肠胃脂膏殆尽，气虚下陷，圊时后重脱肛，眼眶内陷，神烦，全不思食，延成襟口重症，慎防
汗喘虚脱之变，脉虚数近弦，舌苔光红，姑拟人参石莲饮为法，冀其转机，附方请正。
台参须 鲜佛手露 青蒿露（各一两，代水炖冲） 江枳壳 地榆炭 泽泻 红白扁豆花（各
十朵焙研分冲） 石莲肉 东白芍（东壁土炒） 煨木香（真川连拌） 车前草 真野术（陈壁土炒） 真陈
不臭阿胶（藕粉炒成珠） 抱木神（辰砂拌） 陈年糕片（绵包入煎）
罗左 脾肾双虚已成休息痢之候，由来半载之久，吸烟之体迁延非宜，所幸胃气尚苏
不致受困，脉双弦而濡，拟宗缪仲淳法，缪氏脾肾双补丸四两，每日清晨、午后空心青盐汤送下三钱。
如酒积者，葛花解酲汤主之。
费三和 酒客，中虚飧泄不已，补中益气汤主之。
胃苓汤法亦主之。
倪（二月） 阴虚湿火下注，肛门血痔，更衣见红，由来日久，即《内经》所谓阴络伤则血
内溢是也。脉象弦数，拟宗丹溪槐角法。

血不止加柿饼炭。
内外痔俱同法，漏管者，加象牙屑。
陈姓患儿，五岁，七月就诊。痢疾持续一个月，大便中赤白脓血混杂。查阅前医所开数方，多为攻伐之剂，导致肝脾营阴受损，肠胃脂膏（肠道润滑物质）几乎耗尽。气虚下陷，排便时肛门重坠并脱出，眼眶内陷，神情烦躁，完全不想进食，拖延而成噤口痢重症，需谨防出现汗出、气喘及虚脱等变证。脉象虚数近弦，舌苔光红无苔。暂拟人参石莲饮为法，希望病情能出现转机，附上方剂请指正。

罗左患者。脾肾双虚，已形成休息痢的证候，病程由来已有半年之久。吸烟体质迁延不愈本非适宜，所幸胃气尚存，不至于受到严重困阻。脉象双弦而濡，拟遵循缪仲淳之法，使用缪氏脾肾双补丸四两，每日清晨、午后空腹时，用青盐汤送服三钱。如果是酒积所致者，以葛花解酲汤为主治。
费三和患者。嗜酒之人，中焦虚弱，完谷不化的泄泻不止，以补中益气汤主治。
也可采用胃苓汤法治疗。
潘左患者（二月）。久痢导致脱肛，应当遵循李东垣补中益气的治法。
倪患者（二月）。阴虚湿火下注，导致肛门血痔，排便时见血，病程由来已久，这正是《内经》所说的“阴络伤则血内溢”。脉象弦数，拟遵循朱丹溪槐角丸法治疗。

Chen (five years old, July). Dysentery has persisted for one month, with red and white pus and blood mixed in the stool. Reviewing several prescriptions from previous physicians, they were all attacking and draining in nature, thereby injuring the liver and spleen Ying-Yin (nutritive yin), depleting the intestinal mucus (intestinal grease), causing Qi deficiency and sinking. During defecation, there is a sensation of heaviness in the anus and prolapse, sunken eye sockets, mental restlessness, complete loss of appetite, progressing to a severe condition of Jin Kou Li (dysentery with inability to eat). One must be cautious of complications such as sweating, panting, and collapse. The pulse is deficient, rapid, and slightly wiry; the tongue coating is bare and red. Tentatively prescribing Ren Shen Shi Lian Yin (Ginseng and Stone Lotus Decoction) as the method, hoping for a turning point in the condition. Attached is the formula for correction.
Taishen Xu (Codonopsis Rootlets), Xian Foshou Lu (Fresh Bergamot Dew), Qinghao Lu (Artemisia Annua Dew) (each one tael, decocted with water instead of regular water and taken by washing down); Jiang Zhike (Aucklandiae Fructus), Diyu Tan (Sanguisorbae Radix Carbonized), Zexie (Alismatis Rhizoma), Hong Bai Biandou Hua (Red and White Lablab Flower) (each ten flowers, roasted, ground, and taken by washing down separately); Shilian Rou (Nelumbinis Semen), Dong Baishao (Paeoniae Radix Alba stir-fried with earth from the east wall); Wei Muxiang (Costi Rhizoma stir-fried with real Chuanlian), Cheqian Cao (Plantaginis Herba), Zhen Yezhu (Atractylodis Macrocephalae Rhizoma stir-fried with earth from an old wall), Zhen Chen Bu Chou Ejiao (Donkey-hide Gelatin stir-fried with lotus root powder into beads), Baomushen (Polycri Poriae Sclerotium mixed with Cinnabar), Chen Niangao Pian (Aged Rice Cake Slices wrapped in cotton and decocted).
Luo Zuo. Spleen and Kidney dual deficiency has led to the presentation of Xiuxi Li (intermittent dysentery), persisting for half a year. For a smoker, prolongation is not advisable. Fortunately, Stomach Qi is still recovering and not severely constrained. The pulse is bilateral wiry and soggy. Following the method of Miu Zhongchun, use Miu Shi Pi Shen Shuang Bu Wan (Miu's Dual Tonification Pill for Spleen and Kidney), four taels total. Take three qian each morning at dawn and after noon on an empty stomach, delivered with Green Salt Water. If caused by alcohol accumulation, Ge Hua Jie Cheng Tang (Kudzu Flower Decoction to Resolve Intoxication) is the primary treatment.
Fei Sanhe. A wine drinker, with middle jiao deficiency and continuous Sun Xie (undigested food in stool). Treat with Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction).
The method of Wei Ling Tang (Stomach and Five-Organ Decoction) is also applicable.
Pan Zuo (February). Long-standing dysentery with rectal prolapse should follow Dong Yuan's (Li Dongyuan's) method of Bu Zhong Yi Qi (Tonifying the Middle and Augmenting Qi).
Certain patient. Rectal bleeding for three years, persistent and lingering. Injury to the Liver and Spleen Ying division. Follow the method of Gui Pi Tang (Restore the Spleen Decoction).
Ni (February). Yin deficiency with damp-heat fire pouring downward, causing anal hemorrhoids. Blood seen during defecation, longstanding. This corresponds to what Nei Jing (Inner Canon) states: "When Yin collaterals are injured, blood overflows internally." The pulse is wiry and rapid. Following Danxi's (Zhu Danxi's) Huai Jiao (Sophora Seed) method.
Chao Huaimi (Roasted Sophora Flower Bud), Hei Jingjie (Schizonepetae Spica charred), Danpi Chao Hei (Moutan Cortex stir-fried until black), Chi Ling (Poria with Red Core), Diyu Tan (Sanguisorbae Radix Carbonized), Nüzhenzi (Ligustri Lucidi Fructus), Jing Yinhua (Lonicerae Japonicae Flos stir-fried until scorched), Zexie (Alismatis Rhizoma), Chao Zhike (Aucklandiae Fructus stir-fried), Dong Baishao (Paeoniae Radix Alba stir-fried with earth from the east wall), Yiren (Coicis Semen), Cheqian Cao (Plantaginis Herba).
If bleeding does not stop, add Shibing Tan (Persimmon Cake Charcoal).
Internal and external hemorrhoids are treated with the same method. For fistula-in-ano, add Xiangya Xie (Tusk Powder).
