Scrotal carbuncle (Nang Yong) is caused by Yin deficiency leading to the flow and accumulation of damp-heat into the scrotum, where it condenses to form swelling. Even after ulceration, when the testicles hang exposed, it does not necessarily endanger life, as the pathogenic toxin emanates from the exterior. Treatment should primarily focus on nourishing Yin and clearing/drainage of damp-heat.
Ruyi Huangjin San (如意黄金散): Applied topically for early-stage scrotal carbuncle presenting with redness, swelling, pain, fever, and a sensation of heaviness or dragging in the lower region.
Ingredients: Tiaranfen (Fen Sheng Mai Dong), Huangbai (Phellodendron Bark), Dahuang (Rhubarb Root), Jianghuang (Turmeric Rhizome) (each 0.5 fen); Baizhi (Angelica Dahurica Root) (3 liang); Nongpo (Magnolia Bark), Chenpi (Tangerine Peel), Gancao (Licorice Root), Cangzhu (Atractylodes Rhizome), Nanxing (Arisaema Rhizome).
Usage: Grind the above herbs into fine powder. Mix with juice of green onion and honey, then apply externally to the affected area.

Perineal abscess (Xuan Yong) is formed due to the deficiency of the Three Yin meridians and the accumulation of damp-heat. This acupoint is located anterior to the anus (Gu Dao) and inferior to the genitalia, also known as the Haidi acupoint.
Initially, it appears like a bean, with slight itching but predominant pain. Over time, it gradually enlarges, becoming red, swollen, and intensely painful. After ulceration and pus discharge, mild cases may develop into fistulas, while severe cases may exhaust Qi and Blood, transforming into consumptive disease (Lao Zhai).
Neixiao Xuoxue Tang (内消沃雪汤): Used to treat early-stage perineal abscess accompanied by aversion to cold and pain. (Formula see section on Carbuncles and Ulcers).
Hupo Lafan Wan (琥珀蜡矾丸): Used to treat perineal abscess, as well as all types of carbuncles and back sores, during the stage where pus is forming or has formed. It is taken prophylactically to prevent the toxin from failing to exit and invading inward, thereby protecting the Pericardium membrane, dispersing toxicity, and relieving pain.
Ingredients: Mingfan (Alum) (1.5 liang 5 qian), Huangla (Yellow Beeswax) (1.2 liang); Hupo (Amber), Zhusha (Cinnabar), Xionghuang (Realgar) (each 2 qian); Ruxiang (Frankincense) (1 qian); Fengmi (Honey) (appropriate amount).
Usage: Grind all aforementioned medicines into fine powder. Melt the beeswax, mix with the medicine powder and honey evenly. Roll into pills quickly while warm, sized like mung beans. Coat with Cinnabar powder. Take 30 pills each time with warm boiled water. For severe conditions, take twice daily (morning and evening) for miraculous efficacy.
