If heat from summerheat injury fails to resolve, and diarrhea precedes dysentery, this indicates that pathogenic factors in the fu organs have transmitted to the zang organs, resulting in heat accumulation and qi stagnation. It is appropriate to use Ziegu's Shaoyao Decoction (Peony Decoction), adding Semen Armeniacae Amarum (Xingren, 杏仁), Cortex Magnoliae Officinalis (Houpo, 厚朴), Fructus Crataegi (Shanzha, 山楂) and other herbs; if the stool turns red (bloody/purulent), it signifies disease progression, requiring the addition of Radix Angelicae Sinensis (Danggui, 当归), Semen Persicae (Taoren, 桃仁), and Sanguisorbae Radix (Diyu, 地榆) to clear qi-level heat and harmonize the blood.
This passage states that when exterior pathogens have been resolved, but diarrhea gradually transforms into dysentery, it means the pathogen has entered internally and transmitted from the fu to the zang organs, which is a most ominous sign. Therefore, Jiegu's Shaoyao Decoction is used: employing the bitter-cold nature of Radix Scutellariae (Huangqin, 黄芩) and Rhizoma Coptidis (Huanglian, 黄连) to clear damp-heat; the acrid-warm nature of Arecae Semen (Binglang, 槟榔) and Fructus Tsaoko (Muxiang, 木香) to unblock qi stagnation; combining with Radix Angelicae Sinensis (Danggui, 当归) and Radix Paeoniae Alba (Baishao, 白芍) to harmonize the blood; and using Fructus Crataegi (Shanzha, 山楂) and Cortex Magnoliae Officinalis (Houpo, 厚朴) to break up accumulations. If red dysentery occurs, indicating the pathology has shifted from qi to blood levels, Semen Persicae (Taoren, 桃fen) and Sanguisorbae Radix (Diyu, 地榆) are added to harmonize the ying level and stop bleeding. This is what is meant by "harmonizing the blood stops dysentery naturally, regulating the qi eliminates tenesmus."
If heat from summerheat injury fails to resolve, accompanied by chest/epigastric oppression, nausea/vomiting, loose stools, and a white tongue coating with a grayish tint, this indicates insufficient stomach yang and obstruction by turbid dampness. It is appropriate to use Fresh Ginger (Shengjiang, 生姜), Rhizoma Pinelliae Preparatum (Banxia, 半夏), Cortex Magnoliae Officinalis (Houpo, 厚朴), Herba Akebiae (Tongcao, 通草), and Liu Yi San (Six-One Powder) to unblock yang and drain turbidity; if heat is severe, add Radix Scutellariae (Huangqin, 黄芩) and Rhizoma Coptidis (Huanglian, 黄连) for bitter-cold heat clearing.
This passage explains that when middle jiao yang is not vigorous, summerheat and turbid pathogens knot internally, causing vomiting above and diarrhea below, with chest and epigastric fullness/oppression. The standard approach uses the acrid-warm properties of Fresh Ginger (Shengjiang, 生姜), Rhizoma Pinelliae Preparatum (Banxia, 半夏), and Cortex Magnoliae Officinalis (Houpo, 厚朴) to unblock yang, and the bland-draining properties of Herba Akebiae (Tongcao, 通草) and Liu Yi San (Six-One Powder) to drain turbidity. However, a white tongue coating with a grayish tint, along with fever and irritability, indicates latent heat pathogens within, requiring the addition of Radix Scutellariae (Huangqin, 黄芩) and Rhizoma Coptidis (Huanglian, 黄连) for bitter-cold heat draining, so that when pathogenic heat resolves and stomach yang recovers, turbid dampness is naturally dispelled.
If summerheat injury causes high body heat, yet with profuse sweating, thirst, yellow tongue coating, mental fatigue, panting, and a large but deficient pulse, this indicates qi deficiency complicated by summerheat. It is appropriate to use Qingshu Yiqi Tang (Clear Summerheat and Boost Qi Decoction) plus Prepared Gypsum (Shu Shigao, 熟石膏) and Fresh Lotus Leaf (Xian Heye, 鲜荷叶) to support righteous qi and expel pathogens.
Summerheat inevitably damages qi, hence profuse sweating; heat damages fluids, hence thirst. This presentation resembles Baihu Tang syndrome, but the tongue is not deeply red, the coating is slightly yellow, and the pulse appears flooding but is soft and powerless, indicating deficiency complicated by summerheat. It cannot be compared to heat transmitting to the Yangming channel, which presents with profuse sweating, thirst, red tongue, yellow coating, and forceful flooding pulse. Additionally, with mental fatigue, limb weariness, rebellious qi, and irritability, this aligns precisely with the *Neijing* saying: "Deficient pulse and hot body result from summerheat injury." Using Dongyuan's Qingshu Yiqi Tang (Clear Summerheat and Boost Qi Decoction) employs the strategy of using Radix Ginseng (Renshen, 人参), Rhizoma Atractylodis Macrocephalae (Baizhu, 白术), Radix Glycyrrhizae Preparata (Gancao, 甘草), and Cortex Magnoliae Officinalis (Houpo, 厚朴) to tonify the central earth; Radix Ophiopogonis (Maidong, 麦冬) and Fructus Schisandrae (Wuweizi, 五味子) to generate fluids; combined with Rhizoma Cimicifugae (Shengma, 升麻) and Radix Puerariae Lobatae (Ge Gen, 葛根) to raise yang; Radix Scutellariae (Huangqin, 黄芩) and Root Alismatis (Zexie, 泽泻) to drain dampness; Fermented Soybean Product (Shenqu, 神曲) and Pericarpium Citri Reticulatae (Chenpi, 陈皮) to strengthen spleen and regulate qi; Gypsum Fibrosum (Shigao, 石膏) and Cortex Phellodendri (Huangbai, 黄柏) to clear internal heat. This embodies the intent of ascending-descending, dispersing-tonifying, and simultaneously resolving pathogens from interior/exterior and upper/lower aspects. Do not arbitrarily increase or decrease ingredients due to the large number of herbs; if used appropriately, efficacy will be as immediate as a drumstick striking a drum.
