by 10 pills. Robust individuals with excess disease may take up to 100 pills. Only severe mania/excess and sudden critical conditions are ineffective with high doses. If after taking medicine and lying down, the throat feels thick/stagnant and obstructed, it is due to Phlegm-Qi ascending and medicine/pathogen attacking each other. Soon, as medicine power prevails, it naturally settles. Or one in a hundred may have slight abdominal pain, back stiffness, due to stubborn phlegm. Evil matter stagnation blocking Qi, slippery intestines, tenesmus, resembling dysentery, resolves quickly. If phlegm-saliva is easy to evacuate, the relief is indescribable, saliva fills the mouth instantly, all orifices feel refreshed. Occasionally, there is momentary fatigue, due to days of sickness and unrest, temporarily overcome by medicine, body temporarily harmonized, like waking from drunkenness, like just bathing, like just waking from sleep, not true deficiency/fatigue. This drug does not cause severe diarrhea or scraping the intestines, but only takes phlegm accumulation and evil matter from chest/stomach sequentially downward, not harming epigastric discomfort. Only lower sticky feces are where medicine power does not reach, hence remove the rest first. I do not elaborate further.
