The four limbs are weak and flaccid, the body feels heavy and sluggish, unable to get out of bed for years, yet appetite remains normal without pain or soreness. It appears similar to dampness but is actually not dampness; rather, it is heat affecting the Lung Meridian (Lung Channel), causing the lung leaves to wither and hang down, unable to govern the qi of the whole body, thus forming Wei Syndrome (Flaccidity Syndrome).
*Ju Yao* states: If the Chi pulse (Kidney pulse) is weak, deficient, slow, choppy, and tight, or if it is flooding and large without root, it is intractable.
**Run Fei Fu Qi Tang (Moisten Lung and Support Qi Decoction)** treats Wei Syndrome (Flaccidity Syndrome) characterized by dry lungs and weak qi.
Ingredients: Poria (Fuling), Ginseng (Renshen), Atractylodes Macrocephala (Baizhu), Licorice Root (Gancao), Ophiopogon Tuber (Maidong), Scutellaria Root (Huangqin) (each 1 qian); Platycodon Root (Jiegeng), Lily Bulb (Baihe), Coix Seed (Yiyiren) (each 8 fen); Schisandra Berry (Bei Wuweiizi) (9 berries); Angelica Sinensis (Danggui), Rehmannia Root (Shengdi) (each 1.2 qian).
Preparation: Decoct in water, add half a cup of lotus root juice, take after meals.
Another prescription: Treats lung dryness caused by fire damaging the lungs, leading to an inability to govern qi.
Ingredients: Gardenia Seed (Shanzhiren) (stir-fried until black), Trichosanthes Seed (Gualouren) (oil removed), Angelica Sinensis (Danggui) (washed with wine), Rehmannia Root (Shengdi), Stemona Root (Baibu) (each 1.5 qian); Placenta Humanum (Ziheche) (2 qian); Asparagus Tuber (Tianmendong) (heart removed), Tangerine Peel (Chenpi), Platycodon Root (Jiegeng) (honey-fried), Ginseng (Renshen), Glehnia Root (Shashen) (each 1 qian).
Preparation: Add two jujubes and thirty stalks of Juncus Effusus (Dengxincao), decoct in water, take after meals.
The term "Nue" (Malaria/Intermittent Fever) implies cruelty or tyranny. Etymologically, it contains the disease radical, hence named Malaria. This disease often occurs in autumn, resulting from summerheat injury sustained in summer, manifesting in autumn. Symptoms vary: chills preceding fever, fever preceding chills, chills only without fever, fever only without chills, severe chills and fever, or mild chills and fever. Analyzed separately: Chills preceding fever indicates initial cold pathogen invasion; fever preceding chills indicates initial heat pathogen invasion. It is also said that fever preceding chills relates to blood deficiency, while chills preceding fever relates to qi deficiency. Chills only without fever suggests severe internal injury; fever only without chills suggests numerous internal pathological changes. Severe chills and fever indicate deep-seated pathogens; mild chills and fever indicate superficial pathogens. There are also cases of prolonged chills or fever lasting months or years without cure, due to violating dietary/lifestyle taboos combined with deficient primordial qi.
Synthetically, Malaria is not solely caused by external pathogenic factors. It often arises from immoderate alcohol consumption daily, injuries from the seven emotions and six desires, combined with phlegm encapsulated by the Spleen failing to disperse, interacting with internal food stagnation and external summerheat, accumulating in the Shaoyang (Lesser Yang) region. The Shaoyang position is the必经 pathway for the interchange of yin and yang in the human body, located between the exterior and interior (half-exterior, half-interior). When yin blood flows through this area and is stimulated, fever ensues; or when yin and yang intersect, alternating chills and fever occur. If uncured for a long time, a mass called "Nuomu" (Malaria Mother) forms, hidden under the hypochondrium. The hypochondrium is the domain of the Shaoyang meridian. To treat this disease, use guiding herbs to direct medicinal action to the Shaoyang region, then administer formulas for resolving food stagnation, transforming phlegm, dispersing wind, and regulating qi, dosed according to severity. No case will fail to respond.
However, why does Malaria present as occurring every day, every other day, or every three days? This is because the duration since infection varies. Cold pathogen acquired in winter without immediate illness, manifesting the following autumn, results in every-three-day Malaria. Every-three-day occurrence implies infection may have lasted a year; every-other-day implies half a year; daily occurrence implies three months. The interval of发作 corresponds to the duration since infection. Physicians deduce internally: seeing every-three-day发作, know it stems from cold, treat with acrid-warm herbs to disperse; seeing every-other-day发作, know it stems from summerheat, treat with heat-clearing herbs. However, symptoms must match before administering medication. For instance, in Western Zhejiang, initial malaria cases are all called "Tai Nue" (Fetal Malaria). Presumably, no one dares to take medicine. If initial malaria is not treated by releasing the exterior, how can pathogens dissipate naturally? Instead, they become more intense. Delaying treatment leads to depletion of qi and blood, hyperactivity of Liver pathogen, suppression of the Spleen meridian, resulting in emaciated limbs, poor appetite, sallow complexion, and edematous skin. Only then is medication considered. Those with robust constitutions may still be saved; those with weak constitutions are beyond saving. Since there is "Tai Nue," there should logically be "Tai Shanghan" (Fetal Cold Damage) and "Tai Liji" (Fetal Dysentery). Why only Malaria has "Tai" while others do not? This statement is highly absurd. Such erroneous views prevent later generations from being enlightened, thereby avoiding misdiagnosis costing lives. Regardless of whether it is initial or recurrent发作, if there is food stagnation, resolve it; if phlegm, transform it; if wind, disperse it; if cold, attack it; if heat, drive it away; if qi stagnation, open it. For new diseases, eliminate the pathogen; for chronic diseases, tonify the yuan qi. The divine application of these principles depends on the practitioner.
