Infants often suffer from cold diarrhea due to improper care during umbilical cord cutting, allowing wind-cold pathogens to invade and transmit to the large intestine. The symptoms include greenish-white stool, abdominal pain, and borborygmus (intestinal rumbling). Ancient methods primarily recommend using Heqi Yin (Harmonizing Qi Decoction) for warming and dispersing [ingredients: Atractylodis Rhizoma (Cangzhu), Perillae Folium (Zisu), Saposhnikoviae Radix (Fangfeng), Poria Cocos with Red Cortex (Chiling), Fermented Soybean (Douchi), Agastaches Herba (Huoxiang), Citri Reticulatae Pericarpium (Chenpi), Magnoliae Officinalis Cortex (Houpo), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao); reference: Zingiberis Rhizoma Recens (Shengjiang), Thaliae Herba (Dengxin)].
Subsequently, use Tiao Zhong Tang (Regulate Middle Decoction) for warming and tonifying [ingredients: Ginseng Radix (Renshen), Poria Cocos (Fuling), Agastaches Herba (Huoxiang), Atractylodis Macrocephalae Rhizoma Stir-fried (Chao Baizhu), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao), Arecae Semen Roasted (Wei Muxiang), Cyperi Rhizoma Processed (Zhi Xiangfu), Amomi Fructus Prepared (Suasharen); reference: Zingiberis Rhizoma Roasted (Wei Jiang)].
[Commentary by Chen Shouzhen] Since the infant's diarrhea is caused by cold at the navel, ancient formulas are generally unsuitable. If the infant is less than one month old, applying Nuanchi Gao (Navel-Warming Plaster) externally can cure it without internal medication. If the diarrhea is severe, simply use Chao Baizhu (Stir-fried Atractylodis Macrocephalae Rhizoma) 8 fen, Bai Fuling (White Poria) 2 qian, Chao Zhuling (Stir-fried Polyporus) 8 fen, Xuan Zexie (Xuan Alisma Rhizome) 8 fen; reference: Ganjiang (Dried Ginger) one slice.
Infant spleen-deficiency diarrhea is mostly due to impaired spleen transportation and transformation. Thus, diarrhea occurs after every meal, accompanied by abdominal fullness without thirst, shortness of spirit, sallow complexion, poor appetite, and emaciation. Ancient methods recommend Shen Ling Bai Zhu San (Ginseng, Poria, and Atractylodes Macrocephala Powder) (see "Spleen Granuloma" chapter for formula details).
[Commentary by Chen Shouzhen] Infant spleen-deficiency diarrhea must be closely monitored to prevent transformation into slow convulsion (Man Jing). The prescription consists of: Bian Dou Yi (Coat of Lablab Seed) 1.5 qian, Chao Baizhu (Stir-fried Atractylodis Macrocephalae Rhizoma) 1.5 qian, Jie Zhuling (Knot Polyporus) 1.5 qian, Chao Zexie (Stir-fried Alisma Rhizome) 1.5 qian, Jilin Renshen Xu (Jilin Ginseng Rootlets) 5 fen, Yun Fuling (Yunnan Poria) 2 qian, Chao Zhike (Stir-fried Aurantii Fructus Immaturus) 5 fen, Bai Qianshi (White Euryale Seed) 1 qian, Zhigancao (Prepared Licorice Root) 5 fen.
Infant sun diarrhea (diarrhea with undigested food) may result from spring wind-pathogen invasion causing clear qi to sink and spleen dysfunction, leading to the excretion of uncooked food. Ancient methods recommend Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) [ingredients: Ginseng Radix (Renshen), Astragali Radix Honey-processed (Zhi Huangqi), Angelicae Sinensis Radix Earth-stir-fried (Tuchao Danggui), Atractylodis Macrocephalae Rhizoma Earth-stir-fried (Tuchao Baizhu), Glycyrrhizae Radix et Rhizoma Preparata (Zhigancao), Citri Reticulatae Pericarpium (Chenpi), Cimicifugae Rhizoma Earth-stir-fried (Tuchao Shengma), Bupleuri Radix Vinegar-stir-fried (Chuchaichu); reference: Zingiberis Rhizoma Recens (Shengjiang), Jujubae Fructus (Dazao)].
If diarrhea persists for a long time with intestinal slippage and incontinence, ancient methods recommend Sishen Wan (Four Spirits Pill) [ingredients: Psoraleae Fructus (Buguzhi), Schisandrae Fructus (Wuweizi), Myristicae Semen (Roudoukou), Evodiae Fructus (Wuzhuyu), ground into powder, formed into pills with ginger juice and jujube paste]. Take 1 qian per dose, washed down with rice water.
[Commentary by Chen Shouzhen] For infant sun diarrhea, although tonifying spleen earth is necessary, selecting Buzhong Yiqi Tang is also inappropriate. Using Sishen San (Four Spirits Powder) to warm the intestines is particularly contraindicated. My prescription uses: Sheng Baizhu (Raw Atractylodis Macrocephalae Rhizoma) 1.5 qian, Chi Fuling (Red Poria) 1 qian, Bai Fuling (White Poria) 1 qian, Chao Dangshen (Stir-fried Codonopsis Radix) 1 qian, Lianzi Rou (Lotus Seed Flesh) 1 qian (remove core), Zhigancao (Prepared Licorice Root) 5 fen, Huai Shanyao (Chinese Yam) 1.5 qian, Bian Dou Rou (Lablab Seed Flesh) 1 qian. If caused by sinking of clear qi, add Shengma (Cimicifugae Rhizoma) 2 fen.
