Whenever a child presents with fever, floating pulse (Fú Mài, floating pulse), warm breath from the mouth, frequent yawning, lethargy and mental dullness, and spasms or convulsions of the hands and feet, this is caused by wind-pathogen invasion. It is appropriate to use Da Qing Gao (Great Blue Ointment) or Ren Shen Qiang Huo San (Ginseng and Notopterygium Powder) for treatment. If the spasms are accompanied by limb fatigue and weakness, and the breath is not hot, it belongs to deficiency syndrome. It is appropriate to use Yi Gong San (Benefit the Work Powder) to tonify the Spleen Qi, and Diao Teng Yin (Hook Vine Decoction) to pacify the Liver Wind, after which the spasms will naturally cease. For infants within one month of age, if they present with spasms and nasal congestion, it also belongs to wind-pathogen invasion. It is appropriate to take seven stalks of scallion white (Cong Bai) and one slice of ginger (Sheng Jiang), grind them finely into a paste, spread it on paper, place it in an adult's palm to heat it, and quickly apply it to the fontanelle (Xin Men). (Note: Medical texts state that the center of the scalp hair whorl on the vertex is Baihui (GV20), one cun anterior to Baihui is Qian Ding (GV21), and three cun anterior to Baihui is the fontanelle.) After a short while, the spasms will stop and the nasal congestion will clear. This method can also be used for ordinary children with wind-cold. Remove it after recovery. It is advisable to cover the area around it with a piece of cotton dyed with madder root (Mian Yan Zhi), secured with hot flour paste, to shield the child from the breath of adults, which is preferable.
Whenever a child eats excessively, damaging the Spleen and Stomach, resulting in frequent vomiting, excessive sleepiness, loss of appetite for milk and food, and sudden spasms, this is caused by food stagnation. It is appropriate to use Xiao Shi Wan (Digest Food Pill). If the food has been digested but the previous symptoms persist, or if signs of deficiency appear, this indicates that the Spleen Earth is injured and the Liver Wood is overacting upon it. It is appropriate to use Liu Jun Zi Tang (Six Gentlemen Decoction) added with Gou Teng (Uncaria Rhynchophylla) to strengthen the Spleen and pacify the Liver. Do not recklessly use digestive and purgative methods, lest the condition deteriorate.
Pediatric tidal fever with spasms refers to spasms caused by persistent tidal fever leading to blood deficiency and tendon tension. Tidal fever means fever occurring at specific times, subsiding when the time passes, and recurring at the same time the next day, like the tides of the sea and rivers, never missing its schedule. Tracing its origin, it is caused by heat inducing spasms, not by fright causing disease. Treating it rashly as absurd "fright wind" would be a major error. The main treatment principles revolve around the four categories: excess, deficiency, cold, and heat. If the cause lies in the Liver, the manifestations include high fever, upward deviation of the eyes, trembling of hands and feet, hot saliva in the mouth, and stiff neck. One should use Di Huang Wan (Rehmannia Pill) to tonify the Kidney and Xie Qing Wan (Drain Green Pill) to treat the Liver. If accompanied by thirst preferring cold drinks and constipation, it belongs to deficient heat in the Liver and Gallbladder meridians; use Chai Shao Shen Ling San. If further accompanied by spontaneous sweating or night sweats, it belongs to blood deficiency in the Liver and Gallbladder meridians; use Di Huang Wan. If there is drooling from the corners of the mouth, it belongs to Liver Wood overacting on Spleen Earth; use Liu Jun Zi Tang. If the cause lies in the Heart, the manifestations include palpitations, upward deviation of the eyes, redness of the sclera, tight jaw, increased salivation, thirst with desire to drink, and trembling limbs. One should use Dao Chi San (Direct Red Powder) to treat the Heart and Di Huang Wan to nourish the Liver. If thirsty and desiring warm drinks, with fatigue and refusal to nurse, it belongs to Spleen deficiency with hyperactive Liver Wood; use Liu Jun Zi Di Huang Wan. If the cause lies in the Lung, the manifestations include mild spasms but wheezing, slight squinting, body heat like fire, sleeping with eyes partially open, cold hands and feet, and pale yellow watery stool. One should use Yi Huang San to tonify the Spleen, Xie Qing Wan to treat the Liver, and Dao Chi San to treat the Heart. If the body has slight heat, it belongs to deficient heat in the Spleen and Lung; use Yi Gong San. If there is wheezing, diarrhea, anorexia, and cold extremities, it belongs to deficient cold in the Spleen and Lung; use Liu Jun Zi Tang added with Pao Jiang (Roasted Ginger) and Mu Xiang (Aucklandia Root). If chronic illness leads to Yuan Qi (Primordial Qi) deficiency, use Liu Jun Zi Di Huang Wan. If the cause lies in the Kidney, the manifestations include mild spasms but unstable sleep, slightly elevated body temperature, tightly fixed and deviated eyes, phlegm sound in the throat, silver-brown stool, indigestion of milk and food, excessive sleepiness, and unresponsiveness. One should use Yi Huang San to tonify the Spleen and Dao Chi San to treat the Heart. If there is vomiting, diarrhea, refusal to nurse, cold extremities, and excessive sleep, it belongs to Cold Water insulting Earth (Kidney Cold invading Spleen); use Yi Huang San. If ineffective, use Liu Jun Zi Tang added with Gan Jiang (Dried Ginger) and Rou Gui (Cinnamon Bark). Generally, the method for treating spasms should prioritize consolidating the Qi of the Spleen and Lung. Because when Earth is prosperous, Metal is generated, thereby restraining Liver Wood so it does not invade the Spleen, and the spasms will cease. Practitioners must examine this carefully and refer to the discussions on excess and deficiency.
