Mr. Chen Wensu said: During the period when smallpox pustules have crusted and fallen off but have not yet fully healed, the functions of the five zang organs are not yet solid, the muscles and cou li (skin and interstices) remain deficient, and the blood and qi circulation is not yet stable. If suddenly attacked by wind-pathogen combing with the muscles and skin, the body fluids will become stagnant and sluggish, thus forming gan-erosion sores (ulcers caused by parasitic or toxic erosion). It is suitable to use Xionghuang San (Realgar Powder) and Mianjian San (Silkworm Cocoon Powder) for treatment. If the condition persists without healing, it will ulcerate and erode tendons and bones, eventually leading to death. I believe that Xionghuang San clears liver heat, kills parasites, and detoxifies; Mianjian San treats continuous oozing of pus and water. Both are excellent external formulas. For patients without residual internal toxins, this method is appropriate. However, if the cause is liver-spleen gan-fire flaring upward, or stomach fire moving internally due to consuming sweet and greasy foods, or heat accumulation in the hand-foot yangming meridians, or deficiency heat in the kidney meridian, the situations vary. The root cause is always insufficient yuan qi (original qi) and excess pathogenic qi, taking advantage of the deficiency to manifest. When pain and swelling occur, use Xianfang Huoming Yin (Immortal Formula for Activating Life Decoction) and Da Wuyi Tang (Large Trogopterus Dung Decoction). When gan-heat causes pathology, use Da Wuyi Tang and Siwei Fei'er Wan (Four-Ingredient Stomach-Strengthening Pills). For kidney meridian deficiency heat, use Dihuang Wan (Rehmannia Pill). One should apply flexibility according to the specific situation, differentiate based on the mutual overcoming relationship among the five zang organs, assess the mother's qi status during breastfeeding, and prioritize supporting stomach qi as the best approach.
A woman suffered from gan syndrome. After bathing in hot water, she developed fever like burning flames and rigid stiffness of the body. This was due to pathogenic heat leaking from the cou li invading internally due to deficiency. She took one dose of Shiquan Dabu Tang (Ten-Agent Great Supplement Decoction) and was immediately relieved. At the same time, others suffering from this condition did not use tonifying agents and even died.
A child, after recovering from smallpox, had whole-body exudation and itching. This was complicated by gan syndrome. He recovered after using Da Wuyi Tang and Chan Chu Wan (Toad Pill). Later, he developed thirst and mouth pain, which were resolved using Chan Chu Wan and Renzhongbai San (Human Tooth Ash Powder).
A child suffered from smallpox toxin eroding the cheeks. I reasoned that liver-spleen heat assisted the gan syndrome. The main treatment was Da Wuyi Tang and Da Lushui Wan (Large Aloe Pill), assisted by Wuwei Yigong San (Five-Ingredient Extraordinary Merit Powder). After more than a month, symptoms gradually improved. Finally, cured by using Wuwei Yigong San alone, assisted by Da Wuyi Tang.
A child suffered from smallpox toxin eroding and rotting the buttocks. Strict application of Xionghuang San worsened the condition. I diagnosed concurrent liver-spleen gan syndrome. Initially, two doses each of Da Wuyi Tang and Xianfang Huoming Yin were used. Then, Jiawei Lushui Wan (Nine-Ingredient Aloe Pill) was used as the main formula, assisted by Wuwei Yigong San. After more than a month, all symptoms gradually healed.
A child suffered from the aforementioned symptoms, with fever, thirst, and afternoon heat in both feet. I diagnosed congenital yin deficiency of the kidney meridian. The patient did not believe this and insisted on taking heat-clearing and detoxifying herbs, resulting in death.
A child had very dense smallpox eruptions. First, limbs suffered from toxic pus ulcers which healed. Later, oral gan spread to erode the gums. I initially used one dose each of Da Wuyi Tang and Xianfang Huoming Yin, then stabilized the condition with Chan Chu Wan and Renzhongbai San.
A child suffered from sunken smallpox toxin erosion. Application of Xionghuang San and ingestion of Jiawei Jiedu San (Modified Detoxification Powder) led to recovery.
A child suffered from facial lesions causing pain. Treatment with Xianfang Huoming Yin, combined with topical Xionghuang San and internal Da Wuyi Tang, resulted in recovery.

Grind into powder. Take 1 qian per dose, decocted with Danzhuye (Young Bamboo Leaf) water.
Baicao San (Rotten Grass Powder): Treats sores caused by scratching smallpox, with dripping pus and blood. Use rotten grass from roofs or old thatch from walls; older is better. If unavailable, wild grass is excellent. Grind into powder and apply. If the whole body is affected, sprinkle generously on mats for the child to sit or lie on. The sores will heal.
Danfen San (Cinnabar Powder): Treats smallpox toxin with dripping pus.
Qingfen (Calomel) Huangdan (Lead Oxide) (each 1 qian) Huanglianmo (Coptis Powder) (3 qian)
Grind evenly and apply to the affected area.
