Pediatric-specific diseases are numerous in variety. Speaking of the important ones, there are already categories such as Umbilical Wind (a neonatal tetanus-like condition), Erysipelas (an acute bacterial skin infection characterized by redness and swelling), Trismus (lockjaw), Bianzheng (physiological changes during infant growth and development), Weak Syndrome (conditions of physical weakness), and Gan Syndrome (chronic malnutrition syndrome due to spleen and stomach dysfunction). However, there are many other names and varying conditions beyond these. Since these are unique to infants and children, a separate section is dedicated to describing them, named "Miscellaneous Diseases." These include Umbilical Dampness, Fetal Startle, Fetal Wind, Fetal Jaundice, Fetal Toxin, Night Crying, Ke Wu (sudden shock from external stimuli causing mental disturbance), and Spitting Milk. Each item is described below in detail.
1. Umbilical Dampness
If care is not taken during umbilical cord cutting, or if clothing and diapers become damp, moisture can invade the umbilicus. In severe cases, pathogenic factors may spread, triggering Umbilical Wind; in mild cases, the pathology remains local, with swelling and persistent dampness. If prolonged, even mild cases can become critical, so treatment must not be delayed. Upon observing Umbilical Dampness, immediately mix equal parts of Alum (Ming Fan) and Dragon Bone (Long Gu), add a small amount of Musk (She Xiang), grind into a fine powder, and apply to the umbilicus. Check every two to three hours. If thick fluid flows out, absorb it with medicinal cotton, change the dressing, and reapply. Alternatively, use equal parts of Dried Ginger (Gan Jiang) and White Litharge Ash (Bai Shi Zhi Hui) mixed with Powdered Lead White (Hu Fen), applied frequently to the umbilicus. One may also use Zhang Huan's Umbilical Sealing Powder, or a mixture of Red Cotton Ash, Yellow Cow Manure Ash, Dragon Bone, Hair Ash, and Dry Carthamus Flower (Gan Yan Zhi) in equal parts, ground into powder and sprinkled on the umbilicus; if dry, apply oil. For early-stage Umbilical Dampness, the *Simple Prescriptions* (*Jian Yi Fang*) recommends burning coarse paper until charred but retaining its properties, then applying the ash to the umbilicus, which is most effective. The dressing should be changed frequently to prevent re-exposure to moisture for gradual recovery. If Umbilical Dampness progresses to red patches forming ulcers that ulcerate, it is called Umbilical Ulcer (Qi Chuang). The treatment is similar to Umbilical Dampness; the key is to keep the area clean to prevent spreading. On the ulcer, use one qian each of Hu Fen and Long Gu, double the amount of Coptis Rhizome (Chuan Lian), grind into fine powder, and apply frequently. If bleeding occurs from the umbilicus, use finely ground White Litharge (Bai Shi Zhi) or a mixture of Long Gu and Ming Fan, adding a little Blood Dragon Resin (Xue Jie) as a dry powder application, continuing until healed. Another condition is Umbilical Protrusion (Qi Tu), usually caused by excessive crying and straining without tight sealing of the umbilicus, leading to further protrusion upon crying. Friction from clothing causes injury; bind tightly with cloth to prevent exposure. For cases where wind-dampness enters internally, causing abdominal distension like blown air, slight noise upon palpitation, and startle-induced crying, internal treatment should use Rhinoceros Horn Decoction for Detoxification (Xi Jiao Xiao Du Yin) or White Peony Decoction (Bai Shao Yao Tang) added with Coix Seed (Yi Ren). Externally, use five qian of Oyster Shell (Mu Li), five qian of Rhubarb (Da Huang), and one qian of Mirabilite (Pu Xiao), ground into powder. Soak cleaned snails in water, take one to two qian of the liquid, and apply to the umbilicus; healing occurs when fluid exits via urination. This method is also applicable for infantile scrotal enlargement.
