Xue Ji (courtesy name Zhongyang) stated: "Guixiong" (Turtle Chest) is caused by lung heat leading to distension and fullness, with pathogenic factors attacking the chest and diaphragm; it may also result from the nursing mother consuming excessive amounts of "Five Pungent Spices" (Wuxin, referring to pungent vegetables like garlic, scallions, etc.), or from the infant retaining undigested milk. It should be treated with Guixiong Wan (Turtle Chest Pill), or formulas such as Songrui Dan (Pine Stamen Elixir) and Baihe Dan (Lily Bulb Elixir).
"Guibei" (Turtle Back) is caused by allowing infants to sit too early, resulting in external wind invading the spine and penetrating into the bone marrow. One method is to apply turtle urine to the intervertebral joints on the back. (The method for obtaining turtle urine: Place a turtle on a lotus leaf; wait until its eyes look around, then quickly shine a mirror upon it, causing the urine to flow out.) Another method involves moxibustion at Feishu (BL13, Lung Shu) acupoint (located 1.5 cun lateral to the lower border of the third thoracic vertebra), Xinshu (BL15, Heart Shu) acupoint (located 1.5 cun lateral to the lower border of the fifth thoracic vertebra), and Geshu (BL17, Diaphragm Shu) acupoint (located 1.5 cun lateral to the lower border of the seventh thoracic vertebra). Use moxa cones the size of a small bean, applying two to five moxibustion sessions (Zhuang) per point. This condition often arises because the infant's primordial Qi (Yuanqi) is not yet sufficient, and the interstices (Couli) are not tight, allowing wind pathogens to invade. Alternatively, it may be due to the accumulation of phlegm-fluids, where wind and heat attack each other. The treatment principle should primarily focus on regulating and tonifying the Spleen and Lungs, supplemented by clearing heat and resolving phlegm. If the cause is the nursing mother consuming rich and greasy foods (Gaoliang Houwei), Qingwei San (Stomach-Clearing Powder) should be administered to the mother, while the infant takes a small amount.
