Shen (19 years old) Appetite is normal. Restlessness and fatigue. Sweating of hands and feet. Slight yellowing of the eyes. Frequent epistaxis. Heat consumes grain; water and grain stagnate to produce dampness; severe dampness generates heat; essence and nutrients fail to distribute to the four extremities, thus causing restlessness and fatigue. Over time, this leads to Wei Huang (Wasting-Yellow syndrome) and Gu Dan (Grain Jaundice). Treatment should employ Zhudu Wan (Pig Stomach Pill), replacing Baizhu (Atractylodis Rhizoma) with Cangzhu (Atractylodis Rhizoma), and heavily using Kushen (Coptidis Rhizoma). (Gu Dan / Grain Jaundice)
Zhang (32 years old) Describes that the initial illness resembled malaria. This was due to Summer-heat pathogen lying latent first, followed by Autumn-coolness invading subsequently. Because of dietary indiscretion, Qi stagnation in the epigastrium generated heat, steaming internally to manifest as Jaundice. This belongs to Gu Dan (Grain Jaundice) among the Five Jaundices. Urine is yellow, stool is constipated. The method is to ventilate and clear Damp-Heat from the Fu organs, but purgatives are contraindicated, lest one violate the Taiyin (Spleen/Stomach) and cause abdominal distension.

Liu (39 years old) Pain below the heart for over a year, recurring frequently. When pain subsides, appetite returns. Gradually developed yellow eyes and red urine. This is caused by congealed stasis and accumulated heat in the collaterals, steaming together with food and drink qi. If attacked too urgently, it must transform into abdominal distension. Warm-dry herbs must be avoided. Proposal: use Hejian Jinlingzi San (Jinlingzi Powder from Liu Wansu), combined with Wuze Guya Zhishi Xiaochaihu Tang method (No-Select Barley Sprout, Aurantium, and Minor Bupleurum Decoction method). Jiang Transformed from Jaundice to Distension. Doubtless Damp-Heat. The method is not wrong. According to the account, only taking small pills is likely not the method of Hejian Zhang Zihe. Warm-purgative methods only attack cold accumulation and cannot drive out Damp-Heat. Still propose Bitter-Acrid and Leaching methods. Zhang (32 years old) Summer-Autumn Jaundice diseases are formed by Damp-Heat steaming. Treatment must use Qi-level ventilation to be effective. For Dampness generates Heat within, external interference from seasonal factors, internal accumulation of undigested food and drink, and Yellow is the color of the Spleen and Stomach. Failure to treat properly leads to Distension. Now, daily treatment reduces symptoms, bowel movements are smooth, the main Fu organ is unblocked, and a light diet can achieve complete recovery. Usually bitter-tasting, excessive thinking injures the Heart, stagnation injures the Spleen, prolonged sitting causes excessive ascending and insufficient descending. This is not the same category as typical Jaundice cases. (Post-Jaundice Stagnation Injuring Heart and Spleen) Guipi Wan (Restore the Spleen Pill).
Yang (70 years old) Summer heat leaks Qi, Spleen fluid escapes outward causing yellowing. This is not Damp-Heat Jaundice. Subsequently, loss of appetite and loose stools. Used Banxiatang (Pinellia Decoction) to open the Stomach and promote fluids. Sleep and eating have been restored. Dew-falling phlegm-blood indicates Qi leakage failing to consolidate, and Descending function is shallow. Do not treat as a young person's heat syndrome. Follow the Qi of Heaven; this is the regulation method for the elderly. (Spleen Fluid Escaping Outward)

Jaundice refers to yellowing of the body, eyes, and urine. The disease arises from Dampness, having Yin and Yang aspects, affecting Fu and Zang organs. The onset of Yang Jaundice: Dampness transforms via Fire, Stasis-Heat resides in the interior, Gallbladder heat causes fluid leakage, merging with Stomach turbid Qi. Unable to ascend and vent, unable to descend and excrete, steaming and obstructing, invading the Lung causes whole-body and eye yellowing. Heat flows to the Bladder, changing urine color to red. Yellow like orange peel color. Yang governs brightness; treatment focuses on the Stomach. The onset of Yin Jaundice: Dampness follows Cold-Water, Spleen Yang cannot transform heat, Gallbladder fluid is obstructed by Dampness, soaking the Spleen, infiltrating muscles, overflowing skin. Color like smoked yellow. Yin governs darkness; treatment focuses on the Spleen. Shaoyang Jaundice (from Cold Damage) and Jingui Jaundice (from Golden Chamber) have different names but similar treatments. There are thirty-five