Plaque-like masses located in the middle jiao (middle burner) are caused by phlegm-fluids; those on the right are caused by food retention (another view holds it is phlegm). Accumulations on the left are blood masses. Qi itself cannot form a tangible mass and aggregate, because a "mass" is a tangible entity formed by the condensation of phlegm, food retention, and dead blood (blood stasis). Treatment involves using pills made from sea stone boiled in vinegar, Sanleng (Rhizoma Sparganii), Pengzhu (Ezhu/Rhizoma Curcumae), Tao Ren (Semen Persicae), Hong Hua (Flos Carthami), Wulingzhi (Faeces Trogopterori), Xiangfu (Rhizoma Cyperi), etc., swallowed with a decoction of Shijian (Lithium Carbonate) and Baizhu (Rhizoma Atractylodis Macrocephalae). Walengzi (Concha Arcae) can dissolve blood masses and secondarily resolve phlegm. Shijian (Lithium Carbonate) is useful for cases with phlegm retention and masses; it cleanses greasy filth and also digests food retention. To treat masses, one must descend fire and eliminate food retention, as food retention is the source of phlegm. To move blood masses formed by dead blood, after the mass is eliminated, one must strongly tonify the body. For all accumulation diseases, purgative drugs should not be used, as they merely damage true qi without removing the disease. Instead, drugs that dissipate accumulations should be used to melt them, thereby eradicating the root. In women, masses are mostly blood masses.
Dai Yuanli stated: Regarding Ji Ju (Accumulation and Aggregation) and Jia (Mobility): If an accumulation forms a mass that cannot move, it is called "Ji"; if it appears and disappears, moves up or down, left or right, it is called "Jia".
Mr. Zhu Danxi treated a case of Ji Ju Jia in the Chen family of Tanpu, Taizhou. He used a decoction of Shuhui Gen (Root of Malva verticillata), strained it, then added Renshen (Radix Ginseng), Baizhu (Rhizoma Atractylodis Macrocephalae), Qingpi (Pericarpium Citri Reticulatae Viride), Chenpi (Pericarpium Citri Reticulatae), Gancao Shao (Apex Radix Glycyrrhizae), and Niuxi (Radix Achyranthis Bidentatae) to brew another decoction. He added finely ground Tao Ren (Semen Persicae) and Xuanningfen (Natrii Sulfas Exsiccatus) in small amounts, and administered it hot. After two doses, the mass should be expelled. If the disease is severe, tonification and support must follow the attack, followed by further adjustments.
Formula Entry:
Xiao Kuai Wan (Mass Dissolving Pill): This is the Xiaoshi Dahuang Wan (Niter and Rhubarb Pill) from *Qianjin Fang*. It only grinds down masses without causing exhaustion, but dosage must be adjusted according to deficiency and excess.
Ingredients: Xiaoshi (Niter, 6 liang), Renshen (Radix Ginseng, 3 liang), Gancao (Radix Glycyrrhizae, 3 liang), Dahuang (Radix et Rhizoma Rhei, 8 liang).
Grind into powder. Place three sheng (approx. 0.9 liters) of three-year-old vinegar (some say three dou) in a porcelain vessel. Use a bamboo stick as a marker; for every one sheng added, mark one "ke" (unit of time/depth). First add Dahuang, stirring constantly until slightly boiling. After one "ke", add the remaining medicines. Simmer over low heat for another "ke" until pillable. Form pills the size of a chicken egg yolk. Take one pill at a time with rice drink. If large pills are difficult, make small pills the size of a wutong seed. Take 30 pills per dose; stool should pass out resembling chicken liver, rice water, or dark red/black colors. After defecation, avoid wind and cold, and nourish with soft porridge.
San Sheng Gao (Three Saints Ointment):
Take half a jin of undigested Dunshi (Calcined Alumen/Dolomite residue), grind into powder, and stir-fry in a clay pot until pale red. Remove from fire, let cool slightly. Then add one liang of Dahuang powder, stir-fry off-heat until cool. Add half a liang of Guixin (Cortex Cinnamomi) powder, stir-fry briefly, then add rice vinegar and boil/stir into a black ointment. Spread on thick paper and apply to the affected area.
For plaques located between the skin and fascia, Qi-tonifying herbs combined with Xiangfu (Rhizoma Cyperi) to open stagnation are required, along with Erchen Tang (Two-Aged Decoction) plus Qi-tonics. First, strictly avoid rich and greasy foods.
