Dingxiang (Caryophylli Flos, five pieces), Shidi (Diospyri Fructus, five pieces), Gancao (Glycyrrhizae Radix et Rhizoma, processed with honey, 0.5 qian), Ganjiang (Zingiberis Rhizoma, 1 qian).
Grind into fine powder, take with boiling water; this is the best method.
("Ao" is identical to "Nao", interchangeable in ancient texts.)
Aonao (Oppressive restlessness): A feeling of depression and discomfort in the heart, more severe than ordinary vexation. It arises because exterior pathogens have not been fully resolved and, taking advantage of deficiency, have sunk inward, knotting and stagnating between the heart and chest.
Zhizi (Gardeniae Fructus, 3 qian), Xiangchi (Fermented Soybean, 5 qian).
Decoct in water and take orally. After ingestion, induce vomiting immediately by stimulating the throat. If Zhishi (Aurantii Fructus Immaturus) is added, it is named Zhishi Zhizi Chi Tang (Bitter Orange, Gardenia, and Fermented Soybean Decoction), used to treat the aforementioned syndrome as well as disorders caused by dietary accumulation.
Hunmao (Fainting and dizziness) with unclear spirit, commonly known as Hunkui (Confusion and stupor). The Classics state: "In cases of various deficiencies combined with cold pathogenic factors, Jue (cold extremities/reversal) occurs, accompanied by Yu Mao (depressive dizziness) and numbness; this is due to the counterflow of cold qi upward."
The treatment method should be warming and tonifying. Furthermore, in Yangming disease patterns, if there is difficulty in urination, alternating difficulty and ease in defecation, occasional slight fever, panting, and dizziness preventing lying down, this indicates the presence of dry stool; the treatment method should be purgation. Additionally, when Shaoyang transmission reaches the fifth or sixth day, gradual onset of coma and muteness occurs, with a facial appearance resembling intoxication, or speaking alone for one or two sentences during sleep. Water is swallowed if offered, but there is no desire to drink if not offered. If physicians fail to recognize this condition and administer Chengqi Tang (Gastrodia and Rhubarb Decoction variants), it would be an error. This is because they do not realize that the heat has transmitted to the Hand-Jueyin Pericardium Meridian. Food is swallowed because the pathogen is not in the Stomach. There is no desire to eat because of the confused spirit. Since the pathogenic heat is not in the Stomach but in the Pericardium, Daodi San (Guide Out Red Powder) combined with Huanglian Jiedu Tang (Coptis Decoction to Relieve Toxicity) should be used.
Mutong (Akebiae Caulis, 1.5 qian), Shengdi (Rehmanniae Radix, 3 qian), Chifuling (Poria with Red Host, 2 qian), Dengxin (Junci Medulla, 20 segments).
Decoct in water and take orally.
Qi rushing from the lower abdomen up to the heart causing pain, resembling the leaping of a river dolphin; this is caused by Yin-cold qi from the Lower Jiao rising upward. Jiangfu Tang (Ginger and Aconite Decoction) added with Rougui (Cinnamomi Cortex), Wuyu (Evodiae Fructus), and Fuling (Poria) should be used as the main treatment. Alternatively, using Juhe (Tangerine Seed), Xiaohui (Foeniculi Fructus), and Chuanlianzi (Meliae Semen) as adjuvant herbs yields particularly effective results.
When a patient's body is extremely hot yet desires to wrap themselves in clothes, the heat is on the skin (exterior/surface) while the cold is in the marrow (interior/deep); when the body is extremely cold yet does not desire to wrap themselves in clothes, the cold is on the skin (exterior/surface) while the heat is in the marrow (interior/deep).
I believe that body heat with a desire for warmth belongs to the category of Taiyang external contraction (Shaoyang/Exterior Cold damage); body coldness without a desire for warmth is a sign of internal stagnation of heat pathogens.
However, there are cases where desiring warmth indicates internal heat, which is "extreme fire resembling water"; and there are cases where not desiring warmth indicates interior cold, which is "extreme water resembling fire". This
