Yige Fanwei: Qi and Food Coagulate
The disease of Yige (Dysphagia and Obstruction) arises from excessive injury by the Seven Emotions and irregular diet. When food causes Qi rebellion, it cannot descend; when Qi is obstructed by food, it cannot circulate. Qi, food, and phlegm-saliva coagulate mutually. When lodged in the throat, it is called Yige (Dysphagia); when lodged in the chest and diaphragm, it is called Ge (Obstruction). It hinders eating and drinking, gradually leading to vomiting, which is the disease of Fanwei (Vomiting after Eating).
Danxi stated: "Qi transforms into accumulation; accumulation transforms into phlegm; phlegm carries blood stasis, thus forming a mass." If this condition is not treated early, it will become a difficult-to-cure disease. In the initial stage, use Wuge Kuanzhong San (Five-Ge Widening the Middle Powder); if long-standing, use Erchen Tang (Two-Aged Decoction) with modifications.
For Yige Fanwei, the general treatment uses Erchen Tang (Two-Aged Decoction) with Ginger Juice and Zhuli (Bamboo Sap) as the main base. For those with Qi deficiency who are fat and white-complexioned, add Renshen (Ginseng Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma). For those with Blood deficiency who are thin and weak, add Danggui (Angelicae Sinensis Radix), Shaoyao (Paeoniae Radix), Taoren (Persicae Semen), and Honghua (Carthami Flos). For heat and oppression in the chest, add Tu Chao Huangqin (Scutellariae Radix stir-fried with soil), Huanglian (Coptidis Rhizoma), Gualou (Trichosanthis Fructus), and Jiegeng (Platycodonis Radix), removing Banxia (Pinelliae Rhizoma). For stagnation due to the Seven Emotions, add Xiangfu (Cyperi Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), Muxiang (Aucklandiae Radix), Binglang (Arecae Semen), and Sharen (Amomi Fructus). For Spleen deficiency failing to transform and transport, add Renshen (Ginseng Radix), Shenqu (Massa Medicata Fermentata), and Maiya (Hordei Fructus Germinatus) to assist. For dry stool constipation, add a small amount of Jiu Zheng Dahuang (Rhei Radix et Rhizoma steamed with wine) and Taoren Paste to moisten. Generally, in Yige disease, blood fluids are dry and the stomach cavity is parched; pills are hard to take, so decocting into a paste is suitable.

Chuanji: There is Deficiency and Excess
Sudden onset of asthma/wheezing is called Excess; chronic onset is called Deficiency. A slippery pulse with warm limbs indicates easy treatment; a choppy pulse with cold limbs indicates difficult treatment. Originally, wheezing accompanied by phlegm sounds upon movement is Phlegm Wheezing. Wheezing that fluctuates, improves after eating, and worsens after eating is Fire Wheezing. Qi rising from below the navel straight up the clear pathways is Yin Deficiency Wheezing. Shortness of breath without phlegm sounds is Qi Deficiency Wheezing. Aversion to cold, fever, and wheezing with a floating-tight pulse is Wind-Cold Wheezing. Rolling sounds in the chest, palpitations, and wheezing indicate Water Retention in the Heart area. Generally, illness reaching the stage of wheezing is a bad omen and difficult to treat.
Empirical formula for treating wheezing generally uses Suzi Jiangqi Tang (Perilla Fruit Descending Qi Decoction) as the main base. Add Zhuli (Bamboo Sap) for Phlegm Wheezing; add Zhizi (Gardeniae Fructus) and Kuqing (Scutellariae Radix) for Fire Wheezing; add Zhimu (Anemarrhenae Rhizoma) and Huangbai (Phellodendri Cortex) and Zhuli (Bamboo Sap) for Yin Deficiency; add Renshen (Ginseng Radix) and Ejiao (Asini Corii Colla) for Qi Deficiency; add Zisuye (Perillae Folium), Mahuang (Ephedrae Herba), and Xingren (Armeniaca Semen Amarum) for Wind-Cold. For Water Wheezing, use Jiaomu (Zanthoxyli Semen) roasted and ground to powder, taken with Ginger Decoction, 2 qian. For Phlegm-Fire wheezing and cough, regular consumption of Yulu Shuang can remove the root cause.
