Mr. Wenren stated: When a patient with smallpox (variolae) suffers from constipation, bloody urine, or the formation of carbuncles and toxic sores, along with the rupture of skin pustules, it is due to excessive internal fire that has failed to be resolved or purged in time. Urgent treatment should involve using Xijiao Dihuang Tang (Rhinoceros Horn and Rehmannia Decoction), Xiaochaihu Tang (Minor Bupleurum Decoction) added with Shengdihuang (Fresh Rehmannia Root), and similar formulas such as Sishun Yin. I opine that if the aforementioned symptoms are caused by excessive heat in the Heart and Spleen, Xijiao Dihuang Tang should be used; if caused by excessive heat in the Heart and Liver, Xiaochaihu Tang added with Shengdihuang should be used. If there is constipation, first use Sishun Yin, followed by Xijiao Tang. If the lesions are red and painful with obstruction of both bowel and bladder, urgently use Huoming Yin (Decoction for Saving Lives) added with Mangxiao (Mirabilite) and Dahuang (Rhubarb). If the lesions are red and painful accompanied by aversion to cold and fever, use Huoming Yin added with Mahuang (Ephedra). If the condition arises from the nursing mother's anger-induced fire, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) and Jiawei Guipi Tang (Modified Restore the Spleen Decoction).
A child presented with red-based pustules, alternating chills and fever with pain, indicating heat in the Liver meridian. Initially, two doses of Jiawei Xiaochaihu Tang were administered, after which various symptoms gradually subsided. Subsequently, three doses of Jiawei Xiaoyao San were given, leading to suppuration. Finally, the child recovered after taking Bazhen Tang (Eight Treasures Decoction).
A man presented with red, painful pustule bases, fever, and thirst. After taking Zicao Yin (Arnebia Decoction), his pustules ruptured and bled. I attributed this to the struggle between wind and fire in the Heart and Liver meridians. He was first treated with two doses of Xiaochaihu Tang added with Shengdihuang and Xijiao (Rhinoceros Horn), and subsequently cured with Shengji Xijiao Dihuang Tang (Shengji Rhinoceros Horn and Rehmannia Decoction).
A child suffered from the aforementioned symptoms, presenting with difficult defecation, red and astringent urination, and thirst for cold drinks. My father diagnosed this as excess-heat in the Intestines and Stomach. He first prescribed Liangge San (Cooling the Chest Powder), after which the condition improved gradually with one dose. The child then recovered after taking Xijiao Dihuang Tang and Celery Juice.
Another child suffered from this condition. Upon examination, it was found that the nursing mother had stagnant fire. She was treated with Jiawei Xiaoyao San and Jiawei Guipi Tang, resulting in recovery. Those who contracted this type of smallpox simultaneously but used miscellaneous medications all died without hope of rescue.
Liangge San treats excess-heat in the Upper Jiao, characterized by irritability, thirst, flushed face, dry throat and sore throat, red and astringent urine and stool, delirious speech, and restless sleep.
Dahuang (Rhei Radix et Rhizoma), Puxiao (Natrii Sulfas), Gancao (Glycyrrhizae Radix et Rhizoma), Lianqiao (Forsythiae Fructus), Shanzhi (Gardeniae Fructus), Huangqin (Scutellariae Radix), Boheye (Menthae Haplocalycis Folium) (equal parts).
Grind the above into fine powder. Take a small amount each time, mixed with decoction of Bamboo Leaves and Honey. The nursing mother should take it concurrently.
