Luoli (Scrofula/Cervical Tuberculous Lymphadenitis), known as Wan Lou by ancient sages, is caused by Yin deficiency with hyperactive fire attacking the channels and vessels, combined with excessive consumption of spicy and roasted foods. Wind-phlegm and heat-toxins clash, forming stubborn nodules that stagnate and do not disperse. Over time, they ulcerate internally. Treatment should nourish Yin, harmonize the Liver, regulate the Spleen, relieve depression, resolve phlegm, and clear heat/dampness. Do not merely use purgative methods to damage the True Yuan (Original Qi). In infants born with this condition due to fetal toxins, it stems from maternal Liver and Gallbladder channel fire stagnation and Qi stagnation. Treatment should focus on soothing the Liver, nourishing the Kidney, and resolving toxins. Although Luoli originates in the Liver, its root lies in the Kidney; excess Liver fire is due to insufficient Kidney Yin. Thus, the root is in the Zang organs, while the branch manifests in the neck and axilla. If the lesion floats in the pulse and has not yet reached the muscles or formed pus/blood, it is easy to treat. If observing the eyes reveals red meridians penetrating the pupil: seeing one meridian indicates death within one year; one and a half meridians indicates death within half a year; two or three meridians follow the same pattern. If no red meridians are seen, treatment is possible. Initially, there may be only one nodule, which later multiplies into dozens of varying sizes around the neck, swelling and suppurating, becoming difficult to heal, hence named "Lou Xiang" (Leaking Neck). If located on the side of the chest or hypochondrium, it is called "Ma Dao" (Horse Knife). If the wound is deep and black, internally rotting, penetrating the throat, causing inability to eat, swollen eyes, protruding tongue, and yellow pus from ears, it is incurable. Additionally, unexplained chills and fever with tubercles like millet seeds on the body, head, and neck, along with hard, painless nodules in the chest, abdomen, back, etc., are called "Jie Feng Qi Zhong" (Wind-Qi Swelling). Determine if it is wind or fire, deficiency or excess, and treat accordingly.
Luoli is caused by heat accumulation and stagnation in the Hand-Shaoyang and Foot-Shaoyang meridians. These meridians have abundant Qi but scarce Blood, leading to hard, non-ulcerating tubercles that spread and connect. If it involves the Yangming meridian, abundant Qi and Blood lead to ulceration, commonly known as "Lan Luoli" (Rotting Scrofula). Shaoyang-originating cases must be treated at the root. Quack doctors often use toxic drugs to attack it blindly, akin to carrying firewood to save a fire, causing greater harm; those with weak Yuan Qi may become consumptive and weak.
Xiakucao (Prunella Vulgaris) can dissipate Qi stagnation and nourish blood vessels, reducing chills and fever. It can be used freely for deficiency patterns. For excess patterns, combine with herbs that move and scatter Qi. External moxibustion can also yield gradual results.
Furthermore, tubercles may appear on the neck, back, body, behind the ears, or on the vertex. If swollen but not red, painful, or suppurring, it is mostly due to phlegm deposition and Qi stagnation, involving Phlegm, Qi, and Heat. If untreated long-term, it becomes Luoli; early treatment is advised. Generally, this arises from emotional Qi stagnation or anger during meals. Women suffer from this most frequently. Treatment should open depression, smooth Qi, benefit the diaphragm, resolve phlegm, and clear the Lungs. However, there is rarely a case without an underlying foundation of Yin deficiency and fire hyperactivity resembling tuberculosis; thus, combining Yin tonification is essential.
Ying Liu (Goiter and Tumors): Ying attaches to the shoulder and neck; Liu forms with Qi condensation. Avoid rich flavors and refrain from arbitrary incision. Large, elongated, hard, and immobile nodules are called Shi Ying (Stone Goiter). Skin color unchanged is Rou Ying (Flesh Goiter). Visible vein knots are Jin Ying (Sinew Goiter). Interlaced red veins are Xue Ying (Blood Goiter). Nodules that wax and wane with sorrow/worry are Qi Ying (Qi Goiter). None of the five Yings should be arbitrarily incised, except Tai Ying (Fetal Goiter), which heals after rupture and removal of fatty matter.
One Prescription for Treating Luoli:
Haizao (Sargassum) (wash away sand/soil, dry), Kunbu (Kelp) (rub off soil, grind finely as above), Heshouwu (Polygonum Multiflorum) (pound in wooden mortar to powder), Zaojiaoci (Gleditsia Spine) (stir-fry until yellow), Gong Shetui (Male Snake Slough) (from trees/walls, use one strip; flat ground ones are female). Grind all five into fine powder and mix evenly. Roast meat from the cut area under the pig's neck until cooked, dip in the powder, and eat. After meals, lie inverted with the affected area facing down for about one hour (Fu Shi). Moxa-burn each nodule seven times (Zhuang), stopping when smoke sensation is felt in the mouth. Once pus is exhausted, peace returns. At the onset, moxa-bust Quchi (LI11), left for males, right for females.