----------------------------------------
① Yulu Shuang: Refers to the fruit of Lychee produced in Ya Men Mountain, Xinhui County, Guangdong Province. It has a white shell and red flesh, remaining on the tree through winter. Its function is to descend Lung Fire and stop cough.
Suzi Jiangqi Tang (Perilla Fruit Descending Qi Decoction)
Suzi Jiangqi Tang consists of Erchen Tang (Two-Aged Decoction), Jiegeng (Platycodonis Radix), Wuweizi (Schisandrae Fructus), and Sangbaipi (Mori Cortex); plus Gualou (Trichosanthis Fructus) and Zhike (Aurantii Fructus Immaturus), wheezing and cough will recover immediately.
Note: Regarding the deficiency and excess of wheezing, careful diagnosis is required. Cold limbs and fine pulse certainly belong to deficiency images. However, if the pulse has a slippery quality, the tongue has a thick coating, there is constipation, and much phlegm, it should not be treated as a deficiency syndrome. Often, using phlegm-resolving and lung-clearing agents assisted by Suhexiang Wan (Styrax Pill) will turn the pulse slippery and large, and hands/feet warm. If mistakenly considered deficiency and given Renshen (Ginseng Radix) or Ejiao (Asini Corii Colla), the phlegm will block more and Qi will wheeze more severely.
Jing Syndrome: There is Yin and Yang
Jing Syndrome involves tension in the neck and back, with the body arching backward like a bird spreading its wings, hence named Jing (Spasm). Caused by Cold, leading to anhidrosis and aversion to cold, it is called Gang Jing (Yang Spasm), belonging to Yang. Caused by Dampness, leading to sweating without aversion to cold, it is called Rou Jing (Yin Spasm), belonging to Yin. Both use Xiao Xuming Tang (Minor Continuation Life Decoction); but if sweating, remove Mahuang (Ephedrae Herba). Due to excessive sweating or severe blood loss, causing tendons to lack blood nourishment, leading to tendon tension and spasm of all joints, use Shiquan Dabu Tang (Perfect Ten Tonifying Decoction).
Xiao Xuming Tang (Minor Continuation Life Decoction) See page 19.
Shiquan Dabu Tang (Perfect Ten Tonifying Decoction) See page 6.

Wu Ji Liu Ju: Always Qi Stagnation with Phlegm and Blood Stasis
Accumulations (Ji) belong to the Five Zang Organs; Gatherings (Ju) belong to the Six Fu Organs. Accumulations are Yin and have fixed locations; Gatherings are Yang and have no constant shape. The Liver's accumulation is called Feiqi (Fat Qi). The Heart's accumulation is called Fuliang (Hidden Beam). The Spleen's accumulation is called Piqi (Pi Qi). The Lung's accumulation is called Xiben (Breath Burden). The Kidney's accumulation is called Bentun (Running Piglet), known in common parlance as Qi Masses. Danxi argued: "Masses are tangible objects; Qi cannot form shapes. They are all Phlegm, Food Accumulation, and Dead Blood." In the center is Phlegm Accumulation; on the right is Food Accumulation; on the left is Dead Blood. In women, masses in the abdomen are mostly Dead Blood. Those that cannot move are called Zheng (Fixed Mass); those that can move are called Jia (Mobile Mass). The general principle is to soften hardness with salty flavors, reduce firmness by scraping, primarily moving Qi and resolving Phlegm. Treat with Kuijian Tang (Softening Hardness Decoction) and Kuijian Wan (Softening Hardness Pill). Externally apply Hupo Gao (Amber Plaster) and Sansheng Gao (Three Sages Plaster).

Apply the medicine thickly. Warm before application.
Wu Lao Liu Ji: All Are Fire Scorches True Nature
Wu Lao (Five Labors) refers to labor injuries of the Five Zang Organs. Liu Ji (Six Extremes) refers to scorched skin, wasted flesh, flaccid tendons, heavy bones, dried fluids, and rapid pulse. Generally, people with consumptive weakness have these symptoms. All result from Uncontrolled Two Fires scorching True Nature, causing Qi, Blood, and Spirit to decline daily, eventually forming the Six Extremes.