diagnostic patterns and twelve formulas. First examine whether Jaundice will develop based on the free flow or obstruction of urination. Whether Jaundice is easy or difficult to cure depends on thirst or lack thereof. Then investigate causes of Stasis-Heat entering the Stomach: external invasion, internal generation, food intake, heavy drinking, labor or sexual excess. There is blood stasis following meridians, water-induced sweat turning yellow. Upper excess presents as whole-body heat; lower stagnation presents as difficult urination. There are also exterior deficiency and interior deficiency, where removal of heat leads to hiccups, fire coercion causes yellowing. Knowing diseases have various causes, treatments must be orderly. Thus, wiry pulse and hypochondriac pain indicate Shaoyang not yet resolved; still prioritize harmonizing. Thirst and desire to drink fluids indicate Yangming transforming to dryness; urgently purge heat. Dampness above uses Acrid dispersion, overcoming with Wind. Dampness below uses Bitter drainage and Acrid leaching. Like mania and blood stasis, attack is necessary. After sweating, white urine indicates appropriate tonification. Wine drinkers often harbor internal heat; first clear the center, add leaching methods, then protect Spleen Yang. Female labor involves turbidity; start with detoxification, follow with opening orifices, finally strongly tonify Kidney Yin. Exterior deficiency solidifies Wei Qi; interior deficiency builds the Middle. Cases involving fire/water coercion and inverted transformations each have formulas and discussions to guide students. Regarding treatment of Cold-Damp in the interior, only one case appears in the Yangming chapter without a formula, pointing to seeking within Cold-Damp. The Spleen fears Wood, prefers Wind-Dryness, controls Water, and dislikes Cold-Damp. Now, the Yin Jaundice syndrome, externally not due to Six Evils, internally not injured by desires, is purely Cold and Damp. Compare to low-lying soil needing wind and sun. Pure Yin disease must be treated with Acrid-Hot herbs undoubtedly. Although no formula is given, the method is clear, so no diversification is needed to avoid confusing people. Examining other scholars' views: Danxi said there is no need to distinguish Five Jaundices, all are like soy sauce, claiming this grasps the essence of treating Jaundice. Little does he know, mixing this to treat Yang Jaundice may not hit the mark but won't worsen it; using it to treat Yin Jaundice causes death upon ingestion, akin to wielding a blade. One careless statement misleads future generations. Only Luo Qianfu possesses outstanding insight, vigorously distinguishing Yin and Yang, following the Cold-Damp indication of Cold Damage, proposing Yinchen Sini Tang (Artemisia and Aconite Decoction) treatment, continuing past and opening future, saving lives with skill. Medicine is a minor art, but its merit is great. Yu Jiayan claimed Zhongjing's formulas for Yin Jaundice were lost, seeming directionless. Unexpectedly, his commentaries on Cold Damage and Golden Chamber span thousands of words, yet at the critical point, he remains obscure. Though called a wise man's slip, it shows a lack of deep understanding. In short, Luo deserves praise for diligent study of ancient teachings; Zhu failed due to complacency with partial success; Jiayan Yu erred in loving to debate. Observing Master Ye's few Jaundice cases, clearly stating that Summer-Autumn Jaundice is formed by Damp-Heat steaming, his treatment of Yang Jaundice is clear in mind. The cases also discuss treating Jaundice while identifying non-Jaundice conditions, deducing that determining disease requires seeking Deficiency/Excess, thus knowing disease requires distinguishing Yin/Yang. Those seeking treatment for Yin Jaundice from Mr. Ye will certainly not use Yang Jaundice methods to treat Yin, thereby prolonging life. Unless one models oneself on Zhongjing while disregarding Danxi, extensively studying sages' theories without falling into one-sided views, how could one reach this level? Fame not floating above reality, the Dao's ability to endure is indeed fitting. (Commentary by Jiang Shiyu)
Xu's Commentary Jaundice variations are numerous. The cases only present one method: draining Damp-Heat. There are no other good formulas. Ignorance of the fact that Jaundice disease, if mild, heals quickly; if severe, forms pockets of yellow water that do not resolve for a long time, changing endlessly, even leading to death. Methods to dissolve water pockets must be examined.
