After sweating therapy, if the pulse is floating and rapid, accompanied by vexation and thirst, Wuling San (Five-Ingredient Powder with Poria) should be administered (Formula located in Taiyang Chapter 41).
After sweating therapy, heat dissipates with the sweat, resulting in a floating and rapid pulse, and symptoms of vexation and thirst. This indicates that sweating has led to Yang deficiency, dampness stagnation in the Spleen Earth, and the rising of ministerial fire. Generally, the confinement of fire and the hibernation of Yang rely entirely on the Yang of the Spleen Earth. If Spleen Yang is lost and dampness moves recklessly, the Qi of the Lung and Stomach fails to descend, and Sovereign Fire flares upward, thus presenting such pulse and symptom patterns. It is appropriate to use Wuling San to dry dampness and leach out water-dampness. However, if these floating, rapid, vexatious, and thirsty pulse and symptoms appear before any sweating therapy has been induced, Da Qing Long Tang (Major Green Dragon Decoction) should be used, rather than Wuling San.
In Shaoyin cold damage disease, with a floating pulse, spontaneous sweating, frequent urination, mental vexation, slight aversion to cold, and cramping/tightness of the legs, administering Guizhi Tang (Cinnamon Twig Decoction) to attack the exterior is an error. Following this error, the patient immediately develops cold limbs (Jue), dry throat, restlessness, vomiting, and rebellious Qi. One should prepare Gancao Ganjiang Tang (Licorice and Dried Ginger Decoction) for them to restore their Yang. If the cold limbs resolve and the feet become warm, one should then prepare Shaoyao Gancao Tang (Peony and Licorice Decoction) for them, at which point the legs will extend naturally. If there is disharmony of Stomach Qi leading to delirious speech, administer a small amount of Tiaowei Chengqi Tang (Regulate the Stomach and Direct to Enter Decoction). If excessive sweating is induced again, or if burning needle therapy is added, Sini Tang (Frigid Extremities Decoction) should be administered (Formula located in Taiyin Chapter 3).
A floating pulse and spontaneous sweating indicate that interior heat is leaking outward. Frequent urination implies that the stool must be hard. Mental vexation is caused by the steaming of Stomach heat affecting the Chong Mai. Although Yangming disease presents for only one day, the aversion to cold will cease spontaneously, followed by spontaneous sweating and aversion to heat. Slight aversion to cold suggests that the exterior pathogen is not yet fully resolved; although sweating occurs, it does not immediately manifest as profuse sweating. This is also an indication for Tiaowei Chengqi Tang (As stated in the Yangming Chapter: In Taiyang disease, after emesis, purgation, or sweating, if mild vexation appears, frequent urination occurs, and the stool becomes hard due to this, Xiao Chengqi Tang can harmonize the condition and cure it; in Yangming disease, if vexation occurs without prior emesis or purgation, Tiaowei Chengqi Tang may be given, which corresponds to this case). The physician, observing the floating pulse and spontaneous sweating, misidentified the presentation as Taiyang Wind Strike syndrome, and erroneously administered Guizhi Tang plus Fuzi (Prepared Aconite Root), increasing the dosage of Guizhi to attack the exterior. This is a major error. Consequently, excessive sweating leads to Yang collapse, causing cold limbs and dry throat. Yang Qi detaches from its root, generating vexation, while Stomach Qi rebels, causing vomiting. At this stage, Gancao Ganjiang Tang should be prepared and administered. Gancao (Licorice Root) nourishes Earth and tonifies the Middle Jiao, while Ganjiang (Dried Ginger) warms the Stomach and directs rebellious Qi downward. As Yang returns and the limbs warm, the cold limbs resolve and the feet become warm. The cramping of the legs is due to dryness of Liver Wood causing tendon contraction. One should then prepare Shaoyao Gancao Tang for them. Gancao relaxes the tendons and relieves urgency, while Shaoyao (White Peony Root) clears wind-heat and moistens dryness, allowing the legs to extend naturally. If Stomach Qi is disharmonious and Earth dryness causes delirium, a small amount of Tiaowei Chengqi Tang will harmonize the Stomach Qi. Using Guizhi to induce sweating constitutes the first deviation/error. If one fails to use Ginger and Licorice to restore Yang but instead induces further sweating, or adds burning needles to severely deplete Yang, this constitutes a second deviation/error. In such cases, Sini Tang should be used urgently to restore Yang. The combination of Ginger, Licorice, and Fuzi provides dual warming of Water and Earth.
