Infants with swelling on the hard palate of the mouth, rising like reed husks in salt water, are named Xuan Chuang (Hanging Sore). Ancient methods primarily used a long silver needle wrapped in cotton, retaining the sharp point to puncture the lesion, draining green, yellow, and red fluids. After puncturing, the mouth was wiped with salt water, followed by applying a small amount of Yizi San (One-Character Powder) mixed with honey. The powder consists of Cinnabar (Zhusha), Borax (Pengsha), Borneol (Longnao), and Mirabilite (Puxiao), ground into fine powder together. It is applied to the inside of the mouth using a goose feather.
[Chen Shouzhen's Note] Xuan Chuang occurs on the upper palate, resembling a purple grape. The tongue is difficult to extend or retract, the mouth cannot close properly, nasal bleeding occurs, and there are intermittent chills and fever. Needle puncture may be omitted. Instead, internally administer Tongcao (Tetrapanacis Medulla), Rhizoma Coptidis (Huanglian), Radix Glycyrrhizae (Gancao), and Juncus effusus (Dengxin), decocted for oral administration. Externally, use equal parts of edible salt and calcined Alum (Ku Fan) roasted until red, ground into fine powder. Apply to the mouth with the tip of a toothpick three to five times until it subsides spontaneously.
When infants develop fleshy growths in the mouth with thread-like roots, protruding and preventing eating, causing excruciating pain upon manipulation, treat with Xu Yu Hui (Ash of Human Hair) dissolved in boiling water for ingestion. If it does not subside, administer a small amount of real Moschus (Shexiang), ground into powder and taken with water, which will resolve it spontaneously.
Saliva flowing from the corners of an infant's mouth is differentiated into Spleen Cold and Spleen Heat. For Spleen Cold, use Jiao Mu (Scorched Wood - likely referring to scorched Atractylodes or similar), Fructus Aurantii (Qingpi), Pinellia Preparata (Fa Xia), with Fresh Ginger (Shengjiang) as a guiding herb, decocted for administration. For Spleen Heat, use Raw Atractylodis Macrocephalae Rhizoma (Bai Zhu), Coicis Semen (Bian Dou), Talcum (Hua Shi), Dendrobii Caulis (Shi Hu), and Radix Glycyrrhizae (Gancao), with Juncus effusus (Deng Xin) as a guiding herb, decocted for administration. Do not misidentify cold versus heat conditions, lest the treatment be counterproductive.
Excessive Fire in the Heart Meridian of infants can cause the tongue to swell and fill the mouth within moments, blocking the throat, a critical condition. Ancient methods first used Salt Plum (Yan Mei) or Pinellia Ternata (Ban Xia) to rub the jaw joint. Once the mouth opens, place Alumen (Zao Fan) on a new tile, heat it with fire until red, let it cool, grind it into fine powder, and apply the medicine to the tongue. Alternatively, mixing Pulvis Pteridii (Pu Huang) with milk for application is also effective.
[Chen Shouzhen's Note] For sudden tongue swelling, use Meihua Bingpian (Plum Blossom Borneol) from the top, ground into fine powder and sprinkled on the tongue; the swelling should recede immediately. If swelling persists beyond the lips, apply this medicine externally. Alternatively, steep the tongue in a concentrated decoction of Rhizoma Coptidis (Chuan Lian) for immediate miraculous effect. However, if tongue swelling is accompanied by phlegm in the throat, it is Hou Feng (Throat Wind). Urgently prepare a mixture of Borneol (Bing Pian) 1 part, Natrii Sulfas (Huo Xiao) 3 parts, Chalcanthite (Dan Fan) 2 parts, Indigo Naturalis (Qing Dai) 2 parts, Bombyx Batryticatus (Jiang Can) 5 parts, and Borax (Peng Sha) 3 parts. Grind into powder, apply to the tongue, and insufflate into the throat for immediate cure.
