Pulse: In cases of bone-steaming consumption fever (a sensation of heat emanating from the deep marrow due to Yin deficiency), the pulse is rapid and deficient; if the pulse is hot, choppy, and small, death is inevitable. If accompanied by sweating and coughing, it cannot be cured by medicine alone.
The syndrome of deficiency and timidity arises from insufficient primordial Qi, deficiency of the Heart and Kidney, or damage to Qi and Blood due to overwork, or excess in alcohol and sexual activity, gradually leading to depletion of True Yin and hyperactivity of Ministerial Fire. When Fire is excessive, it consumes True Yin, resulting in cough, asthma, phlegm, fever, hematemesis and epistaxis, night sweats, seminal emission, upper excess and lower deficiency. Symptoms include heat in the palms and soles, dry skin, aversion to cold in the afternoon, fever at night, or continuous day and night; gastric discomfort, palpitations, vomiting and hiccups, irritability, chest and abdominal pain, fullness and diarrhea, palpitations with fright, pale face with red lips, dizziness, soreness and pain in the waist and back, fatigue and weakness in the four limbs, dark red urine, and a pulse that is rapid and large, or deficient, fine, wiry, and urgent. There are many bizarre symptoms; this condition is difficult to treat. Consumption that cannot tolerate tonification is difficult to treat; hoarseness with ulcers in the throat is difficult to treat; bedsores developing after prolonged bed rest are difficult to treat. All these are caused by Yin deficiency with Fire hyperactivity. They should be treated with Ziyin Jianghuo Tang (Nourish Yin and Descend Fire Decoction) modified, or Qingli Zikan Tang (Clear Li and Nourish Kan Decoction), followed by Ziyin Qinghua Gao (Nourish Yin and Clear Transformation Paste) and Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia). After recovery, Kanli Jiji Wan (Kan and Li Mutual Completion Pill) should be taken as a post-recovery maintenance remedy. The "Lao" syndrome is essentially extreme deficiency involving Phlegm and Blood disorders. It begins as Yin deficiency and timidity, later developing into Lao; the treatment is the same. The pulse for Lao is rapid, large, and deficient. There is also the syndrome of Chuan Shi Lao Zhai (Infectious Consumption Disease), where insects in the viscera gnaw on the Heart and Lungs, named "Zhai". This is Chuan Shi Zhu Gu Lao (Infectious Bone-Consuming Consumption). "Zhu" means to transmit. It transmits from top to bottom, passing through flesh and bone, even leading to family extinction in some cases.
Xu Lao (Deficiency Consumption) is characterized by Yin deficiency and hyperactivity of Ministerial Fire (Yin deficiency with Fire hyperactivity is difficult to treat; Xu Lao that cannot tolerate tonification is difficult to treat).
Ziyin Jianghuo Tang (Nourish Yin and Descend Fire Decoction): Treats Yin deficiency with Fire hyperactivity, fever and cough, expectoration with asthma and urgency, night sweats, and dry mouth. Taking this formula alternately with Liuwei Dihuang Wan greatly tonifies Xu Lao with miraculous effects.

If Kidney Water fails to control Spleen Earth causing much phlegm and saliva, take with Ginger Soup. For those whose desires were awakened too early in childhood, damaging the root, or those with weak constitution who have overindulged and hide it, leading to exhaustion of Primordial Qi, night sweats, seminal emission, mental fatigue, poor muscle formation, pale face, heat in the five centers, fear of heat in summer and cold in winter, lower back pain and heavy knees, dizziness, and blurred vision—thus saying when Water is deficient, Fire prevails; when Fire is excessive, Metal (Lung) is controlled, leading to phlegm and cough. Or wind entering sweat pores causes acne. Once these symptoms appear, deficiency is established. Taking this medicine can ensure safety.

People only know that hundred diseases arise from the Heart, but do not know that hundred diseases arise from the Kidney. Drinking alcohol and eating meat, having sex while drunk and full, lacking restraint in desire, acting recklessly, damages Essence. Kidney Water becomes empty and cannot calm Heart Fire; Heart Fire blazes unchecked, damaging Lung Metal, cutting off the source of Kidney Water. With Metal and Water depleted, they cannot control Liver Wood; Liver Wood becomes excessive, controlling Spleen Earth and generating Fire. Fire burns alone without transformation, so Yang is in excess and Yin is deficient. The disease manifests as isolated heat and does not last long.
Two Warning Verses: Alcohol, lust, wealth, and anger are thieves harming people; how many heroes have been bewitched by them? If one can shake off this mortal dust, one becomes a guest beyond the nine heavens. What matters in floating life cause bias? Cruelly craving flowers and wine harms life; once bedridden with illness, regretting too late, spending all gold cannot cure it.
Liu Cheng'an, son of the Imperial Academy scholar, aged eighteen, suffered from Xu Lao with hot cough, phlegm, and asthma, red face, spontaneous sweating, unable to lie down day or night, with incessant phlegm. After ten days, life was imminent, the family anxious, doctors helpless. I was summoned to diagnose; the six pulses were slightly rapid, indicating Yin deficiency with Fire hyperactivity. I instructed him to drink one cup of milk from a vigorous woman at dawn (Wu Geng), warmed in hot water, taken in 30-40 small sips. By morning, he took one dose of Heche Dihuang Wan (Placenta Rehmannia Pill). Shortly after, he ate a thin porridge made with millet, rice, Shan Yao, Lian Rou, red dates, and walnuts. Half a day later, he took two doses of Qingli Zikan Tang with a little Zhu Li, Tong Bian, and Jiang Zhi, taken frequently. By noon, he ate another bowl of the porridge plus Baixue Gao (Snow White Cake). Half a day later, he finished two more doses of the previous medicine. In the evening, he ate another bowl of porridge and two more doses of the medicine. At night, he stopped taking medicine when sleeping and resumed immediately upon waking. For three days and nights, medicine was constantly taken, Fire gradually subsided, and he could finally lie down. Afterwards, the dosage was halved. After half a month, the illness reduced by 60-70%. He took only one dose of decoction daily and recovered after months of regulation. This case was critically dangerous; without my method, if treated normally with one or two doses daily, he would hardly have survived. A cup of water cannot extinguish a cartload of burning wood; this is exactly what is meant. Those suffering from this illness thereafter should take medicine as described, and doctors should treat accordingly; none will fail to recover. Those with weak Spleen and Stomach causing diarrhea are excluded from this rule.
