Diseases of the ear are caused by the failure of turbid Yin to descend. The ear is an organ of void and rush (referring to the clear and open nature of the auditory orifice); it requires the descent of Yin and the sinking of turbidity for sound to enter and pass through smoothly. If turbid Yin rebels upward, and Jia Wood (Shaoyang Gallbladder/Gallbladder Channel) fails to descend, the Ministerial Fire becomes stagnant and erupts, resulting in heat-induced swelling. Conflict between Wood pathogens causes pain; stagnation of Wood Qi leads to hearing impairment (deafness/heaviness). Over time, Qi and Blood become stagnant and obstructed, muscles rot and ulcerate, forming carbuncles and pus. However, the failure of turbid Yin to descend is fundamentally due to the dysfunction of the central Qi within Wu Earth (Stomach/Yangming Dry Earth). It is appropriate to regulate the middle Jiao Qi, causing turbidity to descend and clarity to ascend, so that ear diseases heal spontaneously. For instance, if there is heat and swelling with pain inside the ear, Chaihu Shaoyao Fuling Tang (Bupleurum, Peony, and Poria Decoction) should be primarily used. If yellow fluid flows from the ear, Lingze Shaoyao Tang (Poria, Alisma, and Peony Decoction) should be primarily used. If hearing gradually declines, Shenfu Wuwei Shaoyao Tang (Ginseng, Poria, Schisandra, and Peony Decoction) should be primarily used.

Diseases of the nose are caused by the lack of clarity in the Hand-Taiyin Lung Meridian. The Lung opens into the nose, which is the passage for Zong Qi (Gathering Qi) to enter and exit for respiration. It is necessary for Lung Qi to descend and for the breath to be clear, so that the nasal orifice remains patent and unobstructed. If Lung Qi rebels and fails to descend, Zong Qi becomes congested and obstructed, leading to symptoms such as nasal congestion and rhinorrhea. Generally, the Lung governs the skin and hair; if external Wind-Cold invades and closes the pores, Lung Qi cannot diffuse. This agitation impacts the nasal orifice, causing sneezing. Its steaming effect on the nasal orifice produces clear discharge. Over time, clear discharge transforms into turbid discharge, causing blockage and stickiness. Further prolonged stagnation turns white discharge into yellow discharge, which becomes foul-smelling and repulsive. If unresolved for a long time, with color and odor resembling pus, a nasal carbuncle disease forms. If the Middle Qi fails to circulate and Lung Metal becomes full and congested, even without external Wind-Cold invasion, but with frequent flow of turbid discharge, this is referred to as Bi Yuan (Nasal Sinusitis/Nasal Polyps). Investigating its root cause, it ultimately stems from Dampness in Earth and Rebellion of Stomach Qi, where turbid Qi fills the upper body, leaving no path for Lung Qi to descend. For instance, if Lung Qi is stagnant and rises, causing nasal congestion and excessive discharge, Jiegeng Yuanshen Tang (Platycodon and Scrophularia Decoction) should be primarily used. If Lung Heat causes nasal congestion with sticky, yellow turbid discharge, Wuwei Shigao Tang (Schisandra and Gypsum Decoction) should be primarily used. If the nostrils feel hot and develop sores, Huangqin Beimu Tang (Scutellaria and Fritillaria Decoction) should be primarily used. If nasal congestion causes heavy voice and unclear speech, Lingze Jiangsu Tang (Poria, Alisma, Ginger, and Perilla Decoction) should be primarily used.
