Tetanus (Jinmai Ji) refers to the tension and spasm of tendons and vessels. Chizong (Chizong, tendon relaxation) refers to the flaccidity of tendons and vessels. Tension causes pulling and contraction, while flaccidity causes extension. Alternating contraction and extension with irregular movements constitute tetany, commonly known as "fa chu" (convulsions). Generally, epilepsy, wind-spasm, and lockjaw (tetanus from wound infection) can all cause convulsions. If there are ulcerated wounds with rotting tissue and bleeding, patients with ulcerative injuries are prone to this condition. For cases due to Qi and Blood deficiency with internal stirring of Liver Fire generating Wind, use Bazhen Tang (Eight Treasure Decoction), Huangqi (Astragali Radix), Gouteng (Uncariae Ramulus cum Uncis), assisted by Dihuang Wan (Rehmannia Pill). If ineffective, focus solely on tonifying Stomach Qi. For cases due to dryness of Blood in the Liver channel generating Wind, use Lingyangjiao San (Antelope Horn Powder) added with Gouteng (Uncariae Ramulus cum Uncis) and Shanzhi (Gardeniae Fructus). For cases involving both Liver Fire and Blood dryness, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) added with Gouteng (Uncariae Ramulus cum Uncis); if ineffective, must concurrently take Liuwei Wan (Six-Ingredient Rehmannia Pill) to tonify Kidney Water to nourish Liver Wood. If caused by a wet nurse's depression and anger leading to Liver Fire affecting the infant, the mother must be treated, referring to the theory of the Five Elements overcoming each other for treatment.
A young woman suffered from scrofula (Luoli, tuberculous lymphadenitis of the neck). After taking sedating medicines, her complexion became yellowish-red, she yawned and ground her teeth. I diagnosed it as deficiency of Qi and weakness of Blood in the Liver channel, with Fire stirring and generating Wind. She was cured using Wuyi Yigong San (Five Flavor Yi Gong Powder) added with Chaihu (Bupleuri Radix) and Shengma (Cimicifugae Rhizoma). Later, due to anger, the symptoms recurred: red face, staring eyes, loud crying, stiff neck, and a surging, rapid pulse at the Guan position. She was first treated with Yigan San (Suppress Liver Powder), then with Dihuang Wan (Rehmannia Pill), resulting in recovery.
A fourteen-year-old boy suffered from this condition, accompanied by yawning, teeth grinding, and hands grasping at clothes. The medicines he had taken were all wind-dispelling agents. I stated that the Blood of the Liver channel had been injured again, and Dihuang Wan (Rehmannia Pill) should be used to nourish Kidney Water to generate Liver Wood. He did not believe this and insisted on wind-dispelling and phlegm-resolving treatments. Consequently, deficiency syndromes arose rapidly: confusion resembling drunkenness, due to excessive deficiency of Stomach Qi leaving the Zang organs without nourishment. I prescribed Sizijunzi Tang (Four Gentlemen Decoction) with one liang of Renshen (Ginseng Radix et Rhizoma), administered three doses in one day. Although he regained consciousness, he lacked strength to move. By the thirteenth dose, I assisted with Dihuang Wan (Rehmannia Pill) ingredients, adding five qian of Huangqi (Astragali Radix) per dose. After more than twenty doses, he recovered. The following year, after marriage, the condition relapsed within a month; the same prescription cured him.
A child developed convulsions after an ulcer healed. After taking Niuhuang Wan (Calculus Bovis Pill), his limbs became weak, neck stiff, eyes staring, lips pale, saliva flowing, and extremities cold. He sought my treatment. I said: The Classics state, "The flourishing aspect of the Spleen manifests in the lips and mouth." Also, "The Spleen governs the four limbs," and "The Spleen governs saliva." This occurred because previous medications wrongly purged, injuring Stomach Qi further, causing Liver Wood to insult Spleen Earth, leading to the aforementioned symptoms. We must first rescue Stomach Qi to nourish the Zang organs. Due to conflicting opinions among others, medication was delayed. The next morning, he indeed exhibited teeth grinding, yawning, lethargy, palpitations, gasping, shortness of breath, and a pale white complexion. They finally believed me. First, Wuyi Yigong San (Five Flavor Yi Gong Powder) was used, followed by Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), resulting in recovery.
A young woman was nearly cured of scrofula (Luoli). Due to diligent needlework, she suddenly developed convulsions. This was due to incomplete Stomach Qi and injury to tendons and vessels from overexertion. She was cured using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) and Wuyi Yigong San (Five Flavor Yi Gong Powder), both added with Gouteng (Uncariae Ramulus cum Uncis). Later, due to labor and anger, tremors occurred during menstruation. She was cured using Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), both added with Gouteng (Uncariae Ramulus cum Uncis).
A child suffered a fall injury. After ulceration, he developed the aforementioned symptoms: complexion either blue or red. After taking wind-phlegm resolving medicines, he exhibited teeth grinding and staring eyes, yet the physician still attempted to treat wind. I said: All trauma conditions are governed by the Liver. Therefore, a blue complexion with convulsions, teeth grinding, and staring eyes belong to deficiency and loss of Qi and Blood in the Liver channel, where Wind Wood contracts and closes, Fire stirs and generates Wind. There is no wind to dispel and no phlegm to purge. He was cured using Dihuang Wan (Rehmannia Pill) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
A child fell and bled. Mistakenly taking Dahuang (Rhei Radix et Rhizoma) and other purgatives, he developed the aforementioned symptoms: occasional irritability, spontaneous sweating, and hands grasping at empty air. This was due to Blood deficiency in the Liver channel and blazing Liver Fire. He was cured using Dihuang Wan (Rehmannia Pill) and Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