2. Fetal Startle
Infants born with immediate symptoms of high fever, vomiting, eyes rolling upward with clenched fists, lockjaw, body and back stiffness, vomiting saliva, convulsions, and startle crying, with an open fontanelle and sunken cheeks, suffer from Fetal Startle. The etiology stems from the mother’s alcohol consumption, sexual indulgence, anger, or fright during pregnancy, or external wind-pathogen invasion damaging fetal qi. Observing the color between the eyebrows: bright red and greenish indicates treatability; dark blue-black indicates incurability. Treatment should first dispel wind-pathogens, then resolve phlegm and regulate qi. Formulas include Blue Gold Pill (Qing Jin Wan) and Uncaria Decoction (Gou Teng Yin). For severe cases, Vermilion Pill (Yin Zhu Wan) is used to promote purgation. If seizures recur frequently, Tai Yi San should be taken after recovery for regulation. Any infant under 100 days experiencing convulsions: true cases dying after two or three episodes are fatal; frequent false cases are not severe. Test breath heat: hot breath indicates false startle, which is treatable. For startle with rising phlegm and loss of consciousness, use Platinum Powder (Bai Jin San). For incessant crying, use Wind-Dispersing Powder (Qu Feng San), and apply Top-Smearing Paste (Tu Ding Gao) to the fontanelle. Generally, infants have fragile zang-fu organs; heavy sedatives like Cinnabar (Zhu Sha) and Mercury Powder (Yin Fen), or toxic substances like Centipede (Wu Gong) and Scorpion Tail (Xie Wei) should be avoided. Use herbs that disperse wind, move stagnation, resolve phlegm, open the lungs, and unblock orifices gradually. Only if ineffective should harsher formulas be considered to avoid delay and blame (see Chapter 5).
3. Fetal Wind
Infants born with body heat and reddened skin, resembling scald burns, are diagnosed with Fetal Wind. Etiology involves maternal spleen-stomach accumulated heat or hereditary congenital diseases passed to the fetus. Treatment includes Qing Wei San and Xiao Yao San administered to the nursing mother, with small doses for the infant. If the skin is denuded but not red, apply rice flour; if denuded, hot, and red, apply gypsum powder. If sores appear on the forehead and feet, with lockjaw and clenched fists before one month of age, and limp waist, it implies both parents had syphilis, deeply infecting the fetus, manifesting immediately after birth; this is untreatable, though rare recoveries occur. Alternatively, muscle rot may persist for years, or bones and sinews may erode, leaving the child permanently disabled (see Chapter 1, Section 4, Paragraph 6).
4. Fetal Jaundice
Slight yellowing of the body after birth is a natural physiological change, initially harmless. If the entire body and face turn golden-yellow, accompanied by high fever, constipation, urine as yellow as gardenia juice, refusal to feed, and incessant crying, it results from damp-heat transmitted from the mother during pregnancy. Mother and infant should jointly consume Di Huang Tang; breastfeeding should be suspended, and the mother takes the medicine. For severe cases, Xi Jiao San is used. If the color is pale yellow with white, it indicates weak stomach qi; Bai Zhu San is appropriate. Slight yellowing requires no medication; if desired, San Huang Tang can clear stomach heat, and the jaundice will subside in a few days. For generalized redness, decoct equal parts of Rehmannia Root (Sheng Di Huang), Trichosanthes Root (Hua Fen), Licorice (Gan Cao), and Forsythia (Lian Qiao) orally. Externally, crush Floating Duckweed (Fu Ping), Isatis Leaf (Lan Ye), and aquatic moss, squeeze juice, mix with Mirabilite and earthy sand, and apply; redness will recede.
