The child's tongue initially presents with a growth resembling a bean, gradually enlarging to the shape of a fungus (She Jun), accompanied by pain, redness, and ulceration, without other systemic diseases. The symptoms are lighter in the morning and heavier in the evening, caused by heat-toxin in the Heart and Spleen. Ancient methods use Cinnabar (Shā) and Human White (Rénzhōngbái), each 0.5 fen; moss from roof tiles (Wǎshàng Qīngtái), Selaginella uncinata (Wǎsōng), and soft chicken manure (Táng Jīfèn), each 1 qian. Two silver crucibles are prepared. The herbs are placed inside, sealed tightly, and then covered with another layer of salt mud. They are calcined over charcoal fire until red-hot, for the duration of three incense sticks. After cooling, the crucible is opened, and the contents are taken out. Plum Blossom Borneol (Méihuā Bīngpiàn) and Musk (Shèxiāng), each 1 fen, are added, and everything is ground into a fine powder. Before application, the blood-filled fungal growth is pricked with a porcelain needle, a small amount of powder is applied, and then covered with Polygonum multiflorum seed powder (Púhuáng). Internally, Erchen Tang (Two-Aged Decoction) is administered with additions of Coptis Rhizome (Huánglián), Scutellaria Root (Huángqín), and Mint (Bòhe), which is effective.
[True Commentary] She Jun (Fungus on Tongue) is a malignant condition. Initially, external treatment is used: 3 qian of Evodia Fruit (Wúzhūyú) is ground into powder, mixed with good vinegar, and applied to both soles of the feet, bound tightly with cloth, and changed every few hours; the effect is miraculous. Alternatively, spider silk is twisted into a thread, looped around the base of the fungal growth, and tightened gradually. When it becomes extremely painful, one must endure it for a moment until the growth falls off and bleeds. Then, Suotak (Baicaoshuang) is applied topically. Internally, Liuwei Dihuang Tang (Six-Ingredient Rehmannia Pill Decoction) with 3 qian of Sophora Flower (Huáihuā) decocted and taken orally is particularly effective.
When a child's tongue frequently flicks into the mouth, it is due to heat in the Heart and Spleen, resulting in dry lips, dry tongue, irritability, fever, and foul-smelling stool. Ancient methods primarily prescribe Xiehuang San (Drain Yellow Powder, containing Agastache Leaf (Huòxiāng Yè), stir-fried Gardenia Seed (Chǎo Zhīzi), Gypsum Fibrosum (Shígāo), Saposhnikovia Root (Fángfēng), Raw Licorice Root (Shēng Gāncǎo), with Centipede Grass (Dēngxīn) as an guide), followed by Xiexin Daochi Tang (Drain Heart and Guide Out Redness Decoction, containing Akebia Caulis (Mùtōng), Raw Rehmannia Root (Shēng Dì), Coptis Rhizome (Huánglián), Raw Licorice Root (Shēng Gāncǎo), with Centipede Grass (Dēngxīn) as an guide), decocted and taken orally.
[True Commentary] The symptom of Nongshe (Playing with Tongue) involves the tongue slightly protruding and retracting. The Classics state: Protruding tongue (Tu She) belongs to excess syndromes, while playing with tongue (Nong She) belongs to deficiency syndromes. It is caused by insufficient fluids in the Spleen and Stomach leading to deficient heat, or thirst with desire to drink water, without a red complexion. It should not be mistaken for excess heat. Treatment should solely employ Sijunzi Tang (Four Gentlemen Decoction) with added Costus Root (Mùxiāng), Agastache Leaf (Huòxiāng), and Kudzu Root (Gégēn). If this occurs after a long illness, it is difficult to treat. Furthermore, if the tongue extends and curls to both sides, trembling visibly, this is named Sheshé Feng (Snake Tongue Wind). Only a large piece of Realgar Essence (Xiónghuáng Jīng) should be applied topically several times to bring relief.
