若痘未出而见抽搐,是热毒内蕴所致,用紫草快斑汤加钩藤钩;
若痘已出且红绽而见抽搐,是热毒作痛所致,用东垣消毒散加钩藤钩;
若痘灌浆期而见抽搐,是气血虚弱所致,用人参四圣散加钩藤钩;
若出现眼睛斜视或直视,是风火相搏所致,用柴胡栀子散,或六味地黄丸加柴胡、山栀;
若口角流涎,是肝木乘脾土所致,用五味异功散加升麻、柴胡、钩藤钩;
若目赤多眵流泪,是肝血虚而生风所致,用四物汤加柴胡、钩藤钩;

Mr. Danxi stated: "Dengfeng (convulsions occurring during the course of smallpox) should be differentiated by the deficiency or excess of Qi and Blood. In cases of deficiency, blood-nourishing formulas are primarily used, supplemented by wind-dispersing herbs. In cases of excess, White Peony Root (Baishao) and Scutellaria Root (Huangqin) serve as the sovereign drugs, while Forsythia Fruit (Lianqiao), Angelica Dahurica (Baizhi), and the root of Cistanches (Xuduan) serve as ministering drugs.
I believe that for the aforementioned symptoms, sweating methods to release the exterior should also be considered. Regardless of whether the pustules have erupted or not, or whether they have crusted over or not, if convulsions are triggered by external pathogenic factors damaging the exterior and internal fire stirring within:
If convulsions occur before eruption, it is due to heat-toxin accumulation internally; use Zicao Kaban Tang (Purple Gromwell Decoction for Rapid Eruption) with Uncaria rhynchophylla (Gouteng).
If convulsions occur after eruption when the pustules are red and blooming, it is due to pain caused by heat-toxin; use Dongyuan Xiaodu San (Dongyuan's Detoxification Powder) with Uncaria rhynchophylla (Gouteng).
If convulsions occur during the suppuration stage, it is due to deficiency of Qi and Blood; use Renshen Sisheng San (Ginseng Four-Sage Powder) with Uncaria rhynchophylla (Gouteng).
If convulsions occur after crusting, it is due to further deficiency of Qi and Blood; use Bazhen Tang (Eight-Treasure Decoction) with Uncaria rhynchophylla (Gouteng).
If there is strabismus or fixed gaze, it is due to the struggle between wind and fire; use Chaihu Zhizi San (Bupleurum and Gardenia Powder) or Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) with Bupleurum Root (Chaihu) and Gardenia Fruit (Shanzhi).
If there is drooling from the corners of the mouth, it is due to Liver Wood overacting on Spleen Earth; use Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) with Cimicifuga Rhizome (Shengma), Bupleurum Root (Chaihu), and Uncaria rhynchophylla (Gouteng).
If there is redness of the eyes with excessive discharge and tearing, it is due to wind generated by Liver Blood deficiency; use Siwu Tang (Four-Substance Decoction) with Bupleurum Root (Chaihu) and Uncaria rhynchophylla (Gouteng).
If opisthotonos occurs, it is due to Water failing to generate Wood (Kidney Water deficiency failing to nourish Liver Wood); use Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) with Bupleurum Root (Chaihu) and Angelica Sinensis (Danggui), followed by appropriate use of Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Gastrodia Rhizome (Tianma) and Uncaria rhynchophylla (Gouteng). Do not directly use wind-dispersing drugs. This is because wind-dispersing herbs can dry the Blood and scatter Qi. One must verify the condition based on three signs: the temperature and warmth of the hands and feet, and then apply tonifying or purging regulation methods, so as to avoid errors. For infants, one should examine the nursing mother and treat accordingly.
A child suffered convulsions as his smallpox was about to erupt. After taking Niuhuang Qingxin Wan (Calculus Bovis Heart-Clearing Pill), he developed trismus and drooling. This indicated that the Spleen and Stomach were injured again, and Liver Wood bullied the Spleen, causing saliva to fail to return to its vessels. First, Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) with Uncaria rhynchophylla (Gouteng) was used, and all symptoms instantly decreased. Subsequently, Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder) with Bupleurum Root (Chaihu) brought recovery.
A woman developed convulsions due to anger while suffering from smallpox. Her scar marks were red, she was irritable and thirsty, and her face and eyes were red. This was caused by excessive sweating leading to Yang collapse and Blood loss. First, Danggui Buxue Tang (Angelica Tonifying Blood Decoction) was administered for two doses, and the irritability and thirst stopped instantly. Then, Bazhen Tang (Eight-Treasure Decoction), Bupleurum Root (Chaihu), Moutan Cortex (Mudanpi), and Uncaria rhynchophylla (Gouteng) were used, curing the fever and convulsions completely. Finally, she recovered using Bazhen Tang (Eight-Treasure Decoction).
A woman suffered convulsions, trismus, and splitting headaches due to anger as her smallpox was nearing cure. Her scar marks were pale white. She was cured instantly using Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Vitex Fruit (Manjingzi) and Uncaria rhynchophylla (Gouteng). Later, due to anger, she suffered convulsions again with dizziness and loss of appetite. She was first treated with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Gastrodia Rhizome (Tianma), and subsequently stabilized with Liujunzi Tang (Six-Gentleman Decoction) and Bupleurum Root (Chaihu).
Danggui Buxue Tang (Angelica Tonifying Blood Decoction): Treats smallpox where Qi and Blood are deficient, causing convulsions, fever, thirst with desire to drink, and a pulse that is large and forceful yet empty upon heavy pressure. (Formula see entry 'Chills and Teeth Grinding')
Baolong Wan (Embracing Dragon Pill): Treats convulsions caused by wind-heat in smallpox, or severe phlegm.
Ingredients: Danxing (Arisaema Cum Bile, 4 liang), Tianzhuhuang (Calamus Tuber, 1 liang), Xionghuang (Realgar), Zhusha (Cinnabar, each 5 qian), Shexiang (Musk, a small amount).
Preparation: Grind into fine powder. Use Licorice Root (Gancao, 1 jin) decocted into paste to form pills. Each liang of powder makes 20 pills. Administer dissolved in Mint Tea or Juncus Effusus (Dengxin) tea.
Note: The previous formulas contain herbs for clearing heat, resolving phlegm, regulating Qi, and dispelling wind targeting the Liver channel. Overuse will re-injure the Spleen and Lung, worsening the condition. If chest/abdominal distension, poor appetite, or vomiting occur, use Renshen Baizhu San (Ginseng and Atractylodes Macrocephala Powder) to strongly tonify Middle Qi.
Method for Preparing Arisaema Cum Bile (Niu Dan Nanxing): In the twelfth lunar month, select large Arisaema Rhizomes (Nanxing), grind them into powder, mix evenly with bile from a yellow cow, place back into the gallbladder shell, tie the opening with thread, and hang in a ventilated place to dry in the shade. It can be used only after one year. Re-processing two or three times is especially excellent.

Chaihu Lian'er Wan (Bupleurum and Coptis Two-Ingredient Pill) (Formula see above)
Liuwei Dihuang Wan (Six-Ingredient Rehmannia Pill) (Formula see entry 'Fever Belonging to Yin and Yang')
Siwu Tang (Four-Substance Decoction) (Formula see entry 'Delayed Smallpox Eruption')
Shiquan Dabu Tang (Ten-Ingredient Great Tonifying Decoction) (Formula see entry 'Abdominal Distension and Shortness of Breath')