In treating consumption, one must distinguish which Zang is deficient or excess, whether Qi or Blood is deficient, and whether Qi or Blood is hot. With true insight, treatment will not err. For Zang deficiency, the pulse of that Zang must be deficient and small. For Zang excess, the pulse of that Zang must be excess and large. For Qi deficiency, the face is pale and spiritless. For Blood deficiency, the face is dark and withered/thin. For Qi Heat, the face is red and shiny, voice distinct and clear, symptoms worse during day, pulse floating and rapid. For Blood Heat, the face is red and dull, voice distinct and turbid, symptoms worse at night, pulse deep and rapid. For both Qi and Blood Heat, symptoms are severe day and night, with shortness of breath and dried fluids. Distinguishing according to this leaves no misses.

Heart pain is actually Epigastric pain. Ancients said Heart Pain has nine types: 1. Worm, 2. Zhu (Infectious Disease), 3. Wind, 4. Palpitation, 5. Fluid, 6. Food, 7. Cold, 8. Heat, 9. Migratory Pain. I examined their words; although they have names, there are no annotations, so doctors' recognition and treatment are not accurate. Since I know them, dare I not instruct later learners?
Worm pain: Restlessness, spitting clear water, crab-claw patterns on the face. Use Baifan (Alumen), Xionghuang (Realgar), and Binglang (Arecae Semen) ground to powder, taken with water.
Zhu disease: No heart pain usually, suddenly painful, delirious speech, pulse fluctuating large and small. Use Yinhua (Lonicerae Flos) one liang, decocted in water and taken.
Wind pain: Due to sleeping exposed in summer, Wind pathogen enters the Spleen. Spleen pain connects to Heart, moving up and down, often with vomiting. Use Huoxiang Zhengqi San (Powder for Rectifying the Qi of Ague-Inducing Pathogens).
Palpitation pain: Pain is not severe, but feels like hidden pain in the chest. Due to Wind-Shock invading the Heart. Treat with Erchen Tang (Two-Aged Decoction) plus Fushen (Poria cum Radix Pini), Yuanzhi (Polygalae Radix), Huanglian (Coptidis Rhizoma), Zhishi (Aurantii Fructus Immaturus), and Danggui (Angelicae Sinensis Radix).
Fluid pain: Due to Phlegm-Fluid retained in the epigastrium, blocking Qi rebellion, causing pain. Dark circles under eyes like smoke stains, chest feels like ice-water. Severe cases use Guntan Wan (Rolling Phlegm Pill) to purge; mild cases use Daotan Tang (Leading Phlegm Decoction) plus Cangzhu (Atractylodis Rhizoma), Xiangfu (Cyperi Rhizoma), and Chuanxiong (Ligustici Chuanxiong Rhizoma).
Food pain: Chest fullness, belching with acid regurgitation, aversion to food smell. Use Pingwei San (Peace the Stomach Powder) plus Zhishi (Aurantii Fructus Immaturus), Shanzha (Crataegi Fructus), Laifuzi (Radish Seed), Shenqu (Massa Medicata Fermentata), and Maiya (Hordei Fructus Germinatus).
Cold pain: External Cold invading Stomach, severe pain, cold limbs, six pulses deep and slow. Pancong San (Scallion Twisting Powder) is main.
Heat pain: Accumulated Heat in Stomach, vexation, body heat, difficulty in urination and defecation. Erchen Tang (Two-Aged Decoction) plus Zhizi (Gardeniae Fructus), Huanglian (Coptidis Rhizoma), Chuanxiong (Ligustici Chuanxiong Rhizoma), and Xiangfu (Cyperi Rhizoma).
Migratory pain: Comes and goes, red face, thirst. Common in thin people, caused by Stomach Fire pain. Siwu Tang (Four Substances Decoction) plus Zhizi (Gardeniae Fructus), Xiangfu (Cyperi Rhizoma), and Chenshi (Citri Reticulatae Pericarpium).
