Jieguo stated: Wind syndromes are characterized by their migratory nature and rapid changes; they invade the Zang organs very quickly, and life or death can be decided in an instant. Therefore, it is urgent to differentiate between exterior and interior, deficiency and excess, and treat accordingly. If the pathogenic factor is at the exterior, Qianghuo Fangfeng Tang (Notopterygium and Saposhnikovia Decoction) is suitable. If it is half-exterior and half-interior, manifested by sweating on the head but no sweat on the body, Qianghuo Tang (Notopterygium Decoction) is suitable. If the pathogen has transmitted inward, severe cases may present with tongue stiffness, trismus (lockjaw), opisthotonos (backward arching of the neck and back), muscle twitching and convulsions, profuse phlegm-saliva, chest and abdominal fullness and stuffiness, or dark and obstructed urination and defecation, occasional sweating, and a pulse that is surging, rapid, and wiry; in such cases, Da Xionghuang Tang (Large Ligusticum and Rhubarb Decoction) is suitable. However, for those who just began to sweat, this is due to severe depression of wind-heat within the interior; thus, the exterior heat slightly resolves, the couli (pores) become loose, and sweating occurs. The appropriate method is to clear heat and dissipate nodulation. If the heat has subsided, the Zang-fu organs have harmonized, yet sweating persists, this indicates exterior deficiency; Baizhu Fangfeng Tang (Atractylodes Macrocephala and Saposhnikovia Decoction) should be used to consolidate the exterior. For trismus where the jaw cannot be opened, the mouth must be pried open to administer medicine, and gruel should be administered frequently. However, when children suffer from this condition, it is often caused by concurrent fright, internal liver fire, and heat generating wind. The Liver governs the five colors and belongs to Wood, which generates wind. Observing the complexion reveals: green corresponds to the Liver, red to the Heart, yellow to the Spleen, white to the Lungs, and black to the Kidneys. For cases involving the Liver channel, Chaihu Qinggan San (Bupleurum Liver-Clearing Powder) is used. For the Heart channel, Zhizi Qinggan San (Gardenia Liver-Clearing Powder) added with Huanglian (Coptis Rhizome) is used. For the Kidney channel, Dihuang Wan (Rehmannia Pill) added with Chaihu (Bupleurum Root) is used. For the Spleen channel, Liu Jun Zi Tang (Six Gentlemen Decoction) added with Shanzhi (Gardenia Fruit) and Chaihu (Bupleurum Root) is used as the main formula, supplemented with herbs that greatly tonify the Spleen and Stomach.
A fourteen-year-old child suffered from scrofula (tuberculous lymphadenitis). Due to mental exertion and heavy academic workload, he experienced headache and fever. Believing it to be a common cold, he used ginger and scallion to induce sweating. Suddenly, his waist and back arched backward, his jaw locked, and he could not speak. His pulse was floating and large, but felt empty upon deep pressure. This was a spasm syndrome resulting from extreme deficiency of Qi and Blood, not tetanus. One dose of Shiquan Dabu Tang (Ten Great Tonifying Decoction) was administered via gavage, after which he regained consciousness after a long time. After several more doses, he recovered. Later, he contracted another cold, leading to cold limbs and profuse sweating, remaining unconscious for a long time. The previous decoction was used with 5 fen of Fuzi (Prepared Aconite Root); one dose restored consciousness. Fuzi was then removed, and he continued taking the prescription for over thirty doses until fully recovered.
Another child suffered from liuzhu (a type of yin abscess with diffuse swelling and no head), presenting with a sallow complexion. Suddenly, he developed tongue stiffness and trismus. His pulse was surging and large but deficient, feeling empty upon deep pressure. This was a syndrome transformation due to deficiency of Lung and Spleen Qi. Four doses of Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) were given first, which slightly alleviated the symptoms. Subsequently, several doses of Shiquan Dabu Tang (Ten Great Tonifying Decoction) were administered, leading to recovery.
A child had a carbuncle on the buttock that failed to heal for a long time. Due to fright, he developed convulsions and trismus. Tuoli San (Support the Interior Powder) was used, containing Renshen (Ginseng Root) and Baizhu (Atractylodes Macrocephala Rhizome) at 3 qian each, Chaihu (Bupleurum Root) at 5 fen, and Goutenggou (Uncaria Rhynchophylla Hook) at 1.5 qian. After four doses, he stabilized. Later, due to food stagnation and fright, he exhibited staring eyes, convulsions, trismus, drooling, and cold fingers. Wuyi Yigong San (Five Agents Efficacious Work Powder) was used because Liver Wood was hyperactive and insulting Spleen Earth, causing dietary retention. Wuyi Yigong San was modified with Goutenggou (Uncaria Rhynchophylla Hook), Muxiang (Aucklandiae Radix), and Ganjiang (Dried Ginger), which restored consciousness.
A child with an ulcerative sore developed spasms similar to the previous case, with a bluish-red complexion. This indicated blood deficiency with heat in the Heart and Liver channels. Bazhen Tang (Eight Treasures Decoction) added with Chaihu (Bupleurum Root) and Mudanpi (Moutan Cortex) was used first. Then, Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) added with Wuweizi (Schisandra Fruit) was used, leading to gradual recovery. Finally, Bazhen Tang (Eight Treasures Decoction) was used to stabilize the condition.
A child with an ulcer suddenly presented with incessant sweating and cold hands and feet. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Rougui (Cinnamon Bark) and Wuweizi (Schisandra Fruit) was administered for several doses, gradually resolving all symptoms. Later, due to overeating, he developed trismus and vomiting. Yigong San (Efficacious Work Powder) added with Shengma (Cimicifugae Rhizoma) was given for four doses, leading to stabilization.
A sixteen-year-old child had a chronic non-healing sore. Due to overexertion, he developed trismus and staring eyes. His pulse was surging and rapid, and the left Guan position was wiry and weak. It was judged that there was deficiency of Qi and Blood in the Liver channel with internal stirring of deficient fire. Dihuang Wan (Rehmannia Pill) material was prescribed for four doses, stabilizing the condition. Subsequently, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used to tonify the Spleen and Lungs; Dihuang Wan (Rehmannia Pill) was used primarily to tonify the Kidneys and Liver; and Jiuwei Lu Hui Wan (Nine-Ingredient Aloe Pill) was added to treat Liver malnutrition (gan gan), leading to the healing of the sore.
Another sixteen-year-old child had a chronic non-healing liuzhu (abscess). Due to labor combined with anger, he suddenly collapsed into unconsciousness, resembling tetanus. His complexion was pale, and he lacked energy to move. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was used, containing 5 qian of Renshen (Ginseng Root) and 1 qian of Rougui (Cinnamon Bark). When there was no response, 1 qian of Ganjiang (Dried Ginger) was added, still without effect. This indicated extreme Yang Qi deficiency where the medicinal strength was insufficient. Urgently, 1 qian of Fuzi (Prepared Aconite Root) was added, which slightly stabilized the condition. Fuzi was then removed, and he took the prescription for over ten doses, gradually recovering his Yuan Qi. Subsequently, Bazhen Tang (Eight Treasures Decoction) and Douchi Bing (Fermented Soybean Cake) were added as adjuvants. After half a year, he recovered. After marriage, due to stress from imperial examinations, his Yuan Qi was damaged, and the previous symptoms recurred, resembling tetanus. His pulse was floating and large, empty upon deep pressure. Shenfu Tang (Ginseng and Aconite Decoction) was administered for four doses, restoring consciousness. Over a hundred doses each of Bazhen Tang (Eight Treasures Decoction) and Dihuang Wan (Rehmannia Pill) material were taken until full recovery.
