Baihu Tang (White Tiger Decoction): Treats Shanghan (Cold Damage), or after vomiting or purging, when pathogenic toxins remain unresolved for seven to eight days. Heat is knotted internally, causing heat in both the exterior and interior, occasional aversion to wind, severe thirst, dryness and irritability on the tongue, and a desire to drink several liters of water. This formula is suitable. It also treats summerheat-stroke in summer, characterized by sweating with aversion to wind and cold, body heat, and thirst.

Qian Zhongyang said: Tidal fever refers to fever occurring at specific times, subsiding after the time passes, and recurring at the same time the next day. There are distinctions among wind-cold, malnutrition accumulation, and food癖 (masses), as well as differences in yin-yang, deficiency-excess, and the five zang organs. For example, fever after sweating, yawning, and reddened face indicates wind-heat. Shanghan epidemics, where yin and yang contend, indicate external contraction heat. Emaciation, dry mouth, bone-steaming night sweats indicate malnutrition heat. Constipation and difficult urination, fever unrelieved by sweating or purging, indicates accumulation heat. Heat first appearing in the abdomen and back, starting at night and stopping in the morning, indicates food heat. Salivation, cough, desire to drink but indigestion of milk and food indicates mass heat. There is also vexatious heat, characterized by coarse breathing, panting, mental restlessness, red cheeks, mouth sores, and concurrent epilepsy symptoms. Rash heat presents with cold ears and nose tips. Blood heat manifests as fever during Si-Wu hours (9 AM - 3 PM) and cooling at night. Deficiency heat presents as fatigue and weakness, often occurring after illness. Yang pathogens affecting the heart cause irregular fever. Contention between yin and yang causes chills and fever alternating like malaria. The aforementioned conditions are common in children. Some are caused by the mother's excessive emotions or rich diet during pregnancy leaving residual heat, or by food stagnation, excessive clothing/bedding warmth, and frequent bathing in hot water. If fever occurs during Yin-Mao-Chen hours (3 AM - 9 AM) with strong vitality and desire to drink, it is excess heat in the Liver Meridian, treated with Chaihu Qinggan San. If fever occurs with weak vitality and desire for warm drinks, it is deficiency heat in the Liver Meridian, treated with Liuwei Dihuang Wan. Fever during Si-Wu hours belongs to the Heart Meridian; excess is treated with Daochi San, deficiency with Mizhi Anshen Wan. Fever during Shen-You-Xu hours (3 PM - 9 PM) belongs to the Lung Meridian; excess is treated with Xiebai San, deficiency with Mizhi Baopi Tang. Fever during Hai-Zi-Chou hours (9 PM - 3 AM) belongs to the Kidney Meridian, treated with Dihuang Wan. Generally, high fever with thirst and constipation indicates excess heat, treated with Erhuang Xijiao San to purge. Fever with thirst for warm drinks and normal stool indicates blood deficiency, treated with Siwu Tang to tonify. After purging, if yin deficiency leaves yang without attachment and fever persists, use Siwu Tang plus Renshen [text missing, likely Huangqi]. After sweating, if yin and yang deficiency prevents qi and blood generation and fever persists, use Sijunzi Tang plus Chuanxiong and Danggui. If after sweating and purging there is vexation, thirst, and red face due to blood deficiency causing agitation, use Danggui Buxue Tang. If convulsions occur due to liver blood deficiency generating internal wind, use Siwu Tang plus Tianma and Gouteng. Red cheeks, dry mouth, dark scanty urine, and dry yellow stool indicate exterior-interior excess heat, treated with Qingliang Yinzi. If stool has passed but fever persists, indicating unresolved exterior pathogen, if Xingxing San is ineffective, add Mahuang to induce mild sweating. If fever persists after sweating, this indicates exterior-interior deficiency, qi failing to return to its source, and yang floating externally causing deficiency heat, treated with Liushen San plus Jingmi. If fever is caused by yang qi sinking into yin, use Buzhong Yiqi Tang. For nursing infants, treat the mother simultaneously.
Clinical Cases:
One child had tidal fever, vexation, thirst, dry hard stool, shortness of breath, cough, and red right cheek. This indicated heat in the Lung and Large Intestine. Used Chaihu Yinzi, cured after one dose. Later, due to slight fright, developed convulsions, teeth grinding, and sudden stupor. This indicated liver-spleen qi and blood deficiency. Cured using Sijunzi Tang, Chuanxiong, Danggui, and Gouteng.
One child had tidal fever, agitation, and cyanotic-red left cheek. This indicated heart-liver blood deficiency. Gradually improved using Mizhi Anshen Wan and Siwu Tang, Fangfeng, Suanzaoren. Later cured using Liuwei Dihuang Wan to regulate and tonify liver and kidney.
One child had tidal fever, convulsions, surging phlegm-saliva, and cold fingers/toes. From Shen-You hours, the left cheek showed white hidden within cyanosis, with intermittent hand-foot convulsions. This indicated liver meridian weakness overcome by lung metal causing tidal convulsions, and spleen earth weakness causing cold extremities. Cured by using Buzhong Yiqi Tang to regulate spleen and lung, and Liuwei Dihuang Wan to tonify liver and kidney. Generally, when pathological qi is in excess, it should be recognized as yuan qi deficiency. Using methods to drain lung metal, attack liver wood, clear heat, and resolve phlegm would be an error.