5. Fetal Toxin
Sores appearing on the head after birth, resembling dry tinea, oozing pus, or forming crusts, are termed Fetal Toxin. This arises from inherited fetal heat, maternal desire toxins, or dietary toxins. Sores starting near the ears/hairline and spreading to the face with itching indicate excess heat in the Liver and Gallbladder; treat with Chai Hu Qing Gan San. Sores starting at the eyebrows and spreading to limbs with oozing pus indicate excess heat in the Liver and Spleen; treat with Qing Wei San, Xiao Chai Hu Tang, or Li Xiao San. Sores on the forehead belong to Heart heat; treat with Lian Qiao Xin, Gardenia Kernel (Zhi Zi Ren), Coptis (Chuan Lian), and Bezoar (Niu Huang). Itchy, weeping sores on the head/face indicate Kidney deficiency heat; treat with Di Huang Wan and Jie Du San. External treatments include: calcined stone, Phellodendron (Huang Bo), and Talcum (Hua Shi) ground and mixed with tung oil; or purple sugarcane peel, Child’s Tea (Er Cha), Blood Dragon Resin (Xue Jie), and Borneol (Bing Pian) mixed with pig bile; or steamed egg yolks mixed with hair ash and cooked in iron until oil extracts, then mixed with Sophora Root (Ku Shen) powder. *Simple Prescriptions* suggests equal parts of Coptis, Phellodendron, and Scutellaria (Huang Qin); apply dry powder if wet, oil if dry.
6. Night Crying
Night crying often results from improper care by the nurse, disrupting day-night cycles. Pathological causes include Cold, Heat, Startle, and Stagnation. Crying due to Cold involves cold spleen qi, night-time yin excess causing abdominal pain, cyanotic face, cold hands, and curved posture; treat with roasted Astragalus (Zhi Huang Qi), Angelica Sinensis (Dang Gui), Licorice (Gan Cao), and Atractylodes Chinensis (Chi Mu Xiang) powder applied to nipples. Crying due to Heat involves heart fire, red face, warm hands, hot breath, supine posture, worsening with light; treat with Uncaria (Gou Teng), Poria (Fu Shen), Licorice, Rapeseed Stem (Deng Xin), Cinnabar (Chen Sha), and Akebia (Mu Tong) decoction. Crying due to Stagnation involves food retention pain, silent crying, straight voice, scant tears; treat with raw Malt (Sheng Mai Ya) and Hawthorn (Shan Zha) decoction. Crying due to Startle involves insufficient heart qi, restless spirit, loud crying with tears; treat startle or use Tian Wang Bu Xin Dan. A single remedy uses the lower half of Cicada Slough (Chan Tui) ground with mint tea. Generally, night crying combines startle and heat; sleep hygiene changes alongside medication are necessary for efficacy.
7. Ke Wu (External Shock)
Infants with weak shen (spirit) may react violently to unusual objects or strangers, causing vomiting of green/yellow/white fluids, abdominal pain, undigested food, changing facial colors, and seizure-like symptoms without eye deviation. Pulse is wiry, rapid, and tense. Severe cases involve abdominal pain, chest oppression, and near-death states; urgent treatment is required. Initially, check the uvula for black/red nodules; puncture or remove them, wipe blood with cotton-wrapped hairpin. General treatment uses Bei Ji Wan, An Shen Wan, or Zhang Huan’s Pi Xie Gao. Ke Wu with night crying involves fear, sleep startles, clinging to mother; treat first with Bai Jie San to release the exterior, then Su He Xiang Wan or Hu Po Bao Long Wan to expel evil and calm the heart. Ke Wu typically involves internal heat followed by external shock, or pre-existing weakness allowing invasion. Healthy infants rarely suffer serious harm from minor shocks. Contact with foul odors causing similar symptoms is called Zhong E (Poisoning by Evil Qi); treat with aromatic turbidity-resolving methods similar to Ke Wu.
8. Spitting Milk
Spitting milk refers to regurgitation without vomiting. Caused by overfeeding or movement, it is common. Frequent spitting requires treatment. Due to overfeeding/weak stomach: use Malt, Tangerine Peel (Chen Pi), Atractylodes (Bai Zhu), and Medicated Leaven (Shen Qu). Due to Heat: red face, slight constipation/dysuria; use Coptis, Poria, Tangerine Peel, and Bamboo Shavings (Zhu Ru). Due to Cold: cyanotic/pale face, watery stools; use Cardamom (Bai Dou Kou), Pinellia (Ban Xia), Ginger (Sheng Jiang), and Amomum (Sha Ren). Weak stomach qi with frequent spitting: drink Fire-Waiting Earth (Fu Long Gan) decoction for immediate cessation.
